Does the Sweating Severity of Primary Hyperhidrosis Sites Affect Post-Sympathotomy Results?

BACKGROUND: Improvement in quality of life (QOL) and patient satisfaction after endoscopic thoracic sympathotomy (ETS) in patients with primary hyperhidrosis may be affected by various factors. We examined whether the preoperative sweating severity of primary hyperhidrosis sites affects postoperative results. METHODS: The records of 112 patients who underwent bilateral ETS were reviewed retrospectively. The patients were divided into three groups according to the sweating severity score obtained from all primary hyperhidrosis sites (primary hyperhidrosis severity score [PHSS]) and analyzed co…

[Anno 1529 - the "English Sweating Sickness" in Vienna, the Turkish troops around Vienna].

After the battle of Bosworth in 1485 London was frightened by a severe epidemic of Sweating Disease. Until 1551 four more followed of which only the 1525 epidemic invaded Central and Northern Europe harassing Vienna during its "First Turkish Siege".People abruptly fell ill with headache, tachycardia, fever and secretion of stinking sweat. They were afflicted with anxiety, deliria and somnolence. The illness mostly lasted for 24 h, but death although could occur earlier. At the beginning of epidemics the lethality was particularly high. The treatment with distinct medicaments had to be started …

Crisponi/cold-induced sweating syndrome: Differential diagnosis, pathogenesis and treatment concepts.

Crisponi/cold-induced sweating syndrome (CS/CISS) is an autosomal recessive disease characterized by hyperthermia, camptodactyly, feeding and respiratory difficulties often leading to sudden death in the neonatal period. The affected individuals who survived the first critical years of life, develop cold-induced sweating and scoliosis in early childhood. The disease is caused by variants in the CRLF1 or in the CLCF1 gene. Both proteins form a heterodimeric complex that acts on cells expressing the ciliary neurotrophic factor receptor (CNTFR). CS/CISS belongs to the family of "CNTFR-related dis…

A laser-engraved wearable sensor for sensitive detection of uric acid and tyrosine in sweat.

Wearable sweat sensors have the potential to provide continuous measurements of useful biomarkers. However, current sensors cannot accurately detect low analyte concentrations, lack multimodal sensing or are difficult to fabricate at large scale. We report an entirely laser-engraved sensor for simultaneous sweat sampling, chemical sensing and vital-sign monitoring. We demonstrate continuous detection of temperature, respiration rate and low concentrations of uric acid and tyrosine, analytes associated with diseases such as gout and metabolic disorders. We test the performance of the device in …

Warmth and nociceptive evoked potentials in cold-induced sweating syndrome type 1.

INTRODUCTION: Cold-induced sweating syndrome type 1 (CISS1), is a rare, severe, autosomal recessive disease. It is characterized by morphological alterations and profuse sweating when ambient temperature is <22 °C. Although some individuals with CISS1 have decreased pain perception, no study has been conducted to evaluate thermal and pain sensations in these patients. The aim of this study was to assess the function of the nociceptive Aδ-fibers and warmth C-fibers by using CO2 laser-evoked potentials (LEPs) in patients affected by CISS1. METHODS: Four patients were studied. Laser pulses were …

Extent of sympathectomy affects postoperative compensatory sweating and satisfaction in patients with palmar hyperhidrosis.

PURPOSE: Endoscopic thoracic sympathectomy (ETS) for the treatment of palmar hyperhidrosis is generally performed at one or two levels ranging between T2 and T4; however, compensatory sweating (CS) is an occasional bothersome side effect. The aim of our study was to evaluate the association between the extent of ETS and the degree of postoperative CS and palmar sweating, as well as patient satisfaction. METHODS: The participants represented a consecutive series of 76 patients who underwent bilateral ETS for palmar hyperhidrosis at level T2 and/or T3. Patients were interviewed by postal questi…

BerEP4 is widely expressed in tumors of the sweat apparatus: a source of potential diagnostic error.

BACKGROUND: A 63-year-old man presented with an ulcerated nodule with a rolled pearly edge on his back. A punch biopsy showed diffuse strong BerEP4 expression and retraction artifact. Consequently the tumor was diagnosed as a basal cell carcinoma but on excision the tumor proved to be a porocarcinoma. Although Jimenez et al. reported BerEP4 expression in porocarcinoma, this result is not widely appreciated. This observation prompted us to investigate BerEP4 expression in sweat apparatus tumors. METHODS: Immunocytochemistry was performed using Dako monoclonal mouse BerEP4 with Ventana Benchmar…

The cholinergic blockade of both thermally and non-thermally induced human eccrine sweating.

Thermally induced eccrine sweating is cholinergically mediated, but other neurotransmitters have been postulated for psychological (emotional) sweating. However, we hypothesized that such sweating is not noradrenergically driven in passively heated, resting humans. To test this, nine supine subjects were exposed to non-thermal stimuli (palmar pain, mental arithmetic and static exercise) known to evoke sweating. Trials consisted of the following four sequential phases: thermoneutral rest; passive heating to elevate (by ~1.0°C) and clamp mean body temperature and steady-state sweating (perfusion…

[A 22 year old woman with fever, night sweats, weight loss and hepatomegaly].

A 22 year old female patient presented with fever, night sweats, weight loss and hepatomegaly associated with elevated inflammatory parameters and liver enzymes. Computer tomography revealed a mass located between the inferior vena cava and the psoas muscle as well as enlarged celiac, retroperitoneal and retrocaval lymph nodes. Biopsies of the retrocaval mass led to the diagnosis of retroperitoneal fibrosis. Within a few days of treatment with corticosteroids clinical presentation improved and imaging studies detected complete regression of the retrocaval mass after 6 months.

Endocrine mucin-producing sweat gland carcinoma: report of a case and review of the literature.

Mucinous carcinoma of skin (MCS) is an uncommon adnexal tumor of disputed differentiation. In 1995, Rahilly et al. reported a case of MCS with neuroendocrine differentiation (E-MCS). Since that report, seven additional cases have been published. Here, we report on a case of E-MCS and discuss the differential diagnoses.

Chronological changes of postsympathectomy compensatory hyperhidrosis and recurrent sweating in patients with palmar hyperhidrosis.

OBJECT: The author sought to investigate the temporal changes of postsympathectomy compensatory hyperhidrosis and recurrent sweating in patients with primary palmar hyperhidrosis. METHODS: The author examined 91 consecutive patients for this prospective 6-year study. The patients were interviewed at least twice during a 6-month interval; the first follow up was conducted at a median of 1.7 years after surgery (range 2.5-60.5 months). Overall, 24 patients (26.4%) were followed for more than 2 years. Attention was focused on patient satisfaction and the incidence of compensatory hyperhidrosis a…

Botulinum toxin A (Botox) and sweating-dose efficacy and comparison to other BoNT preparations.

BACKGROUND: Botulinum toxin type A (BoNT/A) is 20-50 times more effective than Botulinum toxin type B (BoNT/B) concerning the treatment of muscular hypercontractions [Sloop, R.R., Cole, B.A., Escutin, R.O., 1997. Human response to botulinum toxin injection: type B compared with type A. Neurology 49, 189-194]. Botulinum toxins block motor nerves as well as autonomic fibres [Rand, M.J., Whaler, B.C., 1965. Impairment of sympathetic transmission by botulinum toxin. Nature 206, 588-591]. OBJECTIVE: Purpose of this study was to analyse the dose dependent reduction of sweating using the BoNT/A prep…

Anatomy of the sweat glands, pharmacology of botulinum toxin, and distinctive syndromes associated with hyperhidrosis.

For a long period the therapeutic modalities to treat focal hyperhidrosis (HH) were very limited. Due to this the problem of focal HH was delt with stepmotherly. Nowadays we can consider BTX as the therapy of choice for axillary HH after topical treatment with aluminium salts have failed. The amount of successful reports on botulinum toxin (BTX) in the treatment of focal HH brought a change and the interest for this specific disorder grew. This article gives details on anatomy and physiology of sweating and mechanism of BTX. Further distinctive syndromes associated with HH, which all can be tr…

Immunohistochemical study of carbonic anhydrase in mixed tumours and adenomas of sweat and sebaceous glands.

Immunohistochemical distribution of carbonic anhydrase II (CA) in mixed tumours and adenomas of sweat gland origin and in sebaceous adenomas was demonstrated by the PAP method. Normal sweat glands, both eccrine and apocrine, clear cells of the secretory coils, and ductal epithelial cells all showed conspicuous staining for CA, and sebaceous glands were also positive. Mixed tumours of the skin indicated strongly positive staining for CA in the luminal cells of tubular and duct-like or cystic structures, while most of the other tumour cells were negative. In solid or massive foci, CA positive ce…

Transient acantholytic dermatosis (Grover's disease). A skin disorder related to heat and sweating.

We report seven cases of transient acantholytic dermatosis (Grover's disease) to exemplify a causal association with heat and sweating. The excessive heat and sweating was related to the use of a hot tub, a hot water bottle, a steam bath, an electric blanket, the prolonged wearing of a polyester suit, and postoperative bed confinement.
