PELVIC REHABILITATION FOR URINARY INCONTINENCE AFTER RADICAL PROSTATECTOMY.

Radical prostatectomy (RP) performed by open, laparoscopic, or robotic approach is considered the gold standard for localized prostate cancer (PCa). However, it carries the risk of postprostatectomy urinary incontinence (UI) and erectile dysfunction (ED) which significantly reduce patients' satisfaction with surgery and quality of life (QoL), therefore it is important to decrease the possibility or severity of these complications to a minimum. There are several preoperative prognostic factors such as urethral length and closing pressure obtained by magnetic resonance imaging and profilometry, …

Stress Urinary Incontinence Among Young Nulliparous Female Athletes.

Urinary incontinence (UI) is described as unintentional voiding of urine that is usually seen in post-partum and post-menopausal women due to the weakening of pelvic floor muscles (PFM). Recent studies have shown an increase in the prevalence of stress urinary incontinence (SUI) among young nulliparous female athletes. The association between UI and high-impact physical activity is due to increased intra-abdominal pressure during high-impact sports exceeding intra-urethral pressure. Usually, the levator ani muscle (LAM) helps in urethral closure. However, weakening or injury of LAM can reduce …

Urethral bulking agents for female stress urinary incontinence.

Urethral bulking agents (UBAs) to treat stress urinary incontinence was first described in the 1930s, when paraffin was used to increase urethral resistance. Since then, several agents have been introduced to the market, with varying degrees of safety, efficacy, and durability. The agents currently available include calcium hydroxyl apatite (Coaptite®), carbon coated zirconium (Durasphere®); polydimethylsiloxane elastomer (Macroplastique®); and polyacrylamide hydrogel (Bulkamid®). The latest product, PDMS-U (Urolastic ®) is a silicone gel that polymerizes when injected. The short term efficacy…

Current treatments for female primary stress urinary incontinence.

Stress urinary incontinence (SUI) affects millions of women worldwide. Pelvic floor muscle training is the first-line treatment for SUI, and if this fails, midurethral sling surgery has become the gold-standard treatment. More recently, complications from midurethral mesh slings, particularly chronic pain and dyspareunia, have become a major concern. Although traditional SUI treatments, such as colposuspension and fascia slings, are used, the future of SUI treatment likely will rely on less invasive alternatives. Modern bulking agents could have the potential to become a first-line treatment f…

[Urinary incontinence revealing vescicouterine fistula: about a case].

Vescicouterine fistula (VUF) is a rare lesion accounting for only 4% of all urogenital fistulas. It is most often secondary to cesarean section and forms a connection between the bladder and the uterus. We report the case of a young patient with VUF secondary to cesarean section. This study aims to investigate the clinical and therapeutic features of this disorder.

Markov chain decision model for urinary incontinence procedures.

Purpose Urinary incontinence (UI) is a common chronic health condition, a problem specifically among elderly women that impacts quality of life negatively. However, UI is usually viewed as likely result of old age, and as such is generally not evaluated or even managed appropriately. Many treatments are available to manage incontinence, such as bladder training and numerous surgical procedures such as Burch colposuspension and Sling for UI which have high success rates. The purpose of this paper is to analyze which of these popular surgical procedures for UI is effective. Design/methodology/ap…

Management of Urinary Incontinence Following Suburethral Sling Removal.

PURPOSE: We evaluated urinary incontinence outcomes following synthetic suburethral sling removal in women. MATERIALS AND METHODS: We reviewed a prospectively maintained database of 360 consecutive women who underwent transvaginal suburethral sling removal from 2005 to 2015. We excluded patients with neurogenic bladder, nonsynthetic or multiple slings, prior mesh for prolapse, concomitant surgery during sling excision, urethral erosion or fistula, postoperative retention or less than 6-month followup. Demographics, sling type, indications for removal, time to removal and patient reported outc…

Long-term outcomes after stress urinary incontinence surgery.

OBJECTIVES: To compare the long-term risk of repeat stress urinary incontinence (SUI) surgery after different types of initial SUI surgery and to identify predictors of time to repeat SUI surgery in a large, population-based cohort. METHODS: We used de-identified, adjudicated health care claims data from approximately 100 employer-based plans across the United States from 2000 to 2009. We identified the index SUI surgery in women aged 18 to 64 years. Kaplan-Meier survival curves were used to estimate cumulative incidence of repeat surgery through 9 years. Cox proportional hazards models were …

[Female stress urinary incontinence first line surgical treatment].

The sub uretral slings (SUS) have supplanted all other techniques in the treatment of stress urinary incontinence in first-line both at ease, results, consequences and cost. However the results are comparable with the abdominal retropubic colposuspension. The elective indication for BSU is stress incontinence. In the mixed incontinence, the results are satisfactory and depend on the degree of urge incontinence. The intrinsic sphincter deficiency with mobile uretra can be used in indications laying BSU. Only a BMI ≥ 35 is a pejorative factor on the outcome of the SUS. Currently, all routes retr…

[Stress urinary incontinence in women--current treatment alternatives].

BACKGROUND: Stress urinary incontinence in women occurs frequently. This paper gives a review of current treatment alternatives. MATERIAL AND METHODS: The paper is based on the report from the 3 rd International Consultation on Incontinence, the Cochrane database, a PubMed search, and my own clinical experience as a urogynaecologist. RESULTS: General practitioners may initiate conservative treatment without extensive evaluation. The first line treatment is pelvic floor muscle exercises (8-12 strong contractions X 3 every other day). Motivation and compliance are extremely important. Electros…

Urinary incontinence after pelvic trauma: a case report.

Stress and Urge urinary incontinence may develop after a pelvic trauma especially after pelvic bone fractures. Incontinence may persist even though any type of bladder neck suspension is performed if malunion occurs between fracture ends. In stress and urge urinary incontinence developed after pelvic trauma, patients should also be evaluated for malunion of fractures which may lead to bone spurs and during any type of bladder neck suspension these should also be removed.

Bedside urodynamic studies: simple testing for urinary incontinence.

Through proper evaluation and assessment, a large number of patients suffering from urinary incontinence may be effectively treated. Urodynamic studies, which are tests used to assess lower urinary tract function, may be performed using elaborate machinery or simple equipment found in most office settings. Bedside urodynamic studies may be performed without machinery. Patients with symptoms of stress incontinence or urge incontinence are candidates for bedside urodynamic techniques. Using the assessment tool results, pharmacologic therapy and behavioral treatment may be instituted. This articl…
