Knowledge of Sickle Cell Disease, Awareness of Sickle Cell Trait Status and Its Impact on Relationships Among Students at a Historically Black College.

Sickle cell disease (SCD) primarily affects people of African American descent in the United States. Many individuals do not know their sickle cell trait (SCT) status or might not be aware of SCD. The purpose of this study was to assess SCD knowledge, awareness of SCT status, and its impact on relationships. A cross-sectional study was conducted from August 2018 to June 2019 among students at a Historically Black College and University. Logistic regression was utilized to determine if the student's SCD knowledge and SCT status had a significant impact on the student's choice of relationships. …

Loss of Bone in Sickle Cell Trait and Sickle Cell Disease Female Mice Is Associated With Reduced IGF-1 in Bone and Serum.

Characterization of the bone phenotype of 24-week-old female transgenic sickle cell disease (SCD), sickle cell trait (SCT) revealed significant reductions in bone mineral density and bone mineral content relative to control with a further significant decreased in SCD compared with SCT. By microcomputed tomography, femur middiaphyseal cortical area was significantly reduced in SCT and SCD. Cortical thickness was significantly decreased in SCD vs control. Diaphysis structural stiffness and strength were significantly reduced in SCT and SCD. Histomorphometry showed a significant increase in osteo…

Lower efficacy of transfusion of red blood cells from donors with sickle cell trait.

Sickle cell trait (SCT), the heterozygous state for the hemoglobin S (HbS) mutation, affects roughly 1 in 13 African American individuals and is common among blood donors recruited for antigen-matched transfusions in sickle cell disease (SCD). While individuals with SCT are typically asymptomatic, it is unclear whether red blood cells (RBCs) from SCT donors have impaired storage quality and transfusion efficacy. Here, we integrate multi-omics to characterize RBCs from donors SCT and evaluate their performance post-transfusion. We first profiled RBCs from 174 HbAS and 248 HbAA volunteers, ident…

Major Discordance in Hemoglobin A1c Measurement in Sickle Cell Trait: Effect of Analytical Methods.

Hemoglobin A1c (HbA1c) is widely used for diagnosing and monitoring diabetes mellitus and reflects long-term glycemic control. However, hemoglobin variants may interfere with certain assay methods, leading to misleading results. We report the case of a 54-year-old woman with sickle cell trait and long-standing diabetes who presented with markedly discordant HbA1c values obtained using two different methods. High-performance liquid chromatography (HPLC) showed a significantly elevated HbA1c level, whereas a fluorescence immunoassay yielded a substantially lower value, despite persistently eleva…

Not so benign: Life-threatening hematuria from renal papillary necrosis in sickle cell trait.

Renal papillary necrosis (RPN) is an uncommon but important cause of hematuria in patients with sickle cell trait. We report a 28-year-old female with sickle cell trait and beta thalassemia who developed recurrent, transfusion-dependent gross hematuria. Despite extensive imaging, endoscopic evaluation, and conservative management, bleeding persisted. Ureteroscopy demonstrated findings consistent with RPN. The patient required intensive multidisciplinary care and 21 units of packed red blood cells. Hematuria ultimately resolved following treatment with oral epsilon-aminocaproic acid. This case …

Microsurgical outcomes of free flaps in patients with sickle cell trait: A case series.

BACKGROUND: Sickle cell trait (SCT) is considered a benign carrier state. Though not associated with vaso-occlusive crises of sickle cell disease (SCD), physiological stress like hypoxia, hypothermia, and sympathetic activation experienced during surgery can lead to sickling in patients with SCT. Some reports suggest that perioperative sickling may lead to thrombosis or flap loss. In the context of microsurgery, SCT is assumed to pose less risk than SCD due to lower hemoglobin S (HbS) levels. This study aims to characterize microsurgical outcomes in patients with SCT. METHODS: We identified a…

Rare Cause of Acute Loss of Vision in a Patient With Sickle Cell Trait.

Sickle cell disease (SCD) is a prevalent inherited blood disorder with various ocular manifestations, including sickle cell retinopathy (SCR), characterized by retinal microcirculation impairment and ischemic complications. We present the case of a 21-year-old male with sickle cell trait who experienced a sudden, painless loss of vision in his left eye. Ophthalmologic examination revealed vitreous hemorrhage and neovascularization, indicating SCR. Initial treatment with hydroxyurea and exchange transfusions led to partial improvement. However, due to persistent vitreous hemorrhage, the patient…

Outcomes of Patients With Sickle Cell Disease and Trait After Congenital Heart Disease Surgery.

BACKGROUND: Sickle cell disease (SCD) is a hemoglobinopathy that can cause multiorgan dysfunction. This study assessed the perioperative outcomes of patients undergoing operations for congenital heart disease who had SCD or sickle cell trait (SCT). METHODS: We performed a retrospective review of patients with SCD or SCT who had records in The Society of Thoracic Surgeons Congenital Heart Surgery Database between 2014 and 2019. The primary outcome was operative mortality. Secondary outcomes included postoperative complications. One-to-one propensity score matching was performed between the SCD…

COVID-19 in individuals with sickle cell disease/trait compared with other Black individuals.

In the United States, COVID-19 has disproportionately affected Black persons. Sickle cell disease (SCD) and sickle cell trait (SCT) are genetic conditions that occur predominantly among Black individuals. It is unknown if individuals with SCD/SCT are at higher risk of severe COVID-19 illness compared with Black individuals who do not have SCD/SCT. The objective of our study was to compare COVID-19 outcomes, including the disease manifestations, hospitalization, and death, among individuals with SCD/SCT vs Black individuals who do not have SCD/SCT. We leveraged electronic health record data fro…

(Lack of) knowledge of mothers about sickle cell trait and disease: a qualitative study.

OBJECTIVE: to analyze the understanding of mothers about sickle cell disease and/or trait of the family from a diagnosed child. METHODS: this is a qualitative study, using a semi-structured interview with 23 mothers, at a sickle cell disease outpatient clinic of a public institution, from October to December 2017. Analysis was thematic. RESULTS: all participants had sickle cell trait as well as the parents of their children. Twenty children were diagnosed with sickle cell disease by Heel Prick Test, and three, after hospitalization due to the disease. Most did not know how to report the pres…

Impact of sickle cell trait on morbidity and mortality from SARS-CoV-2 infection.

The COVID-19 pandemic has highlighted racial health disparities within the United States. Although social determinants of health are the most likely drivers of this disparity, it is possible that genetic traits enriched in the black population like sickle cell trait (SCT) could worsen the morbidity and mortality of infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Patients admitted for SARS-CoV-2 infection who identified as black or African American were included in the study (n = 166). Blood remnants were tested for SCT, and clinical data were abstracted from the ch…

Coevality of Systemic Lupus Erythematosus With Sickle Cell Trait: A Not So Uncommon Entity.

The coexistence of systemic lupus erythematosus (SLE) with sickle cell trait is quite sparingly reported in literature. Here, we narrate the case of a 17-year-old girl from Eastern India with sickle cell trait who presented with acute lupus pneumonitis. The challenges to the final diagnosis of SLE with sickle cell trait were because of the often lesser degree of clinical suspicion at the outset. In this report, we discuss this not so uncommon combination of conditions and review related literature. This girl, who was a known case of sickle cell trait, presented with fever, cough, shortness of …

Sickle cell trait and risk of cognitive impairment in African-Americans: The REGARDS cohort.

BACKGROUND: Sickle cell anemia may be associated with cognitive dysfunction, and some complications of sickle cell anemia might affect those with sickle cell trait (SCT), so we hypothesized that SCT is a risk factor for cognitive impairment. METHODS: The Reasons for Geographic and Racial Differences in Stroke (REGARDS) study enrolled a national cohort of 30,239 white and black Americans from 2003 to 7, who are followed every 6 months. Baseline and annual global cognitive function testing used the Six-Item Screener (SIS), a validated instrument (scores range 0-6; ≤ 4 indicates cognitive impair…

Association of Sickle Cell Trait With Ischemic Stroke Among African Americans: A Meta-analysis.

IMPORTANCE: African Americans and individuals of African ancestry have a higher risk of stroke compared with non-Hispanic white individuals. Identifying the source of this disparity could provide an opportunity for clinical stroke risk stratification and more targeted therapy. Whether sickle cell trait (SCT) is an indicator of increased risk of ischemic stroke among African Americans is still unclear. OBJECTIVE: To examine whether SCT is associated with a higher risk of incident ischemic stroke among African Americans. DESIGN, SETTING, AND PARTICIPANTS: This meta-analysis assessed the associ…

Association Between Sickle Cell Trait With Selected Chronic Medical Conditions in U.S. Service Members.

INTRODUCTION: Sickle cell trait (SCT), the heterozygous carrier state for hemoglobin S, is present in an estimated 1.6% of all newborns and 7.3% in black individuals in the USA. SCT has long been considered a benign condition with anticipated normal life expectancy and no increased risk for chronic diseases. The medical literature is inconclusive on the potential association between SCT and chronic medical conditions (CMC) including chronic kidney disease, venous thromboembolism, and stroke. Studies addressing these questions are lacking particularly in non-Black young adults. MATERIALS AND M…

A Case of Autosplenectomy in Sickle Cell Trait Following an Exposure to High Altitude.

A 24-year-old man presented with acute abdominal pain upon ascent to moderate altitude (3500 m). An immediate evaluation revealed a splenic infarct, and he was evacuated to sea level. Upon recovery, he was sent back to 3500 m without detailed etiological evaluation, whereupon he experienced recurrent episodes of left-side subcostal pain. Imaging suggested autosplenectomy, and workup revealed a negative thrombophilia profile but was positive for sickle cell trait (SCT). Individuals with SCT can be asymptomatic until exposure to severe hypoxia, upon which they can manifest clinically as sickle c…
