Obstructive Sleep Apnea and Cognitive Impairment: Implications for Surgical Outcomes.

The influence of coexisting obstructive sleep apnea (OSA) and cognitive impairment (CI) on perioperative outcomes is unclear, despite its potential clinical significance. We aimed to compare clinical outcomes between surgical patients with OSA or high risk OSA (OSA/HR-OSA) who have CI and those without CI. We assessed 655 surgical patients aged ≥ 65 years. All participants completed the STOP or STOP-Bang questionnaires to assess for risk of OSA and were classified as OSA/HR-OSA or No-OSA/LR-OSA. To assess for probable CI, participants completed the in-person or telephone Montreal Cognitive Ass

Social Vulnerability and Subclinical Cardiovascular Risk in Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is an established risk factor for cardiovascular disease (CVD). Multiple clinical factors, including OSA severity, obesity, smoking, and hypertension, mediate this risk. However, whether the association between OSA and CVD is influenced by non-clinical factors, specifically social determinants of health, remains unclear. We examined the association between neighborhood-level social determinants of health, indicated by the Social Vulnerability Index, and subclinical CVD risk in adults with untreated severe OSA. Racial and ethnic minority status (REMS) and socioecon

Characterizing deep inspiration dynamics across obstructive sleep apnea severity levels.

BACKGROUND: Obstructive sleep apnea (OSA) is characterized by recurrent upper airway obstructions during sleep. While the apnea-hypopnea index (AHI) is the standard for assessing OSA severity, it fails to capture physiological details in respiratory signals, particularly those related to airway responses to obstruction. This study investigates deep inspiration patterns following apneas and hypopneas across OSA severity levels to uncover airflow dynamics beyond what AHI reflects. METHODS: Using data from 202 participants in the Sleep Heart Health Study (SHHS), respiratory airflow signals were

Current Evidence on Cardiovascular Risks in Female Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) exhibits distinct features in women across the lifespan, largely driven by hormonally mediated changes in sleep and respiratory physiology. Women remain under-represented in research cohorts, and few studies are designed to investigate sex-specific differences. Because of atypical clinical presentations related to menstrual cycling and lower anthropometric indices, OSA in women is frequently underdiagnosed in clinical practice. Women with OSA have higher risks of cardiovascular disease, mortality, and health and welfare costs than men, with disparities evident bot

Advances in cardiovascular risk assessment indicators in obstructive sleep apnea.

Obstructive sleep apnea (OSA) is a highly prevalent sleep-related breathing disorder that is closely associated with cardiovascular morbidity and mortality. Although the apnea-hypopnea index (AHI) remains the conventional metric for diagnosing and grading OSA severity, it inadequately reflects the heterogeneous pathophysiological burden of the disease and has limited predictive value for cardiovascular outcomes. This review summarizes recent advances in cardiovascular risk assessment indicators in OSA, with a focus on mechanisms of OSA-related cardiovascular injury, limitations of traditional

Predictors for cure of obstructive sleep apnea in bariatric patients.

INTRODUCTION: Obstructive sleep apnea (OSA) is common among candidates for bariatric surgery and is often underdiagnosed. Given that obesity and OSA share comorbidities and mutually reinforce each other, it is important to identify factors that predict OSA resolution after bariatric surgery. However, studies on this topic are limited and offer contradictory results. The objective of the study was to identify predictors of OSA resolution one year after bariatric surgery. METHODS: A prospective study was conducted with bariatric surgery candidates aged 18-65. Patients underwent preoperative car

Prematurity and obstructive sleep apnea in children: The perfect storm.

As survival rates for premature infants improve, understanding the link between prematurity and obstructive sleep apnea (OSA) has become critical for enhancing outcomes in this high-risk population. Children born severely prematurely face a threefold increased risk of developing OSA due to anatomical and physiological factors, such as an abnormal upper airway, unstable ventilatory control, bronchopulmonary dysplasia, and increased susceptibility to respiratory infections. The multisystemic impact of prematurity also increases the likelihood of adverse OSA outcomes, such as neurodevelopmental d

Obstructive sleep apnea and the gut microbiota - mechanisms and opportunities.

PURPOSE OF REVIEW: The relationship between obstructive sleep apnea (OSA) and the gut microbiota is increasingly recognized, yet the involvement of specific microbial taxa and the direction of causality remain unclear. This review synthesizes current evidence linking gut dysbiosis with disordered sleep, with a focus on OSA and its associated complications. RECENT FINDINGS: Studies of alpha and beta microbial diversity in OSA patients, sampled at different sites, have had inconsistent results. Members of the genus Fusobacterium and family Lachnospiraceae are enriched in some studies of OSA and

Obstructive sleep apnea and associated mediators with left ventricular remodeling.

BACKGROUND: The association between obstructive sleep apnea (OSA) and subclinical cardiac structural changes remains conflicted. This study aimed to investigate the link between OSA and left ventricular remodeling (LVR) and to explore the mediating roles of body mass index (BMI) and blood pressure. METHODS: We retrospectively analyzed 518 patients with OSA. Regression models were used to assess the independent association of OSA with left ventricular hypertrophy (LVH). Mediation analysis was employed to quantify the proportion of the effect mediated by BMI and systolic blood pressure (SBP).

Morpho-clinical insights into moderate-to-severe obstructive sleep apnea.

INTRODUCTION: Obstructive sleep apnea syndrome results from recurrent upper airway obstruction during sleep. Moderate-to-severe is common in referred sleep clinics and poses significant health risks. We assessed whether a morphotype model, a clinical-complaints model, and a combined morpho-clinical model can predict MS-OSA. METHOD: In a prospective cohort of referred adults undergoing type I polysomnography (n = 925, 2018-2024), we recorded demographics/anthropometry and two novel dynamic metrics, the cricomental angle (CMA) and inspiratory Mallampati score (I-MS). Logistic regression models

REM-related obstructive sleep apnea: low AHI-high hypoxemia paradox.

PURPOSE: REM-related obstructive sleep apnea (OSA) presents a paradox where a low apnea-hypopnea index (AHI) may mask significant disease. We compared polysomnography, comorbidities, and oxygenation metrics across OSA phenotypes to determine if AHI-based severity classifications underestimate the nocturnal hypoxemia in REM-related OSA. METHODS: Retrospectively, 378 OSA patients were classified as REM-related (AHIREM ≥2× AHINREM; AHINREM <5·h⁻¹, n = 30), REM-aggravated (AHIREM ≥2× AHINREM; AHINREM ≥5·h⁻¹, n = 63), or stage-independent (n = 285). We analyzed sleep architecture, AHI values, oxyg

Clinical and prognostic characteristics of obstructive sleep apnea among smokers.

INTRODUCTION: Smoking is a major public challenge, with profound implications for individual and societal health, intertwining with many medical issues such as the on growing of obstructive sleep apnea (OSA) incidence with studies highlighting a potential association between these two entities, suggesting that smoking may contribute to the pathophysiology of OSA and the worsening of the severity degree of OSA symptoms. AIM: This study seeks to determine the clinical and prognostic characteristics of OSA among smokers and secondly to look for factors predictive of OSA severity. METHODS: A cro

Influence of Obstructive Sleep Apnea on Oxidative Stress in Pregnancy.

Obstructive sleep apnea (OSA) is common during pregnancy and linked to adverse outcomes. While oxidative stress is a proposed pathogenic mechanism, evidence in pregnant populations remains limited. This multicenter, prospective study evaluated oxidative stress through protein carbonyl levels in 171 pregnant women and 86 cord blood samples. Polysomnography (PSG) performed during pregnancy categorized participants with the apnea-hypopnea index (AHI) in OSA, rapid eye movement (REM) OSA, and supine OSA. Protein carbonyl levels were measured by the dinitrophenyl hydrazine (DNPH) method. No signifi

Obstructive Sleep Apnea and Cardiometabolic Disease: Obesity, Hypertension, and Diabetes.

Obstructive sleep apnea (OSA) is a highly prevalent sleep disorder, characterized by recurrent upper airway obstruction during sleep, resulting in intermittent hypoxia, increased sympathetic activation, and sleep deficiency. Over the past 2 decades, there has been a robust body of evidence to support a strong link between OSA and cardiometabolic diseases. Obesity is an important risk factor for OSA. Observational studies indicate that OSA is a strong risk factor for the development of hypertension and diabetes. Moreover, clinical and experimental studies support a causal role of OSA in hyperte
