Chordoma of skull base presenting as nasopharyngeal mass.

While the nasopharynx is most commonly regarded by the otolaryngologist as a primary site of neoplastic involvement, it is also an avenue of spread of base-of-the-skull tumors presenting as bulging nasopharyngeal masses. Chordoma is a relatively rare tumor of the skull base and sacrum thought to originate from embryonic remnants of the notochord. Chordomas arising from the skull base/clivus are typically locally aggressive with lytic bone destruction. The optimal treatment may be photon/proton radiotherapy alone or combined with a gross total resection, when feasible. We report a case of intra…

Spontaneous skull base meningoencephaloceles and cerebrospinal fluid fistulas.

Cerebrospinal fluid (CSF) fistulas are characterized by the egress of CSF from the intracranial cavity through an osteodural disruption between the subarachnoid space and a pneumatized structure within the skull base. Depending on the cause, CSF fistulas are classified as acquired or congenital, and acquired fistulas are further classified as traumatic, nontraumatic, or spontaneous. Spontaneous CSF fistulas are considered to result from a multifactorial process and have been postulated to represent a variant of idiopathic intracranial hypertension. However, an anatomic predisposition involving…

Use of osteogenesis distractors in cloverleaf skull reconstruction.

Cloverleaf skull deformity (Kleeblattschädel-Syndromen, trilobular skulls) results from synostosis of multiple cranial sutures. The number of sutures involved, the pathogenesis of the synostosis, and the associated anomalies and syndromes are variable. All forms of cloverleaf skull are associated with a high morbidity and mortality. Management of surviving infants requires multiple decompressive and reconstructive operative procedures. Maximal advancement of the forehead/brow at the initial surgery is usually not enough to correct the associated proptosis and a second brow advancement must be …

Intraventricular tension pneumocephalus after endoscopic skull base surgery.

BACKGROUND AND STUDY AIMS: Postoperative pneumocephalus is commonly observed after neurosurgical intracranial procedures and is usually of l … ent of an external ventricular drain prevented further deterioration. Endoscopic exploration of the skull base reconstruction revealed a one-way ball valve mechanism as the source of pneumocephalus. The skull base reconstruction was revised with autologous fascia lata and repositioning of the pedicled nasoseptal flap. CONCLUSION: Intraventricular tensi …

[Fractionated stereotactic irradiation of skull-base related tumours].

The prognosis of the treatment of brain tumours depends on two main factors: biological nature and localisation of the neoplasm. Requirement … adiation necrosis of the pathological tissue can be achieved. Management principles of treatment of skull-base related tumours are very similar due to high risks of functional morbidity evoked by surgical injury to the cranial nerves, brainstem structures, vessels of the Willis circle and those of the substantia perforata anterior and posterior, etc. Su …

Intradiploic ciliated epithelial inclusion cyst of the skull.

A 56-year-old woman presented with a cystic skull lesion in the right temporal bone detected after resection of breast cancer. She underwent resection of the skull tumor for pathological diagnosis and treatment. The tumor was covered with ciliated epithelium and there were no malignant findings. The pathological diagnosis was ciliated epithelial inclusion cyst. Intradiploic inclusion cysts of the skull presenting as a calvarial defect include epidermoid cysts and dermoid cysts, which are clinically difficult to differentiate. Ciliated epithelium lining an intradiploic inclusion cyst is very ra…

Nasopharyngeal carcinoma with skull base erosion cytologic findings.

Nasopharyngeal carcinoma (NPC) occurs more frequently in patients with south-east Asian racial backgrounds. This disease may spreads superiorly to the skull base and intracranium followed by skull base destruction. We report a 56 year-old man presented with headache and diplopia. Magnetic resonance imaging (MRI) revealed extension of destructive mass from ethmoid sinus to the parenchyma. Intraoperative touch cytology showed loose syncytial sheets of pleomorphic abnormal epithelial cells, dyskeratotic cells with abnormal chromatin clumping and irregular nuclear outlines, in a necrotic backgroun…

Solitary plasmacytoma of the skull: Two case reports.

Solitary plasmacytoma of the skull is rare and few cases have been reported in the English literature. Plasmacytoma of the skull has a wide spectrum of pathology, including a quite benign, solitary plasmacytoma (SPC), and an extremely malignant, multiple myeloma (MM) at the two ends of the spectrum. The prognosis for solitary plasmacytoma of the skull appears to be good when it can be diagnosed on strict criteria. The clinical features of solitary plasmacytoma of the skull are complex and not easily identified, resulting in a high misdiagnosis rate. A comprehensive examination and analysis whi…

Paraperesis: a rare complication after depressed skull fracture.

Depressed skull fracture is an inward buckling of the skull bones, often because of direct blow to a small surface area of the skull with a blunt object. Monoparesis is often among its clinical presentations, but midline depressed skull fracture presenting as motor weakness of both lower limbs (Paraperesis) has not yet been reported. We report the case of 55 year old male admitted to emergency department with alleged history of hit on head by a wooden rod. He had pain, mild swelling and a small cut over scalp without any symptoms & signs of neurological deficit. On day two of admission patient…

Endoscopic resection of an anterior skull-base Schwannoma.

Sinonasal Schwannomas represent less than 4% of all head and neck Schwannomas. These neural sheath tumors arise from the ophthalmic and maxi … rhinitis and chronic rhinosinusitis. We report a case involving a 51-year-old male with an anterior skull-base Schwannoma that was excised endoscopically. Clinical features, imaging characteristics, histopathology, and treatment of sinonasal Schwannomas are discussed.

Endoscopic skull base reconstruction after endoscopic endonasal approach.

Recent advances in technology and techniques in neurosurgery and otolaryngology have revolutionised skull base surgery with the introduction of endoscopic endonasal approaches. One of the greatest limitations of endoscopic endonasal skull base surgery is the repair of osteodural defects. Numerous reconstructive techniques have been developed that maximise endoscopic endonasal approach efficacy and minimise complications. The choice of technique for closure depends on different factors and is based on an individual approach in each patient. In particular, size, the localisation of the defect an…

Surgical treatment and results in growing skull fracture.

Growing skull fracture is a rare complication of skull fracture and remains almost undetected in the first few years of life. Here, we report a series of 11 patients with growing skull fracture treated at our institute over a period of five years and discuss their clinical features, radiological findings, and principles of management. Of the 11 patients, six were females and five males, with the age ranging between 9 months and 12 years (mean, 3 years). Progressive scalp swelling was the most common presenting feature. Other clinical features included generalised tonic clonic seizures, eyelid …

Endoscope-assisted microsurgical resection of skull base meningiomas.

This study aims to determine the value of endoscope assistance in the microsurgical resection of skull base meningiomas. Fourty-six patients harboring a skull base meningioma underwent an endoscope-assisted microsurgical resection. In 30 patients (65%), tumor parts which could not be visualized under the microscope were detected with the endoscope. In 26 patients (56%), these tumor remnants were removed under endoscopic view. Gross total resection was achieved in 35 patients (76%) and near-total resection in 11 (24%). There was no surgical mortality. The major complication was new cranial nerv…

Base-of-skull/infratemporal fossa shell fragment retrieval.

PURPOSE: Shell fragments and bullets are frequently observed to have settled in the skull base and infratemporal fossa (ITF) with the impact resulting in a new surgically complicated trauma. This compact region is one of the most difficult and dangerous locations for lodged objects. When called upon to retrieve these objects, the surgeon faces a complicated challenge. The complexity of the task depends on many factors, such as size, deformation, and surface hooks, as well as identification of the precise anatomic location. MATERIALS AND METHODS: New surgical approaches and successful methods …
