Although mycotic infections of the urinary system occur not infrequently, obstruction of the upper urinary tract because of mycetomas or fungus balls is exceedingly rare. Treatment consists of antifungal treatment and appropriate urinary drainage. In cases resistant to treatment with external drainage and chemotherapy, open surgery is typically performed to remove the mycetoma. However, this can also be achieved percutaneously using mechanical thrombectomy devices, as shown in this case of a 4-month-old infant presenting with renal failure due to bilateral obstructing mycetomas.

PURPOSE: We report our experience with a new percutaneous thrombectomy device for the treatment of thromboembolic occlusions of peripheral arteries.

The transluminal endarterectomy catheter (TEC) system was employed as a percutaneous rotational mechanical thrombectomy device in 1-week-old thrombi in five dogs (five thrombosed polytetrafluoroethylene grafts and four thrombosed native superficial femoral arteries). Luminal patency was restored in all vessels with no angiographic evidence of significant distal embolization. The TEC catheter was also successfully used for recanalization of the superior vena cava in a patient with superior vena cava syndrome.
