A 55-year-old Japanese woman presented with a 2-year history of right-sided epiphora and mucoid discharge. Slit-lamp examination revealed the presence of an additional punctum in the medial canthus. Dacryoendoscopic examination was performed to assess the connectivity of the supernumerary punctum, confirming its direct communication with the upper lacrimal canaliculus. Fistulectomy was subsequently performed under local anesthesia with complete excision of the supernumerary punctum. Histopathological examination of the excised specimen revealed a fistula lined with stratified squamous epitheli

A 72-year-old man presented with a mass in the lower palpebral conjunctiva on the left side that recurred after biopsy at another clinic. On the first examination, a hard, yellow-white mass was located in the lower palpebral conjunctiva on the left side with some vessels running onto the tumor surface. A small piece of the tumor was removed for pathologic examination, and the results of the examination corresponded to folliculosebaceous cystic hamartoma. The patient noticed reduction in size of the residual tumor at 3 weeks postbiopsy. Slit-lamp examination revealed considerable regression of

Our patient was a 34 year-old male who presented with a painless conjunctival mass that had developed 3 months before his first visit. On performing slit-lamp biomicroscopy, a lobulated pink-yellowish solid mobile mass was observed on the nasal bulbar conjunctival surface of his left eye. The tumor was excised, and histopathologic examination of the tumor revealed a sebaceous adenoma. Systemic examination was normal. No recurrence was observed during the 24-month follow-up period. Sebaceous adenoma of the bulbar conjunctiva is an extremely rare benign tumor, which may be observed to be isolate

A 47-year-old man developed a painful right red eye for 72 hours with a 20/25 decreased visual acuity. He had no medical history. Slit-lamp examination revealed a painful nodular scleritis at the equator of the globe in the infero-temporal quadrant. There was a moderate intraocular inflammation in the anterior segment. Fundus examination revealed a grade 1 hyalitis and a focal retinitis with vasculitis and arterio-veinous occlusion toward the scleritis zone. Syphilis and HIV serology were positive and the scleritis resolved 5 days after a penicillin G medication. Syphilitic scleritis are relat

Exciting new discoveries into the pathophysiological mechanisms of immunologic and inflammatory ocular disease continue. The genetic locus for susceptibility to Behçet's disease and Sjögrens syndrome has been further characterized. Important guidelines for serial slit-lamp examinations for the early detection of iridocyclitis in juvenile rheumatoid arthritis is suggested. In ocular allergy, further investigation of the interaction between mast cell and eosinophil sheds more light on the pathophysiological mechanism of vernal keratoconjunctivitis and vernal corneal ulcers. Unusual manifestation

Anterior lenticonus is a rare condition, in which there is a conical or spherical protrusion of the anterior surface into the anterior chamber. This is demonstrated by this case report, which has not been previously described in the Black African or Zairian ophthalmological literature. The patient was 28 years old and complained of gradual deterioration of vision over several years. The visual acuity was 20/200. The slit lamp biomicroscope showed an anterior lenticonus with microopacities of the lens. On fundus examination, a fundus albipunctatus-like appearance with midperipheral retinal flec…

Retained cilia in the eye following injury are rare. There are reports of long term tolerance to cilia in the anterior chamber but no information about the effect on the corneal endothelium. A 31-year-old man was seen with a 19-year history of a retained eyelash embedded in a corneal scar and extending across the anterior chamber to the chamber angle. The follicle was absent. There was no cellular inflammatory reaction, and the eye otherwise appeared normal on slit-lamp examination with normal vision. However, there was the appearance of an encrusted deposit on the surface of the lash and a ma

After reviewing the definition, incidence and main etiologic factors of cystoid macular edema (CME) the authors analyze the structural and physiologic characteristics of the macular area, including the blood-retinal barriers, and the basic concepts of retinal edema. Personal observations on three different groups of diseases associated with CME, retinitis pigmentosa, pars planitis and aphakia, are presented. CME evaluation was made by slit-lamp microscopy, fluorescein angiography and vitreous fluorophotometry. In the situations examined, the main factors involved in CME formation appear to be

This article reports the clinical findings in a work force of 30 individuals who were exposed to silver nitrate and silver oxide. Six individuals had argyria and 20 had argyrosis (deposition of silver in the eye). Measurements of blood silver levels were included as part of the examination. The results of this examination generally support the benign nature of argyria, although the question of silver causing a decrement in kidney function and night vision is not settled. Periodic slit lamp examinations as well as monitoring of silver air concentrations are necessary to assure that engineering …

The authors have studied the vitreous in 150 aphakic eyes after uneventful operations. They were examined by the slit-lamp and the Goldmann three-mirror glass. Two types of lesions were revealed: (1) 42% showed rupture of the anterior hyaloid membrane; this is not considered dangerous. (2) 69.3% showed posterior detachment of the vitreous, a more serious lesion. It is often associated with retinal lesions such as haemorrhage, pigmentary anomalies, and retinal tears. In view of such lesions, one cannot consider the aphakic patient as a normal subject and aphakia as a physiological condition. Th

A 19-year-old girl suffering from the nephropathic form of cystinosis presented with the full clinical and biochemical picture of Fanconi's syndrome. Polyuria, polydipsia and photophobia associated with severe growth retardation and rickets were the main clinical features. The essential biochemical abnormalities consisted of hypophosphataemia, hypokalaemia and hypo-uricaemia together with a systemic acidosis and a reduced creatinine clearance rate. There was generalized amino-aciduria, glycosuria and bicarbonaturia. In addition, the fractional excretion of potassium, phosphate and uric acid wa

We present a compact, automated, stereo-optical-sectioning imaging system for clinical assessment of the anterior segment, using a digital micromirror device (DMD) as a spatial light modulator. The platform synchronizes two CMOS cameras with the DMD, which projects programmable slit beams to produce depth-resolved cross-sectional views of the anterior eye. By adjusting the micromirror activation time, the system can also extend the dynamic range of the acquired images, accommodating both dim (e.g., the cornea) and highly reflective regions (i.e., the iris). In a pilot study, the prototype inst

A male patient in his 40s visited our ophthalmology outpatient department with complaints of periocular swelling, redness and decreased vision in the right eye. Initial examinations revealed anterior chamber cell 4+, vitreous cell 2+ and fibrous proliferation with haemorrhage in the superior and temporal regions around the swollen optic disc with macular oedema. Despite initial treatment, the patient showed no improvement. Subsequent slit-lamp examination revealed a nematode in the anterior chamber, prompting surgical removal and identification as Gnathostoma spinigerum This case underscores t

This study aims to report a rare instance of corneal decompensation brought on by Coniochaeta hoffmannii fungus invasion of a bandage contact lens (BCL). A 71-year-old man with pseudophakic bullous keratopathy (PBK) had BCL treatment for four months to symptomatically reduce pain and itching in his right eye. However, the patient unexpectedly lost his vision. The slit-lamp examination revealed an edematous cornea; the extensive direct inspection raised suspicion of BCL. For morphological characterization, the BCL extracted was inoculated onto 5% sheep blood agar and PDA. By Sanger sequencing m

To report a case of toxic keratopathy secondary to the self-application of seawater eye drops. A 60-year-old male who presented with unexplained unilateral decrease in vision and corneal thinning. Best-corrected visual acuity was 20/400 OD, slit-lamp examination indicated diffuse corneal edema with central thinning, intact sensation, and no vascularization. Laboratory analysis of the eye drops in conjunction with clinical symptoms and findings was consistent with toxic keratopathy. Toxic keratopathy can masquerade as the other forms of keratopathy, and a thorough history taking and laboratory

The authors report a female case of systemic lupus erythematosus (SLE) that was diagnosed as Wilson's disease (WD) when neurological manifestations were evident three years after the first admission. The brain imaging studies showed bilateral symmetrical basal ganglia involvement, slit lamp examination revealed Kayser-Fleischer ring of the cornea, and 24-hour urinary copper and serum ceruloplasmin also confirmed the diagnosis. The patient also had hemolytic anemia and hypoparathyroidism, which are rare presenting features of WD. SLE may be associated with WD, and presence of neurological, beha…

Pseudoexfoliation syndrome (PES) is a common age-related disorder affecting 60-70 million people worldwide. Patients with PES have abnormal production and deposition of fibrillar material in the anterior chamber of the eye. These exfoliated fibrils, easily detected by ocular slit-lamp examination, have also been found to exist systematically in the skin, heart, lungs, liver and kidneys. Recently, myriad studies have associated PES with systemic conditions such as increased vascular risk, risk of dementia and inflammatory state. We review here the most current literature on the systemic implica
