Pathophysiological aldosterone levels modify the secretory activity of cardiac progenitor cells.

Cardiac progenitor cells (CPCs) trigger regenerative processes via paracrine mechanisms in response to changes in their environment. In the present study we explored alterations in the secretory activity of CPCs induced by raised aldosterone levels symptomatic for heart failure. The cytokine profile of the supernatant of CPCs that were treated with the mineralocorticoid showed an induction of interleukin-6 secretion. Mass spectrometric analyses revealed an increase in the abundance of secreted proteins associated with regeneration and cell migration like gelsolin and galectin-1. Differential r

Infectious agents in the pathogenesis of primary biliary cirrhosis.

Primary biliary cirrhosis (PBC) is a chronic progressive cholestatic liver disease which is characterized by the breakdown of self-tolerance to the highly conserved pyruvate dehydrogenase complex, specially the pyruvate dehydrogenase E2 complex (PDC-E2). The breakdown of the tolerance to such antigens leads to an autoimmune process characterized by portal inflammation and immune-mediated destruction of the intrahepatic bile ducts. Epidemiological studies have suggested that infections agents can trigger or even exacerbate the disease. Among other gram negative bacteria, Escherichia Coli, and N

Scapular instability associated with brachial plexus irritation: a proposed causative relationship with treatment implications.

Brachial plexus irritation and other compression neuropathies can be diverse in their presentations and can cause a myriad of signs and symptoms. The purpose of this paper is to review the pertinent anatomy, kinesiology, and neurophysiology and to outline one possible cascade of events that may contribute to more diffuse upper extremity symptoms. Scapular instability and local myofascial trigger points resulting in possible secondary muscle imbalances are described. Their possible relationship to brachial plexus irritation in addition to the implications of the irritation are also discussed. T…

Risk analysis of power surging in novel power systems: a hybrid framework driven by accident chain and WPMixer.

To address the complex dynamic power risks in novel power systems induced by the superimposition of "three-high" characteristics (high renewable energy penetration, high direct current infeed ratio, and high load density) and extreme weather, this paper proposes the concept of "power surging" alongside a comprehensive risk analysis and planning methodology. Power surging is defined as the random and drastic variations in power flows across voltage levels or regions, and it is classified into longitudinal and transverse surging based on propagation directions. To overcome the limitations of tra

Dissecting Airborne Allergens.

Asthma is a heterogeneous and very complex group of diseases, and includes different clinical phenotypes depending on symptoms, progression, exacerbation patterns, or responses to treatment, among other characteristics. The allergic phenotype is the most frequent, especially in pediatric asthma. It is characterized by sensitization (the production of specific IgEs) to allergens and frequent comorbidity with rhinitis as well as atopic dermatitis. Given the complexity of allergic asthma, knowledge of it must be approached from different points of view: clinical, histological, physiological, epid

Eruptive xanthomas: a case report with literature review focusing on pathogenesis and clinical management.

A typical case of eruptive xanthomas secondary to untreated type 2 diabetes mellitus complicated with severe hyperlipidemia was reported. The patient had a history of acute pancreatitis one month before the onset of xanthomas. Laboratory tests revealed high levels of triglycerides, cholesterol, blood glucose, and acute inflammatory markers. Histopathological examination showed the foam cells in the dermis. After interventions, including a low-fat and low-carbohydrate diet, anti-diabetic therapy and lipid-lowering therapy, the patient's skin lesions partially resolved within two months. At the

A systematic review and network meta-analysis of randomized controlled trials on the treatment of trigger finger.

This systematic review and network meta-analysis evaluated the comparative efficacy of various interventions for trigger finger relative to open surgery. A total of 24 randomized controlled trials involving 1,660 digits were included. In the short term, corticosteroid injection (SMD = -3.81; 95% CI: -6.09 to -1.53), hyaluronic acid injection (SMD = -3.13; 95% CI: -5.86 to -0.40), and percutaneous release combined with splint use (SMD = -3.07; 95% CI: -5.36 to -0.78) significantly reduced disease severity compared with open surgery. In the long term, corticosteroid injection (SMD = -4.49; 95% C

Sialyl Lewisx Glycomimetics as E- and P-Selectin Antagonists Targeting Hyperinflammation.

Inflammatory disorders, such as sepsis, pancreatitis, and severe COVID-19, often cause immune dysfunction and high mortality. These conditions trigger excessive immune cell influx, leading to cytokine storms, organ damage, and compensatory immune suppression that results in immunoparalysis, organ dysfunction, and reinfection. Controlled and reversible immunosuppression limiting immune cell recruitment to inflammation sites could reduce hyperinflammation and prevent immune exhaustion. PSGL-1 on leukocytes binds to vascular P- and E-selectins via its sialyl Lewisx pharmacophore, triggering key f

Zoster-Associated Prothrombotic Plasma Exosomes and Increased Stroke Risk.

Herpes zoster (HZ; shingles) caused by varicella zoster virus reactivation increases stroke risk for up to 1 year after HZ. The underlying mechanisms are unclear, however, the development of stroke distant from the site of zoster (eg, thoracic, lumbar, sacral) that can occur months after resolution of rash points to a long-lasting, virus-induced soluble factor (or factors) that can trigger thrombosis and/or vasculitis. Herein, we investigated the content and contributions of circulating plasma exosomes from HZ and non-HZ patient samples. Compared with non-HZ exosomes, HZ exosomes (1) contained

Homeostatic plasticity in the retina.

Vision begins in the retina, whose intricate neural circuits extract salient features of the environment from the light entering our eyes. Neurodegenerative diseases of the retina (e.g., inherited retinal degenerations, age-related macular degeneration, and glaucoma) impair vision and cause blindness in a growing number of people worldwide. Increasing evidence indicates that homeostatic plasticity (i.e., the drive of a neural system to stabilize its function) can, in principle, preserve retinal function in the face of major perturbations, including neurodegeneration. Here, we review the circum

Lipids uniquely alter rates of insulin aggregation and lower toxicity of amyloid aggregates.

Amyloid formation is a hallmark of many medical diseases including diabetes type 2, Alzheimer's and Parkinson diseases. Under these pathological conditions, misfolded proteins self-assemble forming oligomers and fibrils, structurally heterogeneous aggregates that exhibit a large variety of shapes and forms. A growing body of evidence points to drastic changes in the lipid profile in organs affected by amyloidogenic diseases. In this study, we investigated the extent to which individual phospho- and sphingolipids, as well as their mixtures can impact insulin aggregation. Our results show that l

Anterior hypopituitarism secondary to biopsy-proven IgG4-related hypophysitis in a young man.

IgG4-related disease (IgG4-RD) is an immune-mediated fibro-inflammatory condition which can affect various organs including the pituitary gland. The true annual incidence of this condition remains widely unknown. In addition, it is unclear whether IgG4 antibodies are causative or the end result of a trigger. With no specific biomarkers available, the diagnosis of IgG4-related hypophysitis remains a challenge. Additionally, there is a wide differential diagnosis. We report a case of biopsy-proven IgG4-related hypophysitis in a young man with type 2 diabetes mellitus. Learning points: IgG4-relat

Up-Regulation of Antimicrobial Peptides Gallerimycin and Galiomicin in Galleria mellonella Infected with Candida Yeasts Displaying Different Virulence Traits.

Galleria mellonella has been described as a cheap and an easy-to-reproduce model for the study of fungal infections. We hypothesized that yeasts with higher virulence potential decrease survival and significantly trigger an immune response in G. mellonella through the regulation of innate immunity-related genes encoding antimicrobial peptides (AMPs) such as gallerimycin and galiomicin. Candida albicans SC5314 and Candida dubliniensis CBS 7987, selected because of their different virulence potential, were used for a killing assay followed by the determination of gene expression using qPCR. The Use of Laser Therapy for Patients with Fibromyalgia: A Critical Literary Review.

The management of pain - despite many anaesthetic drugs - remains to be an urgent task. If the goal is to achieve permanent pain relief - and not to temporarily mask the pain (masking pathology) - then this goal can only be achieved by treating pain with physiotherapeutic methods, the most universal and effective of which, is low level laser therapy (LLLT). The treatment methods vary fundamentally in the case of neuropathic (nonspecific, primary) pain, which includes pain with the localization of the trigger points (TP) and nociceptive (specific, secondary), which includes all types of pain re…

Chronic refractory myofascial pain and denervation supersensitivity as global public health disease.

Chronic pain with a 30.3% global prevalence significantly impacts universal health. Low back pain has a 9.4% prevalence worldwide causing the most widespread disability. Neck pain ranks 4th highest regarding years lived with disability with a 4.9% prevalence worldwide. The principal cause of pain in 85% of patients visiting a tertiary pain clinic has a myofascial origin. The root cause is multifocal neuromuscular ischaemia at myofascial trigger points from muscle tightening and shortening following spondylotic radiculopathy induced partial denervation. Chronic refractory myofascial pain (CRMP)…
