Transcutaneous Teflon injection of the paralyzed vocal cord: a new technique.

Because of anatomical deformity, trismus, or for other reasons, it may on occasion be impossible to visualize a larynx by the usual laryngoscopy methods. Such difficulties in patients who have paralytic dysphonia may also make it impossible to effect improved vocal cord closure by the usual techniques of Teflon injection. We have applied a new technique, detailed in this report, to these problem cases. Following topical anesthesia of the nose, nasopharynx, and larynx, 1% Xylocaine is injected over the cricothyroid membrane. A flexible or telescopic laryngoscope connected to a television camera…

Tetanus: a review.

Tetanus is caused by the organism Clostridium tetani, which produces tetanospasmin, a neurotoxin responsible for the clinical manifestations of muscle rigidity and reflex spasms. The majority of cases follow an anaerobic wound infection associated with trauma. Incubation period is usually 3 days to 3 weeks. 75% of patients present with trismus. Reflex spasms are seen in 70% of patients and characterize the severity of the disease. Treatment involves removal of the offending organism, neutralization of free neurotoxin, controlling rigidity and reflex spasm, and minimizing complications. Diazepa…

Chondrosarcoma of the mandibular condyle.

Our report details an unusual case of chondrosarcoma in a man in his late 50s who presented with symptoms of pain, swelling and trismus. He was initially diagnosed with temporomandibular joint disorder (TMD) and managed conservatively. With time, symptoms progressed, so a cone beam CT scan was advised. The scan revealed an expansile lytic lesion in the condylar region with cortical erosion. It showed heterogeneous density with ring arch calcification around the condylar head, which was suggestive of the cartilaginous lesion. Microscopic examination showed a tumour composed of lobules of atypic…

Optimizing the Synergistic Role of Diode Laser and Drug Therapies in the Management of Oral Submucous Fibrosis: A Case Report.

Oral submucous fibrosis (OSMF) is one of the most common precancerous conditions of the oral mucosa involving any part of the oral cavity resulting in tissue scarring, dysphagia, and trismus. Habits of chewing areca nut, tobacco, and pan masala and smoking have revealed a strong association with the occurrence of OSMF. Due to its high morbidity and high malignant transformation rate (MTR), continuous efforts have been made to develop effective treatment options. Based on the stage of the disease, various treatment modalities such as medical and surgical therapies are available, each with its o…

The Role of Ultrasound in Pediatric Peritonsillar Infection: A Descriptive Analysis.

In this study, we describe the role of ultrasound in diagnosing pediatric peritonsillar abscesses (PTAs). A retrospective chart review was conducted on 54 children aged 4 to 17 years who had an ultrasound performed for suspected PTA. Based on ultrasound imaging, the patients were classified into 2 groups: PTA-positive (8, 14.8%) and PTA-negative (46, 85.2 %). Trismus was significantly associated with PTA (50% vs. 13%, P = .03). PTA-positive patients were more likely to be given steroids, be admitted, and have extended hospital stays (P = .04, .004, and .002, respectively). The 2 groups had no …

Recognizing a Peritonsillar Abscess in Pediatric Patients.

A peritonsillar abscess (PTA) is a localized collection of pus in the peritonsillar space, between the palatine tonsillar capsule and the superior pharyngeal constrictor muscle (G. Gupta & R. McDowell, 2022). It is the most commonly occurring abscess in the head and neck region. Patient presentation usually includes odynophagia, unilateral otalgia, trismus, and voice changes. Managing a pediatric patient may be challenging, as children may not be able to describe their history of illness and symptoms. The management of a PTA can also differ between pediatric and adult patients (S. Ahmed Ali et…

Management of difficult airway in a pregnant patient with severely reduced mouth opening.

Pregnancy is associated with a wide variety of oral and dental changes ranging from gingivitis to odontogenic infections. If left untreated, severe dental abscess can progress to Ludwig's angina, which is a potentially lethal cellulitis that rapidly extends to the neck region and may lead to life-threatening upper airway obstruction. We report the case of a pregnant woman who presented with intense throbbing pain, trismus and severely reduced mouth opening due to dental abscess for the last one week. She required incision and drainage of abscess with extraction of third molar under general ana…

Multidisciplinary management of temporomandibular joint ankylosis in an adult: journey from arthroplasty to oral rehabilitation.

Ankylosis of the temporomandibular joint is a debilitating condition resulting in progressive trismus and facial disfigurement. Common trigger factors include paediatric mandibular trauma, middle ear infection or traumatic childbirth. Although diminishing in incidence among urban population, it is still prevalent in the underprivileged world. Substandard child safety norms, delayed presentation, lack of access to specialties like maxillofacial surgery in rural areas and absence of follow-up contribute to ankylosis. Afflictions in ankylosis are multipronged, involving aesthetic, functional, psy…
