OBJECTIVES: To evaluate the role of intratympanic gentamycin therapy (ITGM) in controlling vertigo in patients with refractory Meniere's who have recurrent or persistent vertigo after endolymphatic sac surgery.
STUDY DESIGN AND SETTING: A retrospective review of 491 patients seen in a university balance clinic between July 1999 and July 2000. Of these, 68 patients had Meniere's disease; 4 patients had recurrent vertigo after endolymphatic sac surgery and were followed for at least 1 year. These 4 patients received a series of ITGM injections. Outcome measures included control of vertigo and a

The treatment of patients with Ménière's disease that do not respond to conventional therapy becomes complicated, particularly when taking into account the issue of hearing damage as well as the control of vertigo. Treatment often involves the intratympanic administration of gentamicin, for which different protocols are used. Hence, it is important that we better understand how this treatment influences hearing, beyond mere audiometric assessments. The aim of this prospective study was to evaluate the effect of intratympanic gentamicin treatment for Ménière's disease on cochlear function, as a

Reading with Orthographic and Segmented Speech (ROSS) programs use talking computers to deal with deficits in word recognition and phonological awareness. With ROSS, children read stories on a computer screen. Whenever they encounter a word they find difficult, they can request assistance by targeting the word with a mouse. The program highlights the word in segments and then pronounces the segments in order. In previous studies, children improved in reading, but children with relatively lower initial phonological awareness (PA) gained less than the others. In order to maximize the benefits fr

RATIONALE: Serotonergic psychedelics are being studied as novel treatments for mental health disorders and as facilitators of improved well-being, mental function, and creativity. Recent studies have found mixed results concerning the effects of low doses of psychedelics ("microdosing") on these domains. However, microdosing is generally investigated using instruments designed to assess larger doses of psychedelics, which might lack sensitivity and specificity for this purpose.
OBJECTIVES: Determine whether unconstrained speech contains signatures capable of identifying the acute effects of p

BACKGROUND: Sensory processing abnormalities are common in schizophrenia (SZ) and impact everyday functions, such as speech perception in noisy environments. Auditory-based targeted cognitive training (TCT) is a "bottom up" cognitive remediation intervention designed to enhance the speed and accuracy of low-level auditory information processing. However, the effects of TCT on behavioral measures of central auditory processing (CAP) and the role of CAP function on verbal learning outcomes in SZ are unknown.
METHODS: SZ (n = 42) and healthy subjects (CTL; n = 18) underwent comprehensive clinica

INTRODUCTION: Profiling Elements of Prosody in Speech-Communication (PEPS-C) is a computerised test for assessing the abilities to understand and use speech prosody in communication. It has been used to obtain a profile of strengths and weaknesses in different prosodic forms and functions for different clinical populations (e.g., autism spectrum disorders, Down syndrome) and second language learners. The 2015 edition of PEPS-C incorporates four new subtests addressing the understanding and production of lexical stress and phrasal stress, and collapses four form subtests (Intonation/Short-Item

Abstract A 28-year-old woman with mitochondrial neurogastrointestinal encephalomyopathy (MNGIE syndrome) undergoing evaluation for multichannel cochlear implantation is described. The case history, diagnosis of mitochondrial disease, and assessment of the benefits of cochlear implantation are documented. The hearing level with cochlear implant and speech recognition were improved significantly for this patient. MNGIE syndrome is a rare congenital disorder of mitochondrial DNA (mt-DNA). It is crucial for the otolaryngologist to have awareness of MNGIE syndrome and other mitochondrial encephalom

BACKGROUND: Since the 1940s, measures of pure-tone sensitivity and speech recognition in quiet have been vital components of the audiologic evaluation. Although early investigators urged that speech recognition in noise also should be a component of the audiologic evaluation, only recently has this suggestion started to become a reality. This report focuses on the Words-in-Noise (WIN) Test, which evaluates word recognition in multitalker babble at seven signal-to-noise ratios and uses the 50% correct point (in dB SNR) calculated with the Spearman-Kärber equation as the primary metric. The WIN

Chronic infantile neurological cutaneous and articular (CINCA) syndrome is a severe auto-inflammatory disease, due to mutation of the CIAS1 gene. CINCA syndrome should be considered the most severe of a spectrum of three disorders all due to mutation of the CIAS1 gene. CINCA syndrome produces a triad of symptoms of neonatal onset: maculopapular urticarial rash, chronic meningitis, and chronic non-inflammatory arthropathy with recurrent fever. CINCA syndrome is also associated with sensory organ damage, especially progressive hearing loss and loss of vision. In this case report, we present the

INTRODUCTION: Central hearing difficulties are a feature of Alzheimer's disease (AD) but not well captured by standard speech perception tests.
METHODS: We developed a non-verbal dichotic listening test (NVDLT) based on everyday sounds, and compared this with a standard verbal dichotic listening test (VDLT) in 36 people with primary progressive aphasia (PPA), 18 with typical AD (tAD), 6 with right temporal-variant frontotemporal dementia (rtvFTD), and 29 cognitively-healthy controls. Daily-life hearing function was assessed using the modified Amsterdam Inventory for Auditory Disability and Ha

Both frequency selectivity and temporal resolution were examined in patients with various inner ear disorders. These included noise-induced hearing loss (n = 24), Menière's disease (n = 16), sudden deafness (n = 25), toxic inner ear damage (n = 14), presbyacusis (n = 38) and degenerative progressive inner ear hearing loss (n = 8). To facilitate quantitative comparison, various factors were introduced, namely, frequency resolution factor (FRF), temporal resolution factor (TRF) and a combined resolution factor (FTRF). The FRF of normal-hearing subjects in background noise conditions was found to

With increasing use of imaging in the investigation of cerebello-pontine angle (CPA) tumours, the role of audio-vestibular and electrophysiological testing has changed. Field performance data on the efficiencies of these tests to screen for CPA tumours are lacking, but must be known to choose an appropriate testing strategy. A prospective observational study of 237 patients attending a neuro-otology clinic for audio-vestibular investigation was carried out. The aim was to provide field performance data on which to base an effective protocol to screen for CPA tumours. All patients presenting at

Recent sound coding strategies for cochlear implants (CI) have focused on the transmission of temporal fine structure to the CI recipient. To date, knowledge about the effects of fine structure coding in electrical hearing is poorly charactarized. The aim of this study was to examine whether the presence of temporal fine structure coding affects how the CI recipient perceives sound. This was done by comparing two sound coding strategies with different temporal fine structure coverage in a longitudinal cross-over setting. The more recent FS4 coding strategy provides fine structure coding on typ

Introduction: Voice-based digital biomarkers have emerged as a promising approach for distinguishing individuals with neurodegenerative disorders, particularly Parkinson's disease (PD), from healthy subjects (HS). With the increasing availability of smartphone and web-based recording tools, voice data can be collected efficiently in both clinical and remote settings. However, further validation is required before such approaches can be translated into routine clinical practice. Methods: This study used a cross-sectional analysis at the recording level, treating repeated recordings from the sam
