Published measurements for the scaphoid are scarce. The purpose of this study is to define anthropometric norms for the waist of the scaphoid to assist in optimizing bone graft quantity and implant use. Computed tomography images of the wrist were reviewed by three surgeons. Anthropometric data were gathered, including the scaphoid waist diameter in two dimensions and the scaphoid waist volume. Each study was measured twice, allowing for determination of inter- and intraobserver reliability. Forty-three studies were examined (23 female and 20 male). Average measurements of the scaphoid waist w

To evaluate the reproducibility of the interpretation of radiographs for the diagnosis of scaphoid fractures, 134 radiographs (60 consecutive patients) were blindly observed by a resident radiologist, consultant radiologist, resident trauma surgeon and consultant trauma surgeon. The results of radiographs were analysed using kappa (kappa) statistics and stratified according to the results of the corresponding bone scan. In 23 patients the bone scan was positive for fracture of the scaphoid. Irrespective of training and experience, the kappa of the comparison of the scores of the radiographs be

When left untreated, scaphoid fractures follow a downward spiral resulting in carpal collapse and arthritis. The purpose of intervention is to allow a cartilage-wrapped bone to heal and maintain a smooth articular surface. Proposed treatment options for scaphoid nonunions have varied success rates. Using the Mack-Lichtman classification for nonunions, a plan can be formulated for individual patients based on motion loss, degenerative changes, carpal collapse, and fracture instability manifested by bone loss. Stable nonunions may benefit from bone grafting and internal fixation. Unstable nonuni

This study describes a novel technique for the preoperative surgical planning for an osteoarticular medial femoral condyle (MFC) graft to replace the proximal pole of a scaphoid. In cases of proximal pole scaphoid nonunion or in Preiser disease, fragmentation of the articular surface can occur, leading to significant pain and disability. Osteoarticular MFC bone grafting can be used to treat these injuries by providing a vascularized osteoarticular surface. Using 3-dimensional imaging and printing we are able to precisely model the injured scaphoid, and also accurately plan the harvest of MFC o

The author deals with the specific problems involved in the diagnosis of scaphoid fractures and the mechanical causes responsible for non-union of the scaphoid bone because of movements of the hand joint and blood supply of the scaphoid bone. Reference is made to details of surgery according to Matti and Matti-Russe which the author considers to be particularly important while assuming that the reader is already familiar with the technique. Emphasis is also placed on the importance of sufficiently long and careful immobilisation and subsequent physiotherapy. Alternative methods of surgery in s…

An adequate management of scaphoid fractures requires fast and reliable diagnosis. In this, proper history taking and physical examination are essential. Routine scaphoid x-rays miss over 20% of all scaphoid fractures. Therefore, in patients with a clinically suspected scaphoid fracture that cannot be proven by scaphoid x-rays, further diagnostic investigation is indicated. Which supplemental diagnostic tool (bone scintigraphy, MRI, CT) is preferred remains unclear. A below-the-elbow cast without immobilisation of the thumb is an adequate treatment for stable fractures. Unstable fractures and

Seventy-one per cent (12 of 17) of scaphoid nonunions previously treated by other methods united when the semi-invasive technique of electrical stimulation was used. Patient acceptance of this electrode technique is high, and morbidity is lower than in patients treated by iliac bone grafting. The treatment of nonunion of the scaphoid by the semi-invasive electrical stimulation technique is a reasonable alternative to bone grafting and provides a salvage procedure when bone grafting or other therapeutic modalities have failed.

Congenital aplasia of the scaphoid is a rare abnormality arising when the distal row of carpal bones is dislocated radially and proximally, the capitate replaces the scaphoid, and the lunate is triangular instead of quadrilateral in shape. The scaphoid bone which makes a functional bridge between the proximal and the distal row is a stabilising element under normal anatomical conditions. If it is absent, the carpus structure is disturbed and its radial part is prone to perilunar dislocation, because stability is provided only by the ligaments. However, a perilunar dislocation associated with a…

The scaphoid is the largest and most significant bone in the proximal carpal row and is essential for wrist biomechanics and stability. Its intricate anatomy and unique blood supply render it vulnerable to overlooked fractures and postfracture complications, often making for an exceptionally challenging radiologic appraisal. Radiographic diagnosis of a scaphoid fracture is particularly difficult because of the bone's complex geometric structure. Its numerous sulci, apices, ridges, and facets can resemble fractures, which may lead to unnecessary immobilization. A significant portion of the scap

Scaphoid nonunion is common, but the exact pathophysiology of this complication is unclear. Explanations include lack of treatment, poor initial treatment, delay in diagnosis, synovial fluid dynamics, precarious vascularity, fracture displacement, and carpal instability. Currently, the diagnosis is best confirmed by classic changes on plain radiographs, instability testing, arthrography, and arthroscopy in selected cases. Nine carpal bones are not benign. The natural history of scaphoid nonunion is one of progressive arthritis. Attempts at obtaining bony healing are therefore recommended. In t

INTRODUCTION: Scaphoid fractures represent the most common carpal bone fractures. These fractures may be overlooked due to insufficient diagnostics and lead to non-union and disability. A standardized diagnostic procedure with X-ray examinations, CT scans and/or MRI is recommended. We report on a case where a scaphoid fracture was overlooked, probably because of an incorrect technique of the computed tomography.
CASE REPORT: A 42-year-old man was admitted to our emergency room after a fall on the out-stretched wrist. X-rays showed no evidence of a bony lesion. In the follow-up examination on

INTRODUCTION: Trapezoid fractures are uncommon in sports. We presented a rare case of a trapezoid fracture associated with a scaphoid fracture caused by punching a ball in a football goalkeeper.
CASE PRESENTATION: A 19-year-old male who played as a football goalkeeper visited our hospital with complaints of sustained pain from the right wrist to the hand after punching a ball. Scaphoid fracture was diagnosed on plain radiographs, whereas trapezoid fracture was overlooked. Computed tomography revealed a displaced trapezoid fracture associated with a scaphoid fracture. Both fractures were succe

BACKGROUND: Arthroscopic reconstruction of the scaphoid is a modern technique for the treatment of scaphoid non-unions. The principle is the arthroscopic resection and filling of the non-union site from the midcarpal joint space.
PROCEDURE: Through minimally invasive placement of the scaphoid screw, the critical blood supply of the scaphoid is respected, which achieves high healing rates between 90 and 100%. The use of a cancellous bone graft seems to allow for healing without carpal malalignment, even in the presence of a humpback deformity with extension malposition of the lunate, although

Sclerotic bone ends and failure of union have quite often resulted from scaphoid fractures due to relatively insufficient blood supply to the proximal fragment. Clearly manifest scaphoid pseudarthrosis can be repaired only by surgery. Transplantation of cancellous or corticocancellous bone in combination with freshening of the sclerotic bone ends are priorities on which emphasis is laid in such surgical intervention. 38 patients with scaphoid pseudoarthrosis received surgical treatment in the casualty ward of Linz, between 1976 and 1982. The shortest follow-up interval for 30 patients was twel

The rare case is reported of fractures concurrently of the bilateral proximal radius and the right proximal carpal row. Initial radiographs of the patient showed only fractures in the bilateral proximal radius and the right triquetral bone. In addition, the bone scan showed focal increased uptake in the right scaphoid bone and lunate bone as well, suggesting fractures. Fractures of the entire right proximal row were confirmed by computer tomography. The patient received functional treatment for the elbows and had a scaphoid plaster-of-Paris cast for the wrist. This resulted in a complete recov…

We describe a 9-year-old male who suffered a linear, non-displaced scaphoid fracture following a pedestrian motor vehicle accident. Initially, X-ray imaging failed to detect the fracture, but MRI confirmed the diagnosis, highlighting the challenges in identifying scaphoid fractures in pediatric patients due to the bone's cartilaginous nature in this age group. The patient was treated with immobilization using a Plaster of Paris cast, consistent with standard management for non-displaced fractures in children. He experienced pain improvement and achieved bone union after eight weeks, returning

Because of their low incidence, studies about carpal fractures are rare. The aim of the present study was to analyze epidemiology and treatment of fractured carpal bones. We retrospectively analyzed data of 178 patients admitted to our emergency room with carpal fractures over 6 years. More males than woman were injured. In 91%, a CT scan was performed. The most commonly affected bone was the triquetrum followed by the scaphoid. Almost all triquetral fractures were treated conservatively as opposed to perilunate dislocations that were all operated on. Half of all patients with scaphoid fractur

Scaphoid fracture is the most common carpal fracture, accounting for 50%-80% of all carpal fractures in the Youngers and manual workers. The nonunion rate of scaphoid fractures was approximately 10-15%. Scaphoid nonunion can lead to wrist deformity, wrist collapse, ischemic necrosis, and traumatic osteoarthritis resulting in the loss of wrist function and seriously influence the patients' lives. Achieving bony union is essential for the treatment of scaphoid nonunion. Although many surgical procedures including various forms of bone grafting have been developed to improve bony union, there is

The use of pulsed electromagnetic fields (PEMF) to stimulate bone growth has been recommended as an alternative to the surgical treatment of ununited scaphoid fractures, but has never been examined in acute fractures. We hypothesised that the use of PEMF in acute scaphoid fractures would accelerate the time to union by 30% in a randomised, double-blind, placebo-controlled, multicentre trial. A total of 53 patients in three different medical centres with a unilateral undisplaced acute scaphoid fracture were randomly assigned to receive either treatment with PEMF (n = 24) or a placebo (n = 29).
