Trapped lung.

Trapped lung is one of the outcomes of fibrinous or granulomatous pleuritis and is a cause of chronic, benign, unilateral pleural effusion. It is characterized by inability of the lung to expand and fill the thoracic cavity due to a restricting fibrous visceral pleural peel. The resulting chronic pleural space is fluid filled, and the persistence of the fluid is solely due to hydrostatic equilibrium. Historically recognized as a complication of therapeutic pneumothorax for treatment of tuberculosis, it is today most commonly a consequence of inadequately treated parapneumonic effusion, but it

Epidemiology of extrapulmonary tuberculosis in the United States, 1993-2006.

BACKGROUND: Almost one-fifth of United States tuberculosis cases are extrapulmonary; unexplained slower annual case count decreases have occurred in extrapulmonary tuberculosis (EPTB), compared with annual case count decreases in pulmonary tuberculosis (PTB) cases. We describe the epidemiology of EPTB by means of US national tuberculosis surveillance data. METHODS: US tuberculosis cases reported from 1993 to 2006 were classified as either EPTB or PTB. EPTB encompassed lymphatic, pleural, bone and/or joint, genitourinary, meningeal, peritoneal, and unclassified EPTB cases. We excluded cases wi

Study of paradoxical response to chemotherapy in tuberculous pleural effusion.

BACKGROUND: Paradoxical worsening of disease, in spite of effective chemotherapy for tuberculosis, has been reported to occur in cases of intracranial tuberculoma, lymph node, and pulmonary tuberculosis. However, only rare case reports describe such paradoxical response in tuberculosis pleurisy. METHODS: Sixty-one patients with proven tuberculous pleural effusion were retrospectively screened in Riyadh, Saudi Arabia, in three major hospitals to look systematically at the incidence and features of paradoxical response. RESULTS: Paradoxical increase in the size of the effusion was detected in

HLA-B27, Mantoux Test, and Eosinophil Count: Help in the Diagnosis of Reactive Arthritis Due to Tubercular Hypersensitivity.

Reactive arthritis (ReA) is a form of inflammatory arthritis usually triggered by gastrointestinal/genitourinary infections. We present the case of a middle-aged man with HLA-B27-associated ReA and tuberculosis (TB) hypersensitivity, manifesting as polyarthritis, inflammatory low back pain, and pleural effusion. The patient had a three-year history of progressive joint pain, initially involving the knees, followed by the bilateral ankle joints and small joints of the hands, wrists, shoulders, elbows, and axial skeleton. He also had a prolonged fever with an exudative pleural effusion, eosinoph

Diagnostic value and safety of medical thoracoscopy in tuberculous pleural effusion.

BACKGROUND: Differentiating tuberculous pleural effusion from other lymphocytic pleural effusions is often challenging. This retrospective study aimed to assess the efficacy and safety of medical thoracoscopy in patients with suspected tuberculous pleural effusion. METHODS: Between July 2005 and June 2014, patients with pleural effusions of unknown etiologies underwent medical thoracoscopy in our institute after less invasive means of diagnosis had failed. Demographic, radiographic, procedural, and histological data of patients with tuberculous pleural effusion were analyzed. RESULTS: During

Mycobacterium-induced transmesothelial migration of monocytes into pleural space: role of intercellular adhesion molecule-1 in tuberculous pleurisy.

The pleural mesothelium is a dynamic cellular membrane with multiple key functions. It plays a pivotal role in pleural inflammation through its release of several cytokines and the expression of cell-surface molecules. The expression of intercellular adhesion molecule (ICAM)-1 in the pleural mesothelium of patients with active pleural tuberculosis and the role of ICAM-1 in monocyte transmigration across pleural mesothelium during tuberculous inflammation was investigated. Results indicate pleural mesothelial cells (PMCs) express ICAM-1 in tuberculous pleuritis. When PMCs were stimulated with b

Polymerase chain reaction of pleural biopsy specimens for rapid diagnosis of tuberculous pleuritis.

SETTING: Government-administered regional teaching hospital. OBJECTIVE: To improve timeliness and sensitivity of laboratory diagnosis of tuberculous pleuritis. DESIGN: We applied polymerase chain reaction (PCR) to detect DNA (IS6110) specific for Mycobacterium tuberculosis complex in pleural biopsy specimens. RESULTS: Of 28 patients with pleural disease, 11 were diagnosed by microbiology (smear/culture of sputum or pleural fluid) with tuberculous pleuritis, eight were diagnosed with tuberculous pleuritis by histology (of pleural biopsies) and/or clinical presentation, and nine were diagnose

Carbon-laden macrophages in pleural fluid of crack smokers.

Carbon-laden macrophages in bronchoalveolar lavage have been noted to be associated with a history of crack smoking. We report herein the finding of carbon-laden macrophages in four cytological preparations of pleural fluid from two crack smokers. The etiology of the two patients' pleural effusions differed; neither had a bronchopleural fistula. Patient 1 had AIDS, Pneumocystis carinii pneumonia, and Kaposi's sarcoma of the right lung with an associated bilateral pleural effusion. Patient 2 was HIV seropositive, had pulmonary tuberculosis, hepatitis A, B, and C, cardiomyopathy, pulmonary embol

IgG4-Related Pleuritis With No Other Organ Involvement.

A 65-year-old man was admitted for productive cough and dyspnea. Bilateral pleural effusions were observed on chest X-ray. Although the bilateral pleural effusions were exudative with an increased number of lymphocytes, bacterial culture and polymerase chain reaction analysis for Mycobacterium tuberculosis were negative. Immunological examinations showed high levels of immunoglobulin G4 (IgG4) in both serum and pleural effusion fluid. Pathologic evaluation of a left pleural biopsy specimen using hematoxylin and eosin staining and immunohistochemical staining showed fibrosis-associated lymphopl

Pleural effusion from blastomycetes in an adult Nigerian: a case report.

A 37 year old hide and skin trader in Northern Nigeria, presented with massive right sided purulent pleural effusion. This started with cough, chest pain and fever for 5 weeks following endotracheal intubation. He had chest tube inserted and was treated with antituberculous drugs for 10 weeks without improvement. Fungal studies on the sputum, and pleural fluid yielded Blastomycetes dermatitidis which responded excellently to ketoconazole and saline pleural lavage. This experience underscores the need for early suspicion of systemic mycosis in suspected cases of tuberculosis with poor or no res

[Asbestosis. Apropos of one case].

Having reported a very complete observation of a patient with asbestosis, as well as silicosis, hitherto undiscovered tuberculosis and a pleural mesothelioma, the authors sum up our present knowledge on the consequences of inhaling asbestos dust. Clinical signs are numerous, benign or malignant, the risks of pleural and broncho-pulmonary cancer being greater in people exposed to asbestos. Diagnosis rests mainly on professional anamnesis and on the presence of great quantities of asbestos bodies in the sputum. Any pleural thickening on X ray should always suggest the development of a mesothelio

[Eosinophilic pleurisy (a study of 27 cases)].

After reviewing the specific literature mainly on cytokines role in eosinophilic pleural chemotaxis, the study on 27 cases of eosinophilic pleurisy is presented. Only 3 cases were malignant pleurisies. The ratio between blood and pleural fluid eosinophils has no semiologic significance. Tuberculosis and parasitoses have no special significance. Autonomous eosinophilic pleurisies are regarded as a distinct nosologic entity. Their prognosis is favourable. The glucocorticoids are the choice treatment.

Pleural fluid lysozyme in human disease.

Lysozyme content was measured in the plasma and pleural fluid of 110 patients with pleural effusions of various causes. The concentration of pleural fluid lysozyme was significantly higher (P less than .001) in patients with tuberculous pleurisy than in those with primary pulmonary carcinoma, metastatic carcinoma of the lung, connective tissue disease, nonspecific pleurisy, or congestive heart failure. Tuberculous patients also had a significantly higher (P less than .001) pleural fluid-to-plasma lysozyme ratio than did the other patients. Plasma lysozyme activity did not differ significantly

Mesenchymal chondrosarcoma - A rare cause of pleural effusion.

Mesenchymal chondrosarcoma is a malignant neoplasm arising from cartilaginous bone or soft tissue. It is uncommon yet devastating. Our patient was a 21-year-old man who presented with pleuritic chest pain and weight loss. His chest radiograph showed left pleural effusion. His pleural effusion analysis was consistent with exudative pleural effusion. Tuberculosis workup was negative. Pleural fluid cytology did not yield malignant cells. Subsequently, his computed tomography of thorax showed left rib sclerotic lesion with soft tissue component. Biopsy of the soft tissue eventually confirmed the d

High-resolution computed tomography findings of pulmonary tuberculosis in lung transplant recipients.

OBJECTIVE: Respiratory infections constitute a major cause of morbidity and mortality in solid organ transplant recipients. The incidence of pulmonary tuberculosis is high among such patients. On imaging, tuberculosis has various presentations. Greater understanding of those presentations could reduce the impact of the disease by facilitating early diagnosis. Therefore, we attempted to describe the HRCT patterns of pulmonary tuberculosis in lung transplant recipients. METHODS: From two hospitals in southern Brazil, we collected the following data on lung transplant recipients who developed pu

[Extrapulmonary tuberculosis].

In spite of the lung being the target organ par excellence of tuberculosis, any other organ and system can be affected. In this article we review the forms of extrapulmonary tuberculosis, with the exception of the pleural form that requires the use of good diagnostic skills. From the dreaded tuberculous meningitis, by way of the insidious affectation of the spondylodiscitis, the so-called ganglionary affectation , the genitourinary affectation, pericarditis, and ending with less frequent forms such as ocular and cutaneous tuberculosis. In each section we will indicate what is most characterist

Spontaneous pneumothorax: a rare complication of miliary tuberculosis.

Pneumothorax is a rare complication of miliary tuberculosis. In this report, a 25-year old patient developing pneumothorax while on the treatment for miliary tuberculosis treatment is presented and the related literature has been reviewed. Pneumothorax, although rare, should be considered when a patient with miliary tuberculosis develops a sudden, severe pain on either side of the chest with breathlessness. The treatment should be tailored according to the size of pneumothorax either pleural drainage through chest tube or needle aspiration. Meanwhile, antituberculous treatment should be contin
