Congenital tuberculosis.

A neonate presented with poor feeding, jaundice and hepatosplenomegaly. He was in liver failure with disseminated coagulopathy. Post-mortem liver biopsy revealed discrete granulomas with central necrosis and acid-fast bacilli on Ziehl-Nielsen's stain. The mother had active tuberculosis at delivery. The principal characteristics of congenital tuberculosis are reemphasized in this observation.

[The risk of infections in internationally adopted children is real].

Each year approximately 400 internationally adopted children (IAC) come to Denmark from countries with high prevalences of infectious diseases. Former studies of IAC have shown that approximately 1% of the IAC are infected with Hepatitis A, 3-6% with Hepatitis B, mostly asymptomatic infections. 8-27% have gastrointestinal parasites and 1-7% have tuberculosis, but 3-21% have a positive Mantoux test. The studies found none with HIV or HCV and only a few with syphilis. The vaccination status was often deficient and the serology results did not match the vaccination records. All studies conclude t

[Recommendations for therapy, chemoprevention and chemoprophylaxis of tuberculosis in adults and children. German Central Committee against Tuberculosis (DZK), German Respiratory Society (DGP)].

Several new international recommendations have been published since the German Central Committee against Tuberculosis (DZK) published its recommendations for drug treatment of tuberculosis (TB) in 2001 and for chemoprevention of latent tuberculosis infection (LTBI) in 2004. These international publications have been integrated in the present new recommendations which describe both the treatment of active TB and preventive treatment, pointing out specific adaptations for Germany. Separate sections deal with the current management of mono-, poly-, and multiresistance or drug intolerance, of TB i

Unexpectedly low drug exposures among Ugandan patients with TB and HIV receiving high-dose rifampicin.

We characterized the pharmacokinetics of standard- and high-dose rifampicin in Ugandan adults with tuberculosis and HIV taking dolutegravir- or efavirenz-based antiretroviral therapy. A liver model with saturable hepatic extraction adequately described the data, and the increase in exposure between high and standard doses was 4.7-fold. This was lower than what previous reports of dose-exposure nonlinearity would predict and was ascribed to 38% lower bioavailability of the rifampicin-only top-up formulation compared to the fixed-dose combination.

Peliosis hepatis in a young adult bodybuilder.

Peliosis hepatis, an unusual condition characterized by blood-filled spaces within the hepatic parenchyma, usually occurs in a variety of chronic wasting diseases such as tuberculosis or malignancy. It has been described in patients undergoing therapy with anabolic steroids in pharmacologic doses over extended periods of time. To our knowledge it has not been described in athletes who utilize them for performance enhancement. This report concerns a 27-yr-old bodybuilder with peliosis hepatis, which developed after chronic intermittent use of anabolic steroids.

Hepatosplenic Tuberculosis on Pretherapy and Posttherapy FDG PET/CT.

A 65-year-old woman underwent FDG PET/CT for presumed hepatosplenic malignancy suggested by an abdominal CT. The images revealed multiple foci of intense FDG activity in both the liver and the spleen. However, a splenic biopsy result revealed no malignant cells, and either tuberculosis or sarcoidosis was proposed. Following an incomplete antituberculosis therapy, a repeat FDG PET/CT showed resolution of the abnormal activity in the liver and the spleen.

Isolated tuberculous liver abscess in immunocompetent children - report of two cases.

Isolated tuberculous liver abscess (TLA) without active pulmonary or miliary tuberculosis, or other clinical evidence of tuberculosis, is distinctly rare and only few cases have been reported in the literature. We report two cases of isolated TLA in immunocompetent children, treated successfully by percutaneous aspiration followed by systemic antituberculous drugs.

Hepatosplenic Tuberculosis Mimicking Metastatic Malignancy and Immune Reconstitution Inflammatory Syndrome (IRIS) in an HIV-Positive Patient.

A man in his late 30s, recently diagnosed with HIV-1, presented with persistent fever, weight loss, and fatigue, despite prior treatment for immune reconstitution inflammatory syndrome (IRIS). Imaging studies revealed splenomegaly, hepatic and splenic micronodular lesions, and pulmonary micronodules. A liver biopsy, pivotal in establishing the diagnosis, demonstrated multiple well-formed epithelioid granulomas with focal central necrosis, indicative of an infectious etiology. Polymerase chain reaction (PCR) analysis of the biopsy fragment confirmed the presence of Mycobacterium tuberculosis. Zoonotic transmission of the Mycobacterium tuberculosis complex between cattle and humans in Central Ethiopia.

INTRODUCTION: The zoonotic transmission of tuberculosis (TB) from cattle to humans has long been recognized, while the reverse transmission from humans to animals has only recently been reported. The socioeconomic conditions in rural Ethiopia are conducive to the zoonotic and reverse zoonotic transmission of TB between cattle and humans. This study aimed to explore the transmission of the Mycobacterium tuberculosis complex between cattle and humans in Central Ethiopia. METHODS: To achieve this objective, a cross-sectional study was conducted involving 1,896 cattle slaughtered at two abattoirs

Imported transmissible diseases in minors coming to Spain from low-income areas.

We prospectively studied the prevalence of imported transmissible diseases in 373 immigrant children and adolescents coming from Sub-Saharan Africa, North Africa and Latin America to Salamanca, Spain. The most frequent transmissible diseases in this group were latent tuberculosis (12.7%), chronic hepatitis B virus infection (4.2%), hepatitis C virus infection (2.3%), syphilis (1.5%) and human T-lymphotropic virus type 1 or 2 infections (1.4%). A total of 24.2% of patients had serologic profiles suggesting past hepatitis B virus infection. Anti-human immunodeficiency virus antibodies were not d

[Infectious complications of biological therapy].

Biological treatment represents a significant progress in the therapy of many serious diseases. Together with the growing knowledge of pathophysiology and subsequent development of new therapeutic agents, this progress will definitely lead to further expansion of biologics. Since biologics interfere with many mechanisms of host defence, which may sometimes be compromised by them, increased risk of infectious complications must be taken into account. Patients treated with biologics are prone to classical virulent infections (e.g. listeriosis, legionellosis and tuberculosis) and opportunistic in

The infection hazards of human cadavers.

Cadavers may pose infection hazards to people who handle them. None of the organisms that caused mass death in the past--for example, plague, cholera, typhoid, tuberculosis, anthrax, smallpox--is likely to survive long in buried human remains. Items such as mould spores or lead dust are much greater risks to those involved in exhumations. Infectious conditions and pathogens in the recently deceased that present particular risks include tuberculosis, group A streptococcal infection, gastrointestinal organisms, the agents that cause transmissible spongiform encephalopathies (such as Creutzfeldt-
