[Clinico-pathological observations in myocardial rupture].

In their material of 10 years authors describe 42 myocardial ruptures developed as complication of acute myocardial infarct. The rupture developed in the acute stage of infarct. Rupture of anterior wall infarct is the most frequent. The most decisive clinical factor in development of rupture is the loading in acute stage, from pathologic aspect the strongly infiltrated, transmural infarct is considered the most decisive pathologic factor. Diabetes mellitus and hypertonia can predispose to myocardial rupture.

Traumatic rupture of the pulmonary artery.

Traumatic rupture of the main pulmonary artery in a surviving patient is very rare. The case history of a 37-year-old man who sustained blunt chest trauma in a motor vehicle accident is presented. A persistent, bloody right pleural effusion led to the diagnosis of a ruptured right main pulmonary artery, which was surgically repaired using cardiopulmonary bypass. Points of diagnostic interest and therapeutic decision making are discussed.

[Traumatic rupture of the thoracic aorta].

A man aged 19 years sustained multiple injuries on account of deceleration. Radiography of the thorax revealed increase in width of the mediastinum. Aortography after 1 1/2 days revealed pseudocoarctation of the aorta corresponding to rupture of the aortic isthmus. This investigation should be undertaken on broad indications in patients with relevant injuries and increase in width of the mediastinum.

Tibialis posterior tendon synovitis and rupture.

The anatomy of the tibialis posterior muscle and tendon is described. It is the main stabilizer of the hind foot. Clinical and radiographic signs of tibialis posterior tendon rupture and the various operative techniques used, are reviewed. The Cobb method, using half the tibialis anterior tendon passed through a hole drilled in the navicular into the tarsal tunnel and anastomosed to the proximal end of the tibialis posterior, seems to give results superior to other methods.

Intraperitoneal rupture of amoebic liver abscess.

A retrospective analysis of 23 cases of amoebic liver abscess with intraperitoneal rupture is presented. Group I consisted of 16 patients with clinical features of generalized peritonitis. Eight of these patients were treated surgically, and four died. Group II consisted of seven patients with features of localized peritonitis and all were treated conservatively. In all cases the diagnosis of liver abscess was established by liver scan or ultrasound examination; peritonitis was established either at operation or, in the conservatively managed group, by aspiration of pus from the peritoneal cav…

Traumatic rupture of the thoracic aorta.

Between 1968 and 1981, 25 patients with acute traumatic rupture of the aorta were treated at the University of Virginia. Twenty-two of these patients (88%) had serious concomitant injuries. The aortic tear was just distal to the left subclavian artery in 19 patients (76%), and at other sites in six patients (24%). Lacerations were at multiple sites in three patients (12%), in the ascending aorta in one (4%), in the distal aortic arch in two (8%), and in the descending aorta well beyond the subclavian artery in six (24%). Two of the patients (8%) died of free rupture of the aorta before reachin…

Spontaneous rupture of the pulmonary artery.

A 45-year-old man presented with the sudden onset of diaphoresis, generalized weakness, and bilaterally achy arms. Physical examination reve … rtery walls suggested cystic medial necrosis. These findings are suggestive of an acute spontaneous rupture of the pulmonary artery, possibly secondary to cystic medial necrosis.

Delayed bladder rupture after augmentation enterocystoplasty.

Delayed bladder perforation with peritonitis following augmentation enterocystoplasty in children with spina bifida is a serious and potentially life-threatening complication. Our experience with 4 such cases is presented. All patients had spina bifida with a neuropathic bladder and they had undergone augmentation enterocystoplasty with a tubular colonic segment of large bowel as part of an undiversion procedure. All patients were being managed with intermittent self-catheterization. The interval from augmentation enterocystoplasty until presentation ranged from 6 months to 3 years. Diagnosis

Myocardial rupture following acute myocardial infarction.

Ten patients, eight males and two females, suffered myocardial rupture following acute myocardial infarction and required surgery. There were five ventricular septal ruptures, four papillary muscle ruptures and one free wall rupture. Ventricular septal rupture was suspected clinically by the appearance of a new systolic murmur, usually associated with a thrill at the left sternal border. A left to right shunt was confirmed by bedside oximetry using a Swan-Ganz catheter. The mean pulmonary to systemic flow ratio was 3.04:1. Following cardiac catheterization all patients underwent corrective sur…

Anal sphincter function after delivery rupture.

Questionnaires were sent to 63 women with anal sphincter rupture that occurred during vaginal delivery two to seven years earlier. Half of them had significant trouble, such as incontinence for gas, dyspareunia, and pain. We performed anal pressure profilometry in 14 of these women and in ten controls for comparison, and found a significant reduced strength in the external anal sphincters of the studied group. When anal sphincter rupture extended through the rectal mucosa, the internal sphincter strength was also reduced. The persisting signs and symptoms of anal dysfunction indicate that wome…

Early radiographic signs of tracheal rupture.

Early diagnosis and repair of tracheal rupture are necessary to prevent acute tension pneumothorax, airway obstruction, and chronic tracheal stenosis. Few reliable radiographic signs of tracheal rupture have been proposed. We diagnosed seven cases of tracheal rupture, two related to blunt trauma and five resulting from tracheal intubation. Early radiographic signs included orientation of the distal portion of the endotracheal tube to the right relative to the lumen of the trachea with an overdistended endotracheal balloon cuff, migration of the balloon toward the endotracheal tube tip, and pne…

[Arachidonic acid metabolism following aneurysm rupture].

Imbalance between the two arachidonic acid metabolites, prostacyclin (PGI2) and thromboxane A2 (TXA2), is thought to be at least in part responsible for the development of cerebral vasospasm following aneurysm rupture. In 12 patients with subarachnoid hemorrhage the pre- and postoperative serum and CSF levels of PGI2 and TXA2 were measured as a function of their stable hydrolysis products, 6-Keto-PGF1 alpha (PGI2) and thromboxane B2 (TXA2), with a highly specific radioimmunoassay. Serum levels of both metabolites were elevated in half of the patients, but no correlation to the clinical course …

Left atrial rupture in osteogenesis imperfecta.

A 60 year old man with features of osteogenesis imperfecta presented in biventricular failure with evidence of mitral regurgitation. He responded to conventional treatment but later collapsed and died at home. Necropsy showed a haemopericardium caused by a tear in the left atrium. There was also rupture of the mitral chordae tendineae.

[Tracheal rupture in closed cervicothoracic injuries].

Two cases are reported of tracheal fracture occurring during blunt cervico-thoracic trauma. The first case emphasized the impossibility of passing a tracheal tube below the fracture preoperatively, without the fibreoptic or rigid tracheoscope which has the added advantage of making an accurate diagnosis of the fracture. The clinical and paraclinical signs could only lead to a suspicion of the diagnosis, but not its confirmation. The second case showed the use of different ventilatory means during and after surgery (separate lung ventilation, jet ventilation, high frequency ventilation) in case

Blunt abdominal trauma causing jejunal rupture.

Two cases of jejunal rupture from blunt abdominal trauma are described. One followed a motorcycle accident, and the other followed a punch to the abdomen. Both patients initially had unremarkable abdominal examinations. After surgical repair of the ruptures, the patients recovered uneventfully. Jejunal rupture secondary to blunt abdominal trauma is not common, but its subdued clinical presentation and the nonspecific laboratory findings make diagnosis difficult. High morbidity and mortality are associated with this type of injury when diagnosis and treatment are delayed. Small bowel rupture sh…

Renal allograft rupture and its management.

Renal allograft rupture, although a rare complication, was encountered in 11 of our 137 cases of live kidney donor transplants. The commonest finding was an associated acute rejection. Our cases were mainly given conservative treatment with repair of the rupture and antirejection therapy. Eight of our kidneys were saved in this way, thus eight of the 11 patients survived. None of the deaths were related to the graft rupture or its effects.
