Top-down and bottom-up processes in speech comprehension.

Speech comprehension includes both bottom-up and top-down processes, and imaging studies have isolated a frontal-temporal network of brain areas active during speech perception. However, the precise role of the various areas in this network during normal speech comprehension is not yet fully understood. In the present fMRI study, the signal-to-noise ratio (SNR) of spoken sentences was varied in 144 steps, and speech intelligibility was measured independently in order to study in detail its effect on the activation of brain areas involved in speech perception. Relative to noise alone, intelligi…

Alleviating speech and deglutition: Role of a prosthodontist in multidisciplinary management of velopharyngeal insufficiency.

Surgical resection of soft palate due to cancer affects the effective functioning of the velopharyngeal mechanism (speech and deglutition). With the loss of speech intelligibility, hyper resonance in voice and impaired function of swallowing (due to nasal regurgitation), there is a depreciation in the quality of life of such an individual. In a multidisciplinary setup, the role of a prosthodontist has been described to rehabilitate such patients by fabrication of speech aid prosthesis. The design and method of fabrication of the prosthesis are simple and easy to perform. The use of prosthesis,…

Rehabilitation of speech and swallowing after burns reconstructive surgery of the lips and nose.

Rehabilitation of speech and swallowing postburns reconstructive surgery has not been previously described in detail in the literature. Seve … ion aimed to 1) facilitate safe transition from nonoral to oral intake, 2) improve articulation and speech intelligibility, and 3) minimize oral contracture development. At 6 months postinjury, the patient can safely tolerate a soft diet, demonstrate speech clarity at preinjury level, and has recovered functional oral range of movement. Rehabilitation of speech and swal …

The use of electropalatography (EPG) in the assessment and treatment of motor speech disorders in children with Down's syndrome: evidence from two case studies.

BACKGROUND: Many children experience significant difficulties in developing key aspects of speech. For some, these communication difficulties are compounded by co-occurring intellectual disabilities. METHOD: This paper presents two case studies from a larger on-going longitudinal study of the effectiveness of using electropalatography (EPG) to address the intelligibility problems experienced by many children and young people with Down's syndrome (DS). EPG, an innovative computer-based tool for assessing and treating speech motor difficulties, enables the speaker to 'see' the placement of his

Linkage of speech sound disorder to reading disability loci.

BACKGROUND: Speech sound disorder (SSD) is a common childhood disorder characterized by developmentally inappropriate errors in speech production that greatly reduce intelligibility. SSD has been found to be associated with later reading disability (RD), and there is also evidence for both a cognitive and etiological overlap between the two disorders. The present study tested whether SSD is linked to replicated risk loci for RD. METHOD: One hundred and eleven probands with SSD and their 76 siblings were tested with measures of speech, phonological memory (Nonword Repetition-NWR), and phonolog

Auditory localization, detection of spatial separateness, and speech hearing in noise by hearing impaired listeners.

In two groups, one with sensorineural and the other with conductive-mixed hearing loss, measures were made of single-source localization and speech intelligibility in both spatially separate and nonseparate noise. There was also a test for detecting when two sounds came from the same location or from separate ones. Localization test results confirmed earlier findings, namely, disruption of vertical plane ability generally, and a further, particular disturbance to horizontal plane localization in the conductive-mixed group. Compared with a normal control group, there were only slight signs of b…

A method of closure after resection of anterior floor of mouth cancers.

A variety of methods may be used to reconstruct head and neck defects after ablative surgery for cancer. Increasingly, choices of reconstructive techniques must be made on the basis of functional results attained. Described is a method of reconstruction for anterior floor of mouth defects with marginal mandibulectomy, which has yielded good functional results. All patients undergoing this form of reconstruction are on a soft oral diet, without drooling and with good speech intelligibility.

Comparison of Hogan pharyngeal flap and sphincter pharyngoplasty in postoperative velopharyngeal function.

The purpose of this study was to compare speech and breathing after sphincter pharyngoplasty and the Hogan pharyngeal flap in the management of cleft-related velopharyngeal insufficiency (VPI). We reviewed 78 patients with VPI who had either the Hogan flap (n=30) or sphincter pharyngoplasty (n=48) between 2009 and 2011. Velopharyngeal function, nasal patency, and speech were compared. In the Hogan flap group, 25 patients had achieved velopharyngeal competence and nine had normal speech. In the sphincter pharyngoplasty group, 29 patients achieved velopharyngeal competence and 20 normal speech.

End-to-End Mandarin Speech Reconstruction Based on Ultrasound Tongue Images Using Deep Learning.

The loss of speech function following a laryngectomy usually leads to severe physiological and psychological distress for laryngectomees. In clinical practice, most laryngectomees retain intact upper tract articulatory organs, emphasizing the significance of speech rehabilitation that utilizes articulatory motion information to effectively restore speech. This study proposed a deep learning-based end-to-end method for speech reconstruction using ultrasound tongue images. Initially, ultrasound tongue images and speech data were collected simultaneously with a designed Mandarin corpus. Subsequen

Design and evaluation of a two-channel compression hearing aid.

The design of a two-channel compression hearing aid for persons with moderate sensorineural hearing losses with recruitment is described. The aid applies slow-acting automatic gain control (AGC) to the whole signal, and then splits the signal into two bands, with separate fast-acting (syllabic) AGC in each band. Trials evaluating the aid have shown that it allows speech in quiet to be understood over a wide range of sound levels without any need to adjust the controls on the aid. It also gives speech intelligibility in noise superior to that allowed by a comparable linear (non-compression) aid…

Articulatory and Vocal Fold Movement Patterns During Loud Speech in Children With Cerebral Palsy.

PURPOSE: Speech motor control changes underlying louder speech are poorly understood in children with cerebral palsy (CP). The current study evaluates changes in the oral articulatory and laryngeal subsystems in children with CP and their typically developing (TD) peers during louder speech. METHOD: Nine children with CP and nine age- and sex-matched TD peers produced sentence repetitions in two conditions: (a) with their habitual rate and loudness and (b) with louder speech. Lip and jaw movements were recorded with optical motion capture. Acoustic recordings were obtained to evaluate vocal f

Comparison of In-Person and Online Recordings in the Clinical Teleassessment of Speech Production: A Pilot Study.

In certain circumstances, speech and language therapy is proposed in telepractice as a practical alternative to in-person services. However, little is known about the minimum quality requirements of recordings in the teleassessment of motor speech disorders (MSD) utilizing validated tools. The aim here is to examine the comparability of offline analyses based on speech samples acquired from three sources: (1) in-person recordings with high quality material, serving as the baseline/gold standard; (2) in-person recordings with standard equipment; (3) online recordings from videoconferencing. The effects of gracilis muscle transplantation on speech in children with Moebius syndrome.

Speech and communication are major problems for children with Moebius syndrome, a congenital anomaly that includes facial and abducens nerve paralysis and, often, other cranial nerve deficits. In addition, these children frequently have severe functional problems such as drooling as well as poor self-esteem. The purpose of this study was to investigate the outcome of speech with bilateral gracilis muscle transplants innervated by the masseteric nerve in children with Moebius syndrome. The outcome of this two-stage procedure was investigated in 12 patients. Assessments were done before and afte

[Hearing impairment in the so-called normal ear during Menière's disease].

A detailed audiological examination of 84 specially selected patients was performed. They had Meniere's disease without any audiometric changes in the so-called normal ear. The hearing function was characterized on the basis of threshold tonic audiometry, ultrasound sensitivity, lower limit of perceived sound frequencies, hearing discomfort level, dynamic range of the hearing field, and intelligibility of low voice speech masked by noise. It was demonstrated that all patients with Meniere's disease on the side of the so-called normal ear, where no symptoms of hearing insufficiency can be detec
