Successful treatment of a patient with refractory nephrotic syndrome with PCSK9 inhibitors: a case report.

BACKGROUND: The proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitor evolocumab is a low-density lipoprotein (LDL)-lowering drug with a new mechanism, which is currently available in Japan. Here, for the first time, we report the successful use of the PCSK9 inhibitor in a patient with refractory nephrotic syndrome. CASE PRESENTATION: A 61-year-old woman was diagnosed with minimal change-type nephrotic syndrome in October 2012. She received prednisolone (PSL) and cyclosporin A (CyA), but she experienced several cycles of relapse and remission and was hospitalized in May 2016 due to

Role of PCSK9 beyond liver involvement.

PURPOSE OF REVIEW: Proprotein convertase subtilisin kexin type 9 (PCSK9) acts as an endogenous natural inhibitor of the LDL receptor pathway, by targeting the receptor to lysosomes for degradation. Beside the liver, PCSK9 is also expressed at significant levels in other tissues, where its function remains unclear. The current review focuses on the extrahepatic actions of PCSK9. RECENT FINDINGS: The generation of liver-specific PCSK9 knockout mice has clearly indicated that PCSK9 affects cholesterol homeostasis via its action on extrahepatic organs. PCSK9 is highly expressed in the intestine,

Emerging therapeutic agents to lower lipoprotein (a) levels.

PURPOSE OF REVIEW: Recent epidemiological and genetic studies have suggested that lipoprotein (a) [Lp(a)] is a causal mediator of cardiovascular disease (CVD). There is now interest in evaluating Lp(a) as a therapeutic target. This review will summarize emerging therapeutic agents to lower Lp(a). RECENT FINDINGS: Apheresis is the most efficacious method to lower Lp(a). Currently, there are no approved drugs to specifically lower Lp(a). However, recent data has demonstrated that Lp(a) can be significantly lowered, along with reductions in other apolipoprotein B-100 (apoB) containing lipoprotei

PCSK9 reduces the protein levels of the LDL receptor in mouse brain during development and after ischemic stroke.

Proprotein convertase subtilisin/kexin type 9 (PCSK9) plays a major role in cholesterol homeostasis through enhanced degradation of the LDL receptor (LDLR) in liver. As novel inhibitors/silencers of PCSK9 are now being tested in clinical trials to treat hypercholesterolemia, it is crucial to define the physiological consequences of the lack of PCSK9 in various organs. LDLR regulation by PCSK9 has not been extensively described during mouse brain development and injury. Herein, we show that PCSK9 and LDLR are co-expressed in mouse brain during development and at adulthood. Although the protein

A secreted serine protease of Paracoccidioides brasiliensis and its interactions with fungal proteins.

BACKGROUND: Paracoccidioides brasiliensis is a thermodimorphic fungus, the causative agent of paracoccidioidomycosis (PCM). Serine proteases are widely distributed and this class of peptidase has been related to pathogenesis and nitrogen starvation in pathogenic fungi. RESULTS: A cDNA (Pbsp) encoding a secreted serine protease (PbSP), was isolated from a cDNA library constructed with RNAs of fungal yeast cells recovered from liver of infected mice. Recombinant PbSP was produced in Escherichia coli, and used to develop polyclonal antibodies that were able to detect a 66 kDa protein in the P. b

The cystatin-related epididymal spermatogenic protein inhibits the serine protease prohormone convertase 2.

The cystatin-related epididymal spermatogenic (CRES) protein is related to the family 2 cystatins of the cystatin superfamily of cysteine protease inhibitors. However, CRES lacks sequences important for cysteine protease inhibitory activity and is specifically expressed in reproductive and neuroendocrine tissues. Thus, CRES is distinct from cystatins and may perform unique tissue-specific functions. The purpose of the present study was to determine whether CRES functions as a protease inhibitor in in vitro assays. In contrast to mouse recombinant cystatin C, recombinant CRES did not inhibit th

How important are entropic contributions to enzyme catalysis?

The idea that enzymes accelerate their reactions by entropic effects has played a major role in many prominent proposals about the origin of enzyme catalysis. This idea implies that the binding to an enzyme active site freezes the motion of the reacting fragments and eliminates their entropic contributions, (delta S(cat)(double dagger))', to the activation energy. It is also implied that the binding entropy is equal to the activation entropy, (delta S(w)(double dagger))', of the corresponding solution reaction. It is, however, difficult to examine this idea by experimental approaches. The pres

Substrate Concentration Modulates Peptide Profile and Bioactivity in Alcalase Hydrolysis of Bovine Plasma Proteins at Fixed Enzyme-to-Substrate Ratio.

Many studies have been conducted testing different enzymes and protein combinations to produce peptides with different types and levels of bioactivities; however, their effect on peptide sequence, peptide size distribution, and bioactivity, while maintaining a constant enzyme-substrate ratio, has been less frequently addressed. In this research, bovine plasma has been hydrolysed by Alcalase at several substrate concentrations (5, 10, 15, and 20 g/L) and at a fixed enzyme-substrate ratio of 1:25 (w/w, Alcalase mass per plasma protein mass). For each substrate concentration, the in vitro antioxi

Characterization and Antioxidant and Angiotensin I-Converting Enzyme (ACE)-Inhibitory Activities of Gelatin Hydrolysates Prepared from Extrusion-Pretreated Milkfish (Chanos chanos) Scale.

Fish gelatin hydrolysates have been shown to possess various biological activities due to their unique Gly-Pro-Y and Gly-X-Hyp sequences. In the current study, fish gelatin was extracted from non-extruded milkfish scale (FSG1) or extrusion-pretreated milkfish scale (FSG2); extracted gelatins were hydrolyzed with different combinations of Flavourzyme and Alcalase to give four different hydrolysates, namely: FSGH1 (FSG1 hydrolyzed with Flavourzyme), FSGH2 (FSG1 hydrolyzed with Alcalase + Flavourzyme), FSGH3 (FSG2 hydrolyzed with Flavourzyme), and FSGH4 (FSG2 hydrolyzed with Alcalase + Flavourzym

Antihypertensive effects of continuous oral administration of nattokinase and its fragments in spontaneously hypertensive rats.

To determine whether the antihypertensive effect of nattokinase is associated with the protease activity of this enzyme, we compared nattokinase with the fragments derived from nattokinase, which possessed no protease activity, in terms of the effect on hypertension in spontaneously hypertensive rats (SHR). In the continuous oral administration test, the groups were given a basic diet alone (control), the basic diet containing nattokinase (0.2, 2.6 mg/g diet) or the basic diet containing the fragments derived from nattokinase (0.2, 0.6 mg/g diet). The group fed the basic diet containing high-d

Identification of Potent ACE Inhibitory Peptides from Wild Almond Proteins.

UNLABELLED: In this study, the production, fractionation, purification and identification of ACE (angiotensin-I-converting enzyme) inhibitory peptides from wild almond (Amygdalus scoparia) proteins were investigated. Wild almond proteins were hydrolyzed using 5 different enzymes (pepsin, trypsin, chymotrypsin, alcalase and flavourzyme) and assayed for their ACE inhibitory activities. The degree of ACE inhibiting activity obtained after hydrolysis was found to be in the following order: alcalase > chymotrypsin > trypsin/pepsin > flavourzyme. The hydrolysates obtained from alcalase (IC50 = 0.8 m

Structural basis for the kexin-like serine protease from Aeromonas sobria as sepsis-causing factor.

The anaerobic bacterium Aeromonas sobria is known to cause potentially lethal septic shock. We recently proposed that A. sobria serine protease (ASP) is a sepsis-related factor that induces vascular leakage, reductions in blood pressure via kinin release, and clotting via activation of prothrombin. ASP preferentially cleaves peptide bonds that follow dibasic amino acid residues, as do Kex2 (Saccharomyces cerevisiae serine protease) and furin, which are representative kexin family proteases. Here, we revealed the crystal structure of ASP at 1.65 A resolution using the multiple isomorphous repla

Synthetic community producing bacterial inhibitory proteins to control postharvest black spot disease of winter jujube.

Postharvest fungal decay, particularly black spot disease caused by Alternaria alternata, poses a significant challenge to the storage and economic value of winter jujube (Ziziphus jujuba Mill. cv. Dongzao). While synthetic communities have emerged as a promising sustainable alternative to chemical fungicides, the specific metabolites driving their biocontrol efficacy remain poorly understood. This study aimed to identify and characterize the primary antifungal components produced by a specialized synthetic community. After systematically excluding the interference of organic acids and hydroge

PCSK9 inhibitors conquer primary prevention: the VESALIUS-CV study.

Elevated low-density lipoprotein cholesterol (LDL-C) represents a key causal factor in atherosclerotic cardiovascular disease. Robust evidence demonstrates that early, marked, and sustained reduction of LDL-C is associated with a significant decrease in the risk of major adverse cardiovascular events. However, a substantial proportion of high-risk patients fail to achieve guideline-recommended LDL-C targets with statin therapy alone, thus requiring additional lipid-lowering treatments. Proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors, particularly the monoclonal antibodies evol

Evaluating enlicitide decanoate: first oral PCSK9 inhibitor for the treatment of adults with hypercholesterolemia.

INTRODUCTION: Atherosclerotic cardiovascular disease (ASCVD) remains the leading global cause of morbidity and mortality, with elevated low-density lipoprotein cholesterol (LDL-C) as a major modifiable risk factor. Despite intensive statin therapy, many high-risk patients do not achieve increasingly stringent LDL-C goals, highlighting the need for additional lipid-lowering strategies. The development of proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors has significantly advanced lipid management, although currently available agents require subcutaneous administration and may be

α-Synuclein Promotes Atherosclerosis by Impairing Macrophage Autophagic Flux.

Atherosclerosis, recognized as a prototypical age-associated vascular disease, is driven by macrophage foam cell formation. Individuals with Parkinson's disease (PD) exhibit an elevated risk for stroke. While α-synuclein (α-Syn) is known for its neuronal role in PD, its presence in the circulation and potential vascular effects remain unexplored. Here, we identify α-Syn as a previously unrecognized circulating factor that promotes atherosclerosis. α-Syn was detected in human atherosclerotic plaques. Genetic deletion of α-Syn reduced atherosclerotic lesion size and macrophage lipid accumulation

Nonstatins for LDL-C lowering: An expansion whose time has come.

The 2026 American College of Cardiology/American Heart Association/Multisociety Dyslipidemia Guideline marks a defining moment for lipidology with the reinstatement of low-density lipoprotein cholesterol (LDL‑C) and non-high-density lipoprotein cholesterol treatment goals and a decisive endorsement of combination lipid‑lowering therapy. Although statins remain foundational, statin monotherapy often fails to achieve sufficient LDL‑C lowering, particularly in high-risk patients or patients with statin intolerance or refusal. These realities elevate nonstatin therapies from secondary consideratio
