Diagnostic dilemmas in pediatric/adolescent dermatology: scaly scalp.

Scaly scalp is a common problem in the pediatric and adolescent population. The possible causes range from the commonly seen tinea capitis and seborrheic dermatitis to rare systemic diseases such as dermatomyositis and Letterer-Siwe disease. In all cases a thorough history and physical examination are important first steps to successful diagnosis and treatment.

Microsurgical replantation of avulsed scalp--two cases report.

Two female patients with scalp avulsion were treated successfully by microsurgical replantation. The first patient suffered an avulsion of the right half of scalp and which survived fully after replantation. The second patient suffered avulsion of the entire scalp as well as the forehead skin and nape of the neck. Almost all of the avulsed part survived except partial skin loss in the posterior neck and left occipital area. Only one temporal artery and one vein was anastomosed in each case. Near normal hair growth recurred in both cases.

Syringoma-like sweat duct proliferation in scalp alopecias.

We have reviewed a series of 585 scalp biopsies taken for histologic evaluation of hair loss and have looked for syringoma-like eccrine sweat duct proliferation. This feature was found in 6 specimens with scarring alopecia and one each with alopecia areata and female-pattern alopecia. Syringoma-like sweat duct proliferation appears to play no role in the etiology of hair loss.

Facial and scalp burns from hair activator products.

Review of 1,008 admissions over the past two years to the Detroit Medical Center Burn Unit revealed 227 patients with facial or scalp burns. Of these, 12 patients sustained burns directly as a result of accidentally igniting a hair activator product called "Jheri-Curl." Eleven patients sustained superficial partial-thickness burns and were adequately treated with Dermaide. One had deeper burns requiring split-thickness skin grafting.

Adriamycin alopecia prevented by cold air scalp cooling.

Preliminary studies are reported on the effectiveness of cold air scalp cooling to prevent alopecia in patients receiving Adriamycin. Cold air produced in a novel way using a vortex refrigeration tube was applied to the scalp for 15 min before and 30 min after the administration of Adriamycin and other cytotoxic agents. Sixteen of 26 patients had no hair loss, four had slight hair loss, and six required a wig. Two subgroups fared particularly well. Four of four patients treated with ABVD for Hodgkin's disease and nine of 13 treated with Adriamycin (40 mg/m2) and vincristine (2 mg) for breast c…

Congenital scalp defect with distal limb reduction anomalies.

Congenital scalp defects and distal limb reduction anomalies can occur as separate entities or in combination with other anomalies. They also occur together in an apparently autosomal dominant syndrome, an example of which is described in the present paper. The literature on the subject is reviewed.

Treatment of a Giant Scalp Arteriovenous Fistula in a Child.

OBJECTIVE: To evaluate the feasibility and safety of direct surgical excision for a giant scalp arteriovenous fistula (sAVF) in a child, and to provide a reference for optimizing treatment strategies for pediatric sAVF. METHODS: A retrospective analysis was conducted of a pediatric case of giant sAVF (Yokouchi type C/Yu grade 4). The lesion measured ~14 cm × 12 cm × 4 cm and involved the right frontal, temporal, and parietal regions. It was supplied by bilateral superficial temporal arteries and drained via the right superficial temporal vein. After a multidisciplinary team (MDT) assessment, …

Dissecting Cellulitis of the Scalp: Linking Pathogenesis to Therapy.

Dissecting cellulitis of the scalp (DCS) is a chronic, inflammatory follicular occlusion disorder characterized by painful nodules, abscesses, and sinus tracts that lead to scarring alopecia. The therapeutic goal is to limit disease progression and the extent of scarring. Although DCS is traditionally managed with systemic retinoids, antibiotics, and surgical interventions, therapeutic responses are variable and long-term remission remains challenging. Recent insights into the immunological overlap between DCS, hidradenitis suppurativa (HS), and other autoinflammatory follicular disorders have…

Case Report: Bimekizumab for dissecting cellulitis of the scalp.

Dissecting cellulitis of the scalp (DCS) is a cicatricial alopecia characterized by follicular occlusion that progresses to abscess formation and rupture, driven by an intense inflammatory cascade. Part of the follicular occlusion tetrad, DCS typically presents with painful nodules and interconnecting sinus tracts localized to the scalp. Treatment of this recalcitrant disease is difficult, and there is a need for efficacious and durable therapies that control inflammation and address the underlying pathophysiology of disease. Here is the first reported use of bimekizumab, a dual inhibitor of i…

Malassezia and Staphylococcus are associated with scalp seborrheic dermatitis.

Seborrheic dermatitis (SD) is a common skin condition affecting the scalp and other sebaceous-rich areas. Most recent studies have focused on the microbiota of individuals with SD and healthy controls, suggesting an association of microbial dysbiosis. The variations in the microbiota at lesional and non-lesional sites of SD patients and healthy controls remain unexplored. The present study aimed to characterize the microbiota in SD patients. We conducted a cross-sectional study to analyse microbial composition and diversity of fungi and bacteria on the lesional and non-lesional sites of SD pat…

A rare case of pigmented seborrhoeic keratosis of scalp.

Seborrheic keratosis (SK) is a prevalent hyperkeratotic dermatological condition characterized by benign proliferation of epidermal keratinocytes, typically occurring in the middle to advanced stages of life. While the trunk is the primary site for lesions, they can also manifest on the extremities, face, and scalp. Although SK is typically benign, there can be morphological overlap with malignant skin lesions, necessitating meticulous differentiation for an accurate diagnosis. This case report describes a 63-year-old male presenting with scalp swelling that was, initially suspected to be mali…

Primary mucoepidermoid carcinoma of scalp: a case report.

BACKGROUND: Mucoepidermoid carcinoma of the skin is an exceedingly rare neoplasm, with only a limited number of cases documented in the literature. Typically, mucoepidermoid carcinoma arises in the salivary glands, where it accounts for 30% of all malignant salivary gland tumors and generally presents as a low-grade malignancy with a low probability of metastasis. CASE PRESENTATION: We report a case of a 49-year-old female patient of African origin who presented to our tertiary center with a longstanding scalp lesion that had been present for approximately 10 years. Initially, the lesion was …

Primary cutaneous neuroendocrine tumour of the scalp with metastasis.

We report the case of a female patient in her 60s diagnosed with a Grade 2 cutaneous neuroendocrine tumour. The patient was referred after dermatological resection of a mass on her right frontal scalp. Pathologic examination at an outside centre initially misclassified the tumour as hidradenocarcinoma. Physical examination revealed a 3.5 cm ulcerative plaque at the scalp vertex. MRI revealed a prominent right pre-auricular lymph node and a 3.5 cm heterogeneous scalp mass suspicious for residual tumour. Both areas were fluorodeoxyglucose avid on positron emission tomography. On surgical re-rese…

Lipedematous scalp and lipedematous alopecia: a narrative review.

Lipedematous scalp (LS) and lipedematous alopecia (LA) are uncommon conditions with an unclear cause. Over the past century, about 115 cases have been documented in the literature. It is unknown if these disorders are indeed so uncommon or if cases that accept the illness as a structural alteration or are asymptomatic make the disease appear more uncommon than it is. Despite the fact that the majority of cases are seen in young and middle-aged people, the cases involving congenital, elderly, and children have also been reported. In the reported cases, female gender was observed to be dominant.…
