Infectious diseases seen in a primary care clinic in Leogane, Haiti.

All diseases diagnosed in a primary healthcare clinic situated in Leogane, Haiti, were recorded prospectively during a 7-month period. Among the patients in this cohort, 2,821 of 6,631 (42.6%) presented with an infectious disease. The three most common syndromes among the patients presenting with infections were respiratory tract infections (33.5%), suspected sexually transmitted diseases--mostly among females with recurrent disease (18.1%)--and skin and soft tissue infections, including multiple cases of tinea capitis (12.8%). Of the 255 patients presenting with undifferentiated fever, 76 (29…

The systematic evaluation of the skin in children.

A large proportion of children seen by primary care pediatricians involve problems related to the skin. In this chapter, the importance of history and, particularly, physical examination in the diagnosis of various skin disorders is discussed. The ability to describe a rash using proper terminology will help narrow the differential and enable one to communicate what is seen to other physicians. The various primary and secondary lesions that may be present will be described along with various dermatoses as examples. Four common skin disorders that often fool the primary care physician are also

In vitro activity of sertaconazole against dermatophyte isolates with reduced fluconazole susceptibility.

We have studied the in vitro antifungal activity of sertaconazole against 114 dermatophytes with low susceptibility to fluconazole following the National Committee for Clinical Laboratory Standards for filamentous fungi (M38-P). However, several important factors such as the temperature (28 vs. 35 degrees C) and time of incubation (4-10 days vs. 21-74 h), have been found to affect dermatophytes. Isolates were recently recovered from human samples. Sertaconazole was active against 114 isolates of 12 fungal dermatophyte species, showing an overall geometric mean of 0.41 microg/ml with a minimum

Structural abnormalities of the hair shaft.

Abnormalities of the hair shaft are varied and often confusing. They do require accurate recognition, which can be helpful in the diagnosis and management of a hair disorder, or in the detection of underlying disease. A reliable evaluation of hair shaft abnormality is dependent on familiarity with the vast structural variations in normal hair, the hair growth cycle, and the changes produced by weathering. These structural abnormalities can be congenital or acquired, and some are associated with hair fragility. The classification used here has the advantage of simplicity and is keyed to easy re

Prevalence of scalp scaling in prepubertal children.

OBJECTIVE: To determine the prevalence and most common etiologies of scalp scaling in infants and prepubertal children and the specificity of head and neck lymphadenopathy for the diagnosis of tinea capitis associated with scalp scaling. DESIGN/METHODS: A cross-sectional study of 300 children, 200 from an urban general pediatric practice and 100 from 2 urban pediatric dermatology practices, was conducted. Half of the subjects were <2 years old, and half were 2 to 10 years old. Demographic data, medical history, and clinical data noting the presence of scalp scaling and other scalp signs and s…

Hair and Nail Conditions: Alopecia Evaluation.

Alopecia affects men and women and can result in significant distress for patients. Alopecias can be categorized as nonscarring or scarring. Nonscarring alopecias include male and female pattern alopecias, alopecia areata, telogen effluvium, traction alopecia, trichotillomania, and tinea capitis. Scarring alopecias include central centrifugal cicatricial alopecia, lichen planopilaris, frontal fibrosing alopecia, discoid lupus erythematosus, dissecting cellulitis of the scalp, folliculitis decalvans, and acne keloidalis nuchae. Evaluation of patients with alopecia involves assessment of the dur…

A Practical Approach to the Diagnosis and Management of Hair Loss in Children and Adolescents.

Hair loss or alopecia is a common and distressing clinical complaint in the primary care setting and can arise from heterogeneous etiologies. In the pediatric population, hair loss often presents with patterns that are different from that of their adult counterparts. Given the psychosocial complications that may arise from pediatric alopecia, prompt diagnosis and management is particularly important. Common causes of alopecia in children and adolescents include alopecia areata, tinea capitis, androgenetic alopecia, traction alopecia, trichotillomania, hair cycle disturbances, and congenital al…

Use of antifungal saponin SC-2 of Solanum chrysotrichum for the treatment of vulvovaginal candidiasis: in vitro studies and clinical experiences.

Saponin SC-2 from Solanum chrysotrichum showed antifungal activity, demonstrated in vitro, which inhibited the growth of dermatophytes, and in vivo, to be effective in the treatment against tinea pedis and pityriasis capitis. Fungistatic and fungicidal activity of saponin SC-2 on Candida albicans and other Candida species, fluconazole and ketoconazole resistaent strains was demostrated. SC-2-associated ultrastructural alterations in several Candida species were observed. An exploratory clinical, randomized, double-blind, and controlled ketoconazole study of ketoconazole was conducted with the

Unusual Presentation of Pemphigus Vulgaris as a Solitary Ulcerative Scalp Lesion: A Case Report.

Pemphigus vulgaris (PV) is a rare autoimmune blistering disease that typically presents with painful oral erosions followed by widespread cutaneous involvement. We report a diagnostically challenging case of PV in a 43-year-old man who initially presented with a solitary, ulcerative-crusted lesion on the parietal scalp, evolving over eight months. The lesion was clinically suggestive of infectious or neoplastic etiologies, including tinea capitis, squamous cell carcinoma, Bowen's disease, and leishmaniasis. Empirical antifungal treatment was ineffective, and mycological studies were negative. …

Diffuse Neurofibroma: An Uncommon Cause of Alopecia.

Alopecia (hair loss) is reported to be associated with infection, genetics, hormonal changes, drugs, or inflammation. The most frequent causes of hair loss in pediatric patients include tinea capitis, alopecia areata, traction alopecia, and trichotillomania. In the adult population, causes to be considered are alopecia areata and hair loss associated with systemic disease and hormonal influence. The clinician must be able to separate the types and causes of hair loss into those that reflect primary dermatologic conditions and those that represent a reaction to systemic disease. Benign cutaneou…

Multiple pigmented basal cell carcinomas arising in the normal-appearing skin after radiotherapy for carcinoma of the cervix.

BACKGROUND: The most frequent radiation-induced skin cancers are basal cell carcinomas (BCCs), and in many cases, they may occur subsequent to tinea capitis, hypertrophic tonsillitis, acne vulgaris, atopic dermatitis, and hyperthyroidosis. BCCs occurring after radiotherapy for carcinoma of the cervix are extremely rare. OBJECTIVE: To describe a case of multiple pigmented BCCs caused by radiotherapy for carcinoma of the cervix. RESULTS: A 76-year-old female presented with four tumoral lesions within a previously irradiated area. The patient had undergone radiation therapy for cervical cancer …
