Microinvasive resectional sympathectomy using the harmonic scalpel. A more effective procedure with fewer side effects for treating essential hyperhidrosis of the hands, face or axillae.

Compensatory sweating remains one of the most unpredictable and debilitating complications of sympathectomy for hyperhidrosis. There appears to be a significant difference in compensatory sweating rates seen between the different techniques utilized to treat hyperhidrosis. Resectional techniques have historically resulted in excellent symptom relief with relatively low levels of severe post-op compensatory sweating. The advent of video-assisted approaches to the treatment of hyperhidrosis has led to other procedures including ablation/cautery, cutting and clipping of the nerve all of which rep…

Early Evaluation of Thermal Radiofrequency vs. Chemical Neurolysis for T2 and T3 Sympathectomy in Post-Mastectomy Pain Syndrome Using Oximetry-Based Perfusion Index Assessment.

BACKGROUND: Post-mastectomy pain syndrome (PMPS) is a chronic neuropathic condition thought to be mediated mainly by the sympathetic nervous system. Effective treatment options for PMPS include T2 and T3 sympathectomy, performed through either thermal radiofrequency (TRF) or chemical neurolysis. OBJECTIVES: This trial compares the efficacy of pulsed radiofrequency (RF) to that of neurolysis for post-mastectomy pain relief. SETTING: This double-blinded, randomized trial was conducted in the National Cancer Institute of Cairo, Egypt. METHODS: Fifty-four female patients with PMPS that did not …

Sympathectomy-induced increases in calcitonin gene-related peptide (CGRP)-, substance P- and vasoactive intestinal peptide (VIP)-levels in parotid and submandibular glands of the rat.

The neuropeptide contents of rat salivary glands were increased four weeks after sympathetic postganglionic denervation (but not after preganglionic denervation): calcitonin gene-related peptide (CGRP) by 400 and 65% in the parotid and submandibular glands, respectively; substance P by 30% in the submandibular gland; and vasoactive intestinal peptide (VIP) by 30% in the parotid gland. The sensory neurotoxin capsaicin prevented the expected increases of CGRP and substance P in the submandibular glands and of VIP in the parotid glands. The CGRP-increase in the parotid gland was, however, only re

Reversal of thoracic sympathectomy through robot-assisted microsurgical sympathetic trunk reconstruction with sural nerve graft and additional end-to-side coaptation of the intercostal nerves: A case report.

Thoracic sympathectomy to treat palmar hyperhidrosis (PH) has widely been performed. Many patients regret the surgery due to compensatory hyperhidrosis (CH), gustatory hyperhidrosis, arrhythmia, hypertension, gastrointestinal disturbances, and emotional distress. Robotic applications in microsurgery are very limited. We report the technique and long-term patient-reported outcomes of bilateral robot-assisted microsurgical sympathetic trunk reconstruction with a sural nerve graft in an interdisciplinary setting. A 59-year-old female suffered from severe adverse effects after endoscopic thoracic …

Prediction of compensatory hyperhidrosis with botulinum toxin A and local anesthetic.

OBJECTIVE: Compensatory hyperhidrosis (CH) is one of the most problematic complications of sympathectomy, which occurs often and is hard to treat. A predictive procedure (PP) for CH can help patients experience compensatory sweating before sympathectomy to determine whether or not to perform sympathectomy. Our study aimed to evaluate the CH after the PP and sympathectomy in patients with primary palmar hyperhidrosis using multiple drugs. METHODS: We reviewed 83 patients who underwent a PP between July 2009 and August 2013 with primary palmar hyperhidrosis. In group A, we used levobupivacaine …

Temporary thoracoscopic sympathetic block for hyperhidrosis.

BACKGROUND: Compensatory hyperhidrosis (CH) is the most common side effect after a thoracoscopic sympathectomy. The fear of CH is the most common reason why patients do not undergo a sympathectomy, because it is an irreversible procedure unless removal clips are used. Unfortunately, clip removal for reversal of postsympathectomy CH has not been reliable. To address this issue, we developed a new technique of a temporary thoracoscopic sympathetic block that can hopefully predict if postsympathectomy CH is going to occur after sympathectomy for medical refractory primary hyperhidrosis (PH). MET…

Diagnostic and therapeutic application of thoracic epidural in a child with intractable cardiac arrhythmias: A case report.

A 3-year-old boy presented with episodes of uneasiness and transient loss of consciousness. Atrial tachyarrhythmias with rapid ventricular rate was diagnosed and initially unsuccessfully treated with oral antiarrhythmic drugs. Subsequent Holter monitoring revealed ventricular arrhythmias. Despite pharmacologic treatment, he needed numerous cardioversions. Surgical sympathectomy was planned. Initially, sympathectomy was achieved using a continuous high thoracic epidural block and was performed to ascertain the efficacy of the thoracic sympathectomy. This successfully reduced the ventricular arr…

Surgical approaches and techniques in the management of severe hyperhidrosis.

Thoracoscopy has afforded a huge advance for upper thoracic sympathetic procedures compared with prior open procedures. Different clinical syndromes of hyperhidrosis exist and require different forms of treatment. The classic severe palmoplantar pattern of hyperhidrosis will not respond effectively in the long term to any nonoperative treatment and requires sympathectomy for cure. Thoracoscopic sympathectomy is the first-line treatment in these patients. The author's preference is sympathotomy at the second or third rib level, because this method spares the ganglion. It is possible that gangli…

Management of severe ischemia of the foot secondary to occlusive vascular disease.

One hundred patients, 111 limbs, with gangrene of the lower extremity were initially treated with lumbar sympathectomy. Lumbar sympathectomy appears to be most beneficial in the management of gangrene of the toe with a limb salvage rate of 75 per cent. The best results were seen when only one toe, not the big toe, was involved. Limb salvage dropped to 38 per cent for gangrene of the foot, and with gangrene of the leg, lumbar sympathectomy had no affect. The presence of diabetes had no affect on limb salvage for gangrene of the toe but did have an apparent affect on limb salvage for gangrene of…

Does the tibial and sural nerve transection model represent sympathetically independent pain?

Neuropathic pain can be divided into sympathetically maintained pain (SMP) and sympathetically independent pain (SIP). Rats with tibial and sural nerve transection (TST) produce neuropathic pain behaviors, including spontaneous pain, tactile allodynia, and cold allodynia. The present study was undertaken to examine whether rats with TST would represent SMP- or SIP-dominant neuropathic pain by lumbar surgical sympathectomy. The TST model was generated by transecting the tibial and sural nerves, leaving the common peroneal nerve intact. Animals were divided into the sympathectomy group and the s…

Treatment of palmar hyperhidrosis: T(4) level compared with T(3) and T(2).

OBJECTIVE: The aim of this study was to compare the outcomes of 3 different levels of sympathectomy. SUMMARY BACKGROUND DATA: Most surgeons still perform T(2) or T(2-3) sympathectomy for palmar hyperhidrosis, but both these treatments can cause severe side effects. Some recent articles have advocated T(4) sympathectomy and obtained satisfactory results. METHODS: Between January 2000 and August 2004, 234 records of patients treated for palmar hyperhidrosis were retrospectively reviewed. Of them, 86 patients were treated with endoscopic thoracic sympathectomy of T(2) (ETS(2)), 78 patients with…

Surgical management of primary hyperhidrosis.

Primary hyperhidrosis, although lacking a precise definition and of unknown aetiology, disrupts professional and social life and may lead to … idrosis but the role of suction-assisted removal of axillary sweat glands remains to be determined. Sympathectomy remains the standard by which other treatments must be judged. For upper thoracic sympathectomy a variety of surgical approaches are used with satisfactory relief of hyperhidrosis. [Retroperitoneoscopic lumbar symathectomy].

During the period between August 1995 and February 1996 at the Second Surgical Clinic of the Faculty Hospital in Olomouc 12 retroperitonescopic lumbar sympathectomies were performed on account of different indications. The authors present an elaborate technique of access into the retroperitoneum and their own technique of lumboscopic sympathectomy. The success of the method was evaluated clinically and by histological evidence of sympathetic ganglia in the collected material. Retroperitoneoscopic lumbar sympathectomy is considered a gentle and safe method which can be used also in other patien…
