Changes of swallowing function after tracheostomy: a videofluoroscopy study.

BACKGROUND: Patients after tracheostomy often present swallowing dysfunctions but little is known about the mechanism underlying dysphagia and its reversibility. The aims of this study were: 1) to characterize swallowing dysfunctions in patients with dysphagia and tracheostomy; 2) to evaluate the reversibility of these changes; 3) to evaluate the possible influence of the underlying disease. METHODS: Prospective, observational, single-center study enrolling patients with tracheostomy admitted to a rehabilitation center over a period of 36 months. All patients who were found to be dysphagic un

Swallowing pressure and pressure profiles in young healthy adults.

OBJECTIVES/HYPOTHESIS: To measure the swallowing pressure (SP) of normal subjects using a 2.64-mm-diameter high-resolution manometry (HRM) catheter with 36 circumferential sensors. STUDY DESIGN: Repeated measures with subjects serving as controls. METHODS: Thirty healthy subjects swallowed water at different temperatures and volumes to examine the maximum SP at the velopharynx, meso-hypopharynx, upper esophageal sphincter (UES), and cervical esophagus, and the duration of lowered pressure at the UES. RESULTS: The maximum SP at any location was unaffected by the volume of water, whereas the

Is swallowing of all mixed consistencies dangerous for penetration-aspiration?

OBJECTIVE: The aim of this study was to determine whether the risk and severity of penetration-aspiration with mixed consistency (MIX), which consists of cooked rice and thin liquid barium (LIQUID), are different from the risks and severities with each single consistency (cooked rice or LIQUID) in dysphagic patients. DESIGN: Dysphagic patients (N = 29) performed a videofluoroscopic swallowing study with the following foods: cooked rice, LIQUID, and MIX. Several components were analyzed using recorded videotapes. RESULTS: The Penetration-Aspiration Scale score for MIX was significantly lower

Prevalence and severity of voice and swallowing difficulties in mitochondrial disease.

BACKGROUND: Mutations of mitochondrial DNA (mtDNA) cause a broad spectrum of clinical phenotypes. Anecdotal evidence suggests that voice and swallow problems are a common feature of these diseases. AIMS: To characterize accurately the prevalence and severity of voice and swallow problems in a large cohort of patients with mitochondrial disease. METHODS & PROCEDURES: Patients with proven mitochondrial disease were sent validated questionnaires to assess both voice and swallow function. The presence of voice and swallow symptoms was correlated with other clinical features of mitochondrial dise

Early deficits in cortical control of swallowing in Alzheimer's disease.

The goal of this study was to determine whether functional changes in cortical control of swallowing are evident in early Alzheimer's disease (AD), before dysphagia (swallowing impairment) is evident. Cortical function was compared between an early AD group and a group of age-matched controls during swallowing. Swallowing oropharyngeal biomechanics examined from videofluoroscopic recordings were also obtained to more comprehensively characterize changes in swallowing associated with early AD. Our neuroimaging results show that the AD group had significantly lower Blood-Oxygen-Level-Dependent (

Clinical lesson: eosinophilic oesophagitis, a new diagnosis to swallow.

Eosinophilic oesophagitis (EoE) is a recently described condition that has gained increasing recognition over the past 5 years. Despite this, many clinicians remain unaware of EoE, often leading to diagnostic delay and therefore significant morbidity. The diagnosis of EoE should be considered in any patient with a history of intermittent or continuous dysphagia, or oesophageal food impaction. It should be strongly suspected in young patients, particularly men, presenting with dysphagia and a history of atopy. Here, three patients are presented that highlight common features of EoE. In addition

Contingent negative variations associated with command swallowing in humans.

OBJECTIVE: To clarify the role of the cerebral cortex in the swallowing movement, the difference between the waveforms of contingent negative variation (CNV) for the command swallowing and movement-related cortical potential (MRCP) obtained during the volitional swallowing task in humans was investigated. METHODS: Subjects were instructed to swallow their saliva as quickly as possible in response to a sound signal 4s after the onset of a self-paced breath holding in the command swallowing task or to swallow it while holding their breath for 4s in the volitional swallowing task. RESULTS: CNV

Age-related differences in laterality of cortical activations in swallowing.

The present study examined age differences in neural lateralization patterns during swallowing and three related tasks, using functional magnetic resonance imaging (fMRI). Ten healthy right-handed young adults (mean age = 21.7 years, SD = 2.1 years) and nine healthy elders (mean age = 70.2 years, SD = 3.9 years) were scanned in a 3-T MRI head scanner. Participants were visually cued to "prepare to swallow," "swallow," "tap your tongue," and "clear your throat" in randomized order. Laterality preference for each task was examined within and between groups using region-of-interest (ROI) analyses

Use of reaction time in the temporal analysis of normal swallowing.

This study was performed to establish the swallowing trigger by using the reaction time from an auditory stimulus. With this stable temporal starting point, we described the chronology of the different acoustic, electrophysiologic, and respiratory events that occurred during swallowing in a population of normal adults. We studied the swallowing reaction time (SRT) in 18 subjects aged 23 to 73 years by using acoustic, electroglottographic (EGG), and aerodynamic recordings. The chronology (the beginning of EGG activity, apnea, and respiratory sound and release) was identified in 91% of the recor

Anxiety, depression and swallowing disorders in patients with Parkinson's disease.

Swallowing disturbances (SDs), anxiety and depression are commonly present in Parkinson's disease (PD) patients. We hypothesized that there is an association between the presence of SDs and the PD affective state. Sixty-nine PD patients were assessed for the presence of SDs by undergoing cognitive screening with the Mini Mental State Examination (MMSE), completing three inventories: a swallowing disturbance questionnaire (SDQ), the Spielberger manual for the trait anxiety and Beck depression inventories. All patients underwent clinical swallowing evaluations by a speech and language pathologis

Is drooling secondary to a swallowing disorder in patients with Parkinson's disease?

Drooling is a common manifestation in Parkinson's disease (PD). It causes psychosocial difficulties and can result in aspiration and chest infection. Previous studies point to an association between swallowing problems and sialorrhea. The aim of this study was to determine if drooling is associated with dysphagia in PD patients. Sixteen PD patients with diurnal drooling were assessed using a modified barium swallowing with videofluoroscopy, and a drooling score. Changes in the oral stage of swallowing were seen in 100% of the patients; and in the pharyngeal stage, in 94% of the patients. The r

Analysis of swallowing sounds using hidden Markov models.

In recent years, acoustical analysis of the swallowing mechanism has received considerable attention due to its diagnostic potentials. This paper presents a hidden Markov model (HMM) based method for the swallowing sound segmentation and classification. Swallowing sound signals of 15 healthy and 11 dysphagic subjects were studied. The signals were divided into sequences of 25 ms segments each of which were represented by seven features. The sequences of features were modeled by HMMs. Trained HMMs were used for segmentation of the swallowing sounds into three distinct phases, i.e., initial quie

Direct measurement of subglottic air pressure while swallowing.

OBJECTIVE: The subglottic pressure theory for swallowing asserts that laryngeal mechanoreceptors have a role in the regulation of swallowing function. The primary purpose of this study was to determine if subglottic air pressure is generated during swallowing in a healthy, nontracheostomized person. METHODS: This pilot investigation used a prospective, repeated-measures design in a single subject. Direct measurement of subglottic air pressure was obtained through percutaneous puncture of the cricothyroid membrane. Swallows were timed with four randomly assigned lung volumes: total lung capaci…

[Specialties in dentistry. Salivary flow and swallowing in Parkinson's disease].

Parkinson's disease is a slowly progressive and irreversible disorder of the nervous system. Drooling is listed as a secondary symptom of Parkinson's disease. Its cause is insufficiently clear. In the literature 2 possible causes are described: hypersalivation and swallowing abnormalities. These parameters have not been measured in a single study before. This article presents a review of the literature on the subject and describes the design for a future clinical study. The aim of this clinical study is to gain an insight into the cause and the prevalence of drooling in Parkinson's patients. A
