BACKGROUND: At the time of benign gynecological surgery, a prophylactic salpingo-oophorectomy or salpingectomy is increasingly being performed concurrently to reduce the risk of future ovarian and fallopian tube cancer. We herein describe a case of hereditary breast and ovarian cancer syndrome in which a hysterectomy and bilateral adnexectomy were performed with a preoperative diagnosis of benign tumor. A detailed pathological examination revealed occult fallopian tube cancer, and additional staging surgery provided an accurate pathology diagnosis.

We report the case of a patient addressed for abnormal uterine bleeding in the context of secondary infertility. Operative hysteroscopy under sedation showed multiple uterine adhesions (Asherman syndrome) and communication with the pelvic cavity suspecting uterine perforation. Ectopic pregnancies occur when a fertilized egg implants outside the uterus, usually in the fallopian tube. Twin ectopic pregnancies are rare and pose significant diagnostic and management challenges. This case report presents the clinical details and management of a unilateral twin ectopic pregnancy in a 31-year-old female patient. The purpose of this report is to highlight the complexities associated with the diagnosis and management of this uncommon condition. In this case, we performed the left salpingectomy. We confirmed pathologically and histologically in pregnancy in the same tube.

Neoplasms of the fallopian tube are extremely uncommon. In addition, the incidence of a mature teratoma of the fallopian tube with incomplete uterine mediastinum is extremely low. In the current case report, we present a case of mature cystic teratoma of the fallopian tube with incomplete uterine mediastinum in a 23-year-old female. This mass was noted on computed tomography (CT) scan and sonographic examination. The patient underwent left salpingectomy and uterine septum incision during surgery. One year later, the individual became pregnant.

Ectopic pregnancies have a negative impact on future fertility. Prompt diagnosis is paramount to preserve tubal function and reproductive potential. Expectant, medical, and surgical management of ectopic pregnancies have similar efficacy in properly selected patients. Medical management has emerged as a safe alternative to surgery and holds promise for preservation of future fertility. Laparoscopic salpingostomy or salpingectomy remains the preferred means of surgical removal of ectopic pregnancies. The most predictive factor of future fertility is the health of the contralateral tube.

Isolated fallopian tube torsion is an uncommon cause of acute abdomen in pregnancy. Patients present with lower quadrant abdominal pain, and some have nausea and vomiting. There is no pathognomic diagnostic sign, so most patients are operated when it is too late to save the tube by detorsion alone. Here we present a case of isolated tubal torsion in a term pregnancy managed by salpingectomy and cesarean section simultaneously. As far as we know this will be the 20th case of reported isolated tubal torsion in pregnancy.

Isolated torsion of fallopian tube is very uncommon during pregnancy. Predisposing factors for torsion are hydrosalpinx, prior tubal operation, pelvic congestion, ovarian and paraovarian masses and trauma. Although the most important clinical symptom is abdominal pain in lower quadrants, the diagnosis is usually established during the operation performed for acute abdomen and salpingectomy is almost always necessary. Two cases of torsion of fallopian tube during pregnancy, one with hydrosalpinx, the other with paratubal cyst are presented and symptoms and predisposing factors are discussed.

In an interesting experiment of nature, we observed the discordant onset of fetal cardiac activity between the two gestations of a heterotopic pregnancy. After initially detecting cardiac activity only in the tubal gestational sac, we later observed cardiac activity in the intrauterine sac 6 days after laparoscopic salpingectomy. This case illustrates the variable onset of cardiac activity. We advise expectant management of the intrauterine pregnancy in a heterotopic gestation when cardiac activity is not detected initially in the intrauterine sac.
