Risk factors associated with moderate and severe traumatic brain injury mortality in admitted older adults: a retrospective, single-center study at a level II trauma center.

PURPOSE: Approximately one in four adults aged ≥65 years experiences a fall each year, and over one-third of these events require medical care. Older adults are at higher risk of complications because of comorbidities. Most prior studies have focused on incidence rather than outcomes, and few have evaluated the association between age and traumatic brain injury (TBI) mortality. This study assessed risk factors and risk-adjusted mortality and morbidity among older adults with moderate or severe TBI. METHODS: We conducted a retrospective review of data from a single-center trauma registry. Incl

Changes in the clinical features and demographics of donors after brain death, before and after the establishment of a regional trauma center: 20 years of experience at a single center in Korea.

PURPOSE: Organ transplantation is considered the definitive treatment for end-stage organ disease. However, the scarcity of donor organs compared to the number of patients awaiting transplants is a major barrier. This study aimed to assess the impact of a regional trauma center on organ procurement and to provide a basis for future collaboration between regional trauma centers and transplant centers. METHODS: This retrospective study analyzed organ donors after brain death over a 20-year period from January 1, 2003, to December 31, 2022. It compared patients before and after the establishment…

Ketamine as primary anesthetic for upper limb trauma during war: a case series of 100 surgeries at Rafik Hariri University Hospital, Lebanon's National War Trauma Referral Center.

BACKGROUND: The Beirut pager explosions on September 17, 2024 resulted in mass casualties with severe upper limb trauma admitted to Rafik Hariri University Hospital (RHUH), Lebanon's national war trauma referral center. Initial opioid-based anesthesia was associated with postoperative respiratory complications and high opioid requirements. Ketamine became available the following day through the International Committee of the Red Cross (ICRC). This study evaluates the transition to ketamine as a primary anesthetic and its effects on pain control, hemodynamics, respiration, and opioid use. METH

Principles of trauma-informed care evident in person centered care for persons living with dementia in low resource settings.

OBJECTIVES: This study identified how long-term care stakeholders describe trauma-informed care (TIC) practices in low resource settings. METHOD: This was a secondary data analysis about person-centered care (PCC) for persons living with dementia (PLWD). The parent study purposively recruited administrators, direct care staff, PLWD, and care partners in two nursing homes and two assisted living communities in urban and rural federally designated medically underserved areas. In depth semi-structured interviews were conducted about PCC; the parent study did not include TIC-specific probes. Resp

Learning curve analysis of the anterior intrapelvic approach for pelvic ring and acetabular fractures during implementation in a trauma center.

PURPOSE: The anterior intrapelvic approach (AIP) is increasingly used for pelvic ring and acetabular fractures, yet its learning curve during structured implementation remains poorly defined. This study evaluated the learning curve associated with AIP introduction in a trauma center. METHODS: A retrospective analysis of a prospectively maintained database was conducted at a level-1 trauma center. Eighty-six consecutive patients treated with AIP by a single senior surgeon (2015-2019) were included. Patients were divided chronologically into two equal groups (first 43 vs. last 43 cases). Diagnostic yield and clinical predictors of blunt cerebrovascular injury: A CTA utilization review at a level I trauma center.

PURPOSE: Blunt cerebrovascular injury (BCVI) is a potentially devastating complication of high-energy trauma that can lead to ischemic stroke if undetected. Computed tomography angiography (CTA) is the primary screening modality, though optimal patient selection criteria remain debated. Rising CTA utilization at our institution prompted a quality improvement review to evaluate the diagnostic yield and identify clinical predictors of BCVI. METHODS: A retrospective review was conducted of trauma patients who underwent CTA of the head and neck at a Level I trauma center between January and Decem

Road traffic injuries in Kandy District, Sri Lanka: a prospective analysis of trauma and disability at a tertiary care center.

BACKGROUND: Road traffic injuries (RTIs) are a leading cause of mortality and disability globally, with the greatest burden borne by low- and middle-income countries (LMICs). Sri Lanka continues to experience substantial RTI burden, yet prospective data describing injury characteristics and short-term functional outcomes outside the capital remain limited. We aimed to describe patterns of injury and trauma severity, modes of transport and access to care, and short-term functional outcomes at discharge and during early follow-up among patients presenting with road traffic injuries to a tertiary
