A Stage-1 trial of a hybrid speech-focused treatment for stuttering delivered in a non-residential semi-intensive format.

PURPOSE: From 1981-1999, Pat Richard Sacco directed a residential, highly intensive, group, hybrid speech-focused treatment program for stut … rs and speech rate. Ratings of speech naturalness were collected from listeners with no training in speech-language pathology. Finally, participant impressions of the treatment program and treatment impacts were gathered from graduation speeches. RESULTS: Relative to baseline speech, by the end of treatment: a) frequency of primary and secondary stuttering behaviors decre …

Children with speech difficulties: an exploratory survey of clinical practice in the Western Cape.

This study investigated the methods of assessment and intervention used by speech-language therapists (SLTs) in the Western Cape when working with children with speech difficulties. Children with speech difficulties are likely to form a considerable part of SLT caseloads in South Africa, but assessment choice may not be clear-cut given the linguistic diversity of the region and that few assessments have been developed specifically for the SA population. Selection of intervention approaches may also pose difficulties, linked to the lack of assessments and the limited evidence base in our contex

Primary progressive aphasia.

Primary progressive aphasia (PPA) is a progressive loss of specific language functions with relative sparing of other cognitive domains at least for the first few years of the illness. Based on the constellation of symptoms, PPA has been recently classified into a nonfluent, semantic, or logopenic variant. Nonfluent variant PPA is characterized by dysfluent and effortful speech, often combined with agrammatism. Also, some patients have initially predominant apraxia of speech. The neuroimaging findings in nfvPPA are in most cases progressive atrophy within the left inferior, opercular, and insu

Neural basis of speech and grammar symptoms in non-fluent variant primary progressive aphasia spectrum.

The non-fluent/agrammatic variant of primary progressive aphasia (nfvPPA) is a neurodegenerative syndrome primarily defined by the presence of apraxia of speech (AoS) and/or expressive agrammatism. In addition, many patients exhibit dysarthria and/or receptive agrammatism. This leads to substantial phenotypic variation within the speech-language domain across individuals and time, in terms of both the specific combination of symptoms as well as their severity. How to resolve such phenotypic heterogeneity in nfvPPA is a matter of debate. 'Splitting' views propose separate clinical entities: 'pr

Physical appearance and voice in male-to-female transsexuals.

It has been suggested that sex judgments of male-to-female transsexuals based on the voice may be influenced by the physical appearance of the clients. To explore this hypothesis, a listener experiment was designed in which a panel of 22 laypersons and 22 students in speech-language pathology rated the "femaleness" of fourteen male-to-female transsexuals from video-recorded speech samples in three modes of presentation: auditory-only presentation, visual-only presentation, and audiovisual presentation. Results indicate that appearance and voice are indeed interacting factors. Ratings from the …

Early intervention for speech impairment in children with cleft palate.

OBJECTIVE: This study explored the effectiveness of a parent-implemented, focused stimulation program on the speech characteristics of children younger than 3 years with cleft lip and palate. The research questions included the following: (1) Can parents be trained to deliver an early intervention (EI) program for children with cleft palate? (2) Does a parent-implemented EI program result in positive changes in speech characteristics? PARTICIPANTS: Ten mother-child pairs in which the child had cleft lip and palate (CLP) and 10 mother-child pairs in which the child did not have a cleft (NCLP).

Neuropathology-specific language features in primary progressive aphasia.

This study aimed to identify language markers for specific neuropathologies using both standard clinical tests and narrative speech analysis. METHODS: We analyzed data from 82 autopsy-confirmed PPA cases, including Alzheimer's disease (AD), transactive DNA-binding protein 43 (TDP-43) type C (TDP-C), Pick's disease, and 4R-tauopathies (progressive supranuclear palsy/ cortico-basal degeneration (PSP/CBD). Dysphagia services in the era of COVID-19: Are speech-language therapists essential?

In the era of coronavirus disease 2019 (COVID-19), many healthcare professionals are being faced with the question of what is considered to be an essential service. This opinion paper has attempted to answer this complex question by understanding the potential relationship between dysphagia and COVID-19 and how speech-language therapists (SLTs) in South Africa should tackle this. It also aims to answer the question through the lens of a risk-benefit discussion based around practices and decision-making. Important gaps in the field relating to how SLT practices need to move forward during this

Auditory processing disorders: an update for speech-language pathologists.

PURPOSE: Unanswered questions regarding the nature of auditory processing disorders (APDs), how best to identify at-risk students, how best to diagnose and differentiate APDs from other disorders, and concerns about the lack of valid treatments have resulted in ongoing confusion and skepticism about the diagnostic validity of this label. This poses challenges for speech-language pathologists (SLPs) who are working with school-age children and whose scope of practice includes APD screening and intervention. The purpose of this article is to address some of the questions commonly asked by SLPs r

Assessment of pragmatic difficulties and socio-emotional adjustment in practice.

BACKGROUND: In professional practice, psychologists and other professionals such as therapists and teachers receive referrals of many children who present with social, emotional and behavioural difficulties that are difficult to understand and assess. The problems of some of these children may stem from pragmatic difficulties in communication. This paper reports the results of a study on the use of checklists in professional practice to assist in the identification of these difficulties. AIMS: (1) To ascertain whether two checklists, Bishop's (1998) Children's Communication Checklist and Good

Issues contrasting adult acquired versus developmental apraxia of speech.

Acquired and developmental apraxia of speech (AOS and DAS) are defined as disorders of the transition from an abstract phonological code into motor commands. However, the natural course of these disorders differs substantially because of the fundamental difference in the developmental stage at which the apraxia expresses itself. In normal and pathological development alike, development of language and speech is an interactive process, involving speech motor control, perception, and psycholinguistic processes. Infant speech develops from random babbling and sensomotoric learning, followed by mo

Using telerehabilitation to assess apraxia of speech in adults.

BACKGROUND: Telerehabilitation is the remote delivery of rehabilitation services via information technology and telecommunication systems. There have been a number of studies that have used videoconferencing to assess speech and language skills in people with acquired neurogenic communication disorders. However, few studies have focused on cases of apraxia of speech. In order to perform accurately differential diagnosis via the telerehabilitation medium, it is important that validation of the assessment of apraxia of speech be established as part of the overall evidence base for telerehabilita

Hearing, speech, language, and vestibular disorders in the fetal alcohol syndrome: a literature review.

Fetal alcohol syndrome (FAS) is characterized in part by mental impairment, as well as craniofacial and ocular anomalies. These conditions are traditionally associated with childhood hearing disorders, because they all have a common embryonic origin in malformations of the first and second branchial arches, and have similar critical periods of vulnerability to toxic insult. A review of human and animal research indicates that there are four types of hearing disorders associated with FAS. These are: (1) a developmental delay in auditory maturation, (2) sensorineural hearing loss, (3) intermitte

South African Speech-Language Therapy and Audiology students' experiences of academic and clinical curriculum transformation.

BACKGROUND: South African Speech-Language Therapy and Audiology (SLH) programmes historically followed Western frameworks, often lacking in cultural and linguistic relevance to the local context. SLH curricula and clinical training continue to face challenges in aligning with South Africa's diverse population and healthcare needs. OBJECTIVES: To investigate South African undergraduate SLH students' experiences of transformation in the curriculum and clinical service provision. METHOD: A cross-sectional survey design was employed on students from four South African universities. Data were col
