A Small Number of Surgeons Perform the Large Majority of Uncommon Nerve Decompression Procedures.

BACKGROUND: Notable surgeon-to-surgeon variation in rates of uncommon surgery can reflect appropriate concentration of expertise with technically difficult or risky procedures that address problematic impairment due to objective pathophysiology. Examples include vascularized tissue transfer or transplantation to address complex tissue loss and release of bony elbow ankylosis. Perhaps more problematic is notable variation in straightforward, discretionary surgeries intended to alleviate pain, offered in the absence of objectively measurable pathophysiology, and without experimental evidence of

Long-Term Patient-Reported Outcomes After Release of the Ulnar Nerve in Guyon's Canal.

BACKGROUND: There are no large studies describing patient-reported outcomes after ulnar tunnel release (UTR). The aims of this study are to describe the causes of ulnar tunnel syndrome (UTS), the incidence of UTR, and identify factors associated with long-term patient-reported outcomes after UTR. METHODS: We reviewed the medical charts of 76 adult patients who had an UTR for UTS at 1 of 5 academic medical centers between January 1, 2003 and January 1, 2017. Of these patients, 30 completed a follow-up questionnaire including the PROMIS Upper Extremity (PROMIS-UE), Numerical Rating Scale for Pa

Does the Surgical Assistant Influence Perioperative Outcomes Surrounding Cubital Tunnel Surgery?

Background: Ulnar neuropathy at the elbow is the second most common upper extremity compressive neuropathy and surgical treatment often involves surgical trainee involvement. The primary aim of this study is to determine the effect of trainees and surgical assistants on outcomes surrounding cubital tunnel surgery. Methods: This retrospective study included 274 patients with cubital tunnel syndrome who underwent primary cubital tunnel surgery at two academic medical centres between 1 June 2015 and 1 March 2020. The patients were divided into four main cohorts based on primary surgical assistant

Nerve potentials and afferent conduction velocities in the differential diagnosis of amyotrophy of the hand.

Nerve action potentials recorded over the elbow after stimulation of the wrist were studied in patients with hand amyotrophy of different origin. In amyotrophic lateral sclerosis the amplitude of nerve action potentials does not change significantly even with extreme muscle wasting and loss of motor units.Studies of multisegmental afferent conduction velocities may demonstrate a strictly localized lesion with normal values above the compression.

Bifid median nerve in patients with carpal tunnel syndrome.

OBJECTIVE: The aim of this study was to investigate the frequency of the anatomic variation of a bifid median nerve in patients with carpal tunnel syndrome (CTS) and to determine the size criteria for sonography. METHODS: On axial sonograms of 320 hands of 170 patients with CTS and 240 hands of 120 unaffected individuals, the median nerve was evaluated morphologically for a bifid median nerve, and the cross-sectional area was measured at 3 levels (radial-ulnar junction, pisiform, and hook of the hamate). Electrophysiologic studies were performed in addition to clinical and sonographic evaluat

Unusual olecranon mass with ulnar nerve compressive neuropathy caused by a haemophilic pseudotumour.

A 55-year-old man with a history of haemophilia A and bilateral haemophilic arthropathy of the elbows presented with an enlarging left elbow mass and worsening paresthesias in the ulnar distribution of the left hand. The mass, originally thought to be olecranon bursitis and treated as such, was found to be due to a haemophilic pseudotumour. The patient underwent successful excision of the haemophilic pseudotumour with concomitant ulnar nerve decompression and had sustained resolution of the pseudotumour and symptoms at 10 years of follow-up. This case demonstrates the need for consideration of

Intraneural microcystic lymphatic malformation of the ulnar nerve at the Guyon canal: Unusual cause of ulnar pain in a child.

We present a case of an unusual cause of ulnar pain on a 9-year-old patient. The patient had pain on the ulnar side of the wrist after a fall. MRI showed a poorly defined lesion on the ulnar nerve at Guyon's canal. The initial diagnosis was traumatic neuropathy. Following conservative treatment of symptoms for one year, the pain started again. Therefore, a new MRI was performed where progression of the lesion was observed. Excision of the lesion was performed and the specimen sent for pathologic analysis. The diagnosis was of microcystic lymphatic malformation. The patient had a satisfactory e

Dysplasia epiphysealis hemimelica of the elbow.

Dysplasia epiphysealis hemimelica (DEH) or Trevor's disease is a rare disorder affecting the epiphyses and short bones of the limbs, and characterized by a benign overgrowth of the medial-half of the epiphysis resembling osteochondroma. Upper limb involvement is very rare and most commonly affects the carpal bones. Only five established cases of DEH around the elbow were found in the orthopedic literature. We herein report an additional case of Trevor's disease of the elbow in a child with a symptomatic ulnar nerve compression and its management. To the best of our knowledge, this is the first

Ganglion cysts and other tumor related conditions of the hand and wrist.

Most regard ganglion, giant cell tumor of tendon sheath and epidermal inclusion cysts as tumor-like conditions as opposed to true neoplasms. Ganglion cysts are the most common lesion of the hand and wrist, accounting for 50% to 70% of all masses identified. The majority of ganglion cysts can be treated nonoperatively but when surgery is performed a low recurrence rate can be anticipated. Giant cell tumor of the tendon sheath hand epidermoid cysts are also common hand lesions that require surgical excision in most instances. Of the three, giant cell tumor of tendon sheath have the most notable

Quality of life and satisfaction in patients surgically treated for cubital tunnel syndrome.

OBJECTIVE: Cubital tunnel syndrome (CuTS) results from compression of the ulnar nerve at the elbow, resulting in pain, weakness, and numbness of the hand and forearm. This work describes the Quality of Life (QoL) and satisfaction after CuTS surgery and identifies the factors associated with those outcomes. METHODS: This cross-sectional study included patients surgically treated for CuTS from January 2011 to September 2021 at the Neurosurgery Clinic of the Clinic Center in Serbia. The questionnaires applied were the Short Form 36 (SF-36), EuroQol instrument (EQ-5D-5 L), Bishop's score and Pati

Indirect decompression of the carpal tunnel during basal joint arthroplasty of the thumb.

PURPOSE: In patients having basal joint arthroplasty, rates of concomitant carpal tunnel syndrome may exceed 43%. Excision of the trapezium during basal joint arthroplasty partially releases the transverse carpal ligament. The objective of this study was to determine whether indirect release of the remaining fibers of the ligament at its insertion on the scaphoid tubercle through the same incision would adequately decompress the carpal canal. This would avoid the morbidity of a second incision. METHODS: We retrospectively reviewed 10 consecutive patients treated over a 2-year period who had c

Short-term outcomes of percutaneous versus subcutaneous fascial flap fixation in cubital tunnel syndrome surgery: A retrospective cohort study.

OBJECTIVE: Percutaneous external suturing has been proposed as an alternative approach to fascial flap fixation during anterior subcutaneous transposition for cubital tunnel syndrome (CuTS), with the aim of minimizing deep tissue disruption and reducing postoperative complications. This study compared short-term outcomes between percutaneous external fixation and traditional subcutaneous fixation of fascial flaps in CuTS surgery. METHODS: In this retrospective cohort study, 114 patients who underwent anterior ulnar nerve transposition with fascial flap coverage were enrolled between January 2

Wheelchair ergonomic hand drive mechanism use improves wrist mechanics associated with carpal tunnel syndrome.

Among conventional manual wheelchair (CMW) users, 49% to 63% experience carpal tunnel syndrome (CTS) that is likely induced by large forces transmitted through the wrist and extreme wrist orientations. The ergonomic hand drive mechanism (EHDM) tested in this study has been shown to utilize a more neutral wrist orientation. This study evaluates the use of an EHDM in terms of wrist orientations that may predispose individuals to CTS. Eleven adult full-time CMW users with spinal cord injury participated. Motion data were captured as participants propelled across a flat surface, completing five tr

Surgical outcome for type I Guyon's canal syndrome with claw hand deformity: a review of 11 patients.

BACKGROUND: Guyon's canal syndrome (GCS) is an uncommon compressive ulnar neuropathy at the wrist, and claw hand deformity could be seen in severe Tpye I cases. Data on whether claw hand deformity could be reversed by surgical treatment is lacking, and we aimed to evaluate the prognosis of these patients. METHODS: 11 patients with claw hand deformity were enrolled in this retrospective study, and the minimum follow-up was set at 1 year. All patients underwent exploration of the ulnar nerve, decompression of Guyon's canal, and removal of potentially concomitant space-occupying lesions. Clinica

Age and the severity of carpal tunnel syndrome.

Several studies have shown an increased incidence of Carpal Tunnel Syndrome (CTS) with increasing age, as well as a longer Median sensory latency in older CTS patients. In this study, data was analyzed from 19 patients with bilaterally normal UE EMG/NCS and no symptoms of CTS (38 hands), as well as 18 patients with unilateral CTS and 21 with bilateral CTS (60 hands) to determine the effect of age on the severity of nerve conduction abnormalities associated with CTS. Most of these parameters showed increasing severity with age. Median motor latency rises sharply with age (r = .41, p = .001), an

Role of Ultrasound to Monitor Patients With Carpal Tunnel Syndrome Treated With a Local Corticosteroid.

OBJECTIVES: Carpal tunnel syndrome (CTS) is an entrapment neuropathy resulting from compression of the median nerve at the carpal tunnel (CT). We assessed the role of high-resolution ultrasound (US) in monitoring patients with CTS treated with corticosteroid injection. METHODS: We performed high-resolution US evaluations of the wrists of patients with CTS before and after treatment with local corticosteroid. Thirty-nine and 17 wrists were included for analysis at the end of 1 and 3 months, respectively. The cross-sectional area (CSA) and anteroposterior diameter (APD) of the median nerve at t
