Increased failure rates after the introduction of the TFNA proximal femoral nail for trochanteric fractures: implant related or learning curve effect?

Background and purpose - Trochanteric fractures are often treated using intramedullary fixation. In our institution, the TFN-Advanced Proximal Femoral Nailing System (TFNA) was introduced as replacement for the Gamma Trochanteric Nail (GTN3) for the treatment of these fractures as a result of a hospital-driven change of trauma implant supplier. We compared trochanteric fracture fixation failure rate between these 2 intramedullary nails. Patients and methods - All trochanteric fractures treated surgically from 2011 to 2019 were retrospectively reviewed for fixation failure. From 2016 only the T

Biomechanical study of rod stress in lumbopelvic fixation with lateral interbody fusion: an in vitro experimental study using synthetic bone models.

OBJECTIVE: Despite improvements in surgical techniques and instruments, high rates of rod fracture following a long spinal fusion in the treatment of adult spinal deformity (ASD) remain a concern. Thus, an improved understanding of rod fracture may be valuable for better surgical planning. The authors aimed to investigate mechanical stress on posterior rods in lumbopelvic fixation for the treatment of ASD. METHODS: Synthetic lumbopelvic bone models were instrumented with intervertebral cages, pedicle screws, S2-alar-iliac screws, and rods. The construct was then placed in a testing device, an

Precision of patient specific screw holes locating surgical guide and pre-bent plates osteosynthesis versus classical workflow in management of class III mandibular fracture.

BACKGROUND: Several treatment modalities have been reported in the management of mandibular fractures using an alternative computer-guided approach through the utilization of different designs of guiding devices. However, these computer-guided methods do not always guarantee accurate anatomical bone reduction. This study aimed to assess the reduction precision of the computer-guided mandibular fracture and internal fixation using screw holes locating surgical guide, as presented earlier in the orthognathic surgery field in various studies to be applied in the field of mandibular traumatology,

Periprosthetic femoral fractures treated by locked plating: feasibility assessment of the mini-invasive surgical option. A prospective series of 36 fractures.

INTRODUCTION: The treatment of periprosthetic femoral fractures by conventional plating is associated with problems related to fracture union and eventual refracture. Additionally, locking nailing cannot be used in all cases because of the risk of malunion. To resolve these issues, locking plates have been proposed to combine the advantages of closed reduction and internal fixation while achieving a higher quality reduction with plate fixation. HYPOTHESIS: Locking plates put into place by a mini-invasive surgical approach result in fixation without substantial misalignment or non-union. PATI

Interventions for treating ankle fractures in children.

BACKGROUND: Ankle fractures, which usually occur after a twisting incident, are a diverse collection of injuries with different levels of complexity and severity. They have an incidence of 1 in 1000 a year in children. Treatment generally involves splints and casts for minor fractures and surgical fixation with screws, plates and pins followed by immobilisation for more serious fractures. OBJECTIVES: To assess the effects (benefits and harms) of different interventions for treating ankle fractures in children. SEARCH METHODS: We searched the Cochrane Bone, Joint and Muscle Trauma Group Speci

Modern Implant Options for the Treatment of Distal Femur Fractures.

Distal femur fractures occur in a periarticular fracture pattern and disproportionately afflict an aging population. Although the goals of treatment have not changed, the emergence of new surgical techniques and devices has recently been developed and refined to treat this challenging fracture pattern. Treatment options include open reduction and internal fixation with periarticular locking plates, intramedullary nails, or distal femur replacement. Despite rapid adoption, these modern solutions display a concerning complication rate, specifically from nonunion and malunion. The indications for

[Treatment of partial articular fractures of the distal radius in active young adults].

The wrist's function is at stake in young active adults with partial intra-articular fractures of the distal radius. The high energy nature of these injuries, displaced fractures with risk of malunion in case of insufficient treatment, and associated ligament or cartilage damage all hinder the prognosis of these fractures. Many classification systems exist to help us analyze and in some cases, select a treatment. Optimal management requires a high-quality preoperative assessment and a precise surgical technique coupled with the use of arthroscopy to deal with joint and ligament injuries in the

Biomechanical assessment of lumbar stability: finite element analysis of TLIF with a novel combination of coflex and pedicle screws.

Purpose: Finite element analysis is frequently used for lumbar spine biomechanical analysis. The primary scope of this work is to illustrate, using finite element analysis, how the biomechanical behavior of the transforaminal lumbar interbody fusion (TLIF), along with a novel combination of the interspinous process device (IPD) and pedicle screws, improves lumbar spine stability. Methods: In this study, unilateral pedicle screw fixation (UPSF) and bilateral pedicle screw fixation (BPSF) were used. Four FE models were developed using ANSYS software, as follows: (1) Intact model; (2) TLIF with "

Development of a radiation-free magnetic guidance device for distal screw placement in intramedullary nails.

This study developed a practical device for guiding distal interlocking screws during intramedullary nail fixation, aiming to address issues associated with the conventional free-hand approach using fluoroscopy. The objectives were to attain precision, reduce radiation exposure, and ease of operation. A small cylindrical magnet was embedded in the distal screw hole to emit signals for positioning. Due to the magnet's relatively weak magnetic force, especially in clinical settings with obese patients, the required long sensing distance is significant, making it susceptible to interference from

Pop-top tibial component: elimination of back-side wear with potential modularity.

Survivorship analysis has demonstrated the excellent long-term clinical success of total knee arthroplasty (TKA). Despite multiple attempts to enhance fixation with cementless technology, cemented TKA continues to be the "gold standard." The rate of loosening has diminished as the accuracy of implantation has been improved through sophisticated instrumentation and enhanced experience. Current technology with respect to computer-assisted surgery serves only to improve the accuracy of implantation, leaving materials as the weakest link in the long-term survivorship of TKA. Failure secondary to p

Maximizing screw length in expandable lateral lumbar interbody spacers with integrated fixation may obviate the need for supplemental pedicle screws.

BACKGROUND CONTEXT: Lateral lumbar interbody fusion (LLIF) is a minimally invasive surgical technique that provides a wide footprint interbody cage for correction of lumbar coronal and sagittal deformity. Traditional spinal interbody fusion procedures utilize pedicle screws and rods for additional stability. An expandable lateral titanium interbody cage with an integrated lateral fixation (eLLIFp) device provides a stand-alone LLIF that is intended to function autonomously. This may reduce the complexity of the surgery and the potential risks associated with supplemental posterior instrumentat
