The management of a pregnancy complicated by uterus myomatosus remains being controversially discussed. Myomectomy early in pregnancy is opposed by exspectative management with myomectomy subsequent to the post partum period. Therapeutic abortion and exstirpation of the gravid uterus represent only hypothetical but not reasonable alternatives. Complications like ureteral obstruction, sepsis, and torsion of the uterus with subperimetrial bleeding as a cause of fever of unknown origin may occur. The case of an uterus--bladder fistula which developed after a Cesarean section and caused repeated spontaneous abortion is presented. With the already existing fistula dry labor was followed by the prolapse of the umbilical cord and finally by spontaneous abortion in the course of 2 successive pregnancies. As surgical solution hysterectomy and closing of the bladder fistula were carried out.

A case of bicornuate uterus associated with unilateral renal agenesis in a 45-year-old woman is reported. The embryologic problems of müllerian or wolffian ducts in this anomaly are discussed. By using radiological, nuclear or ultrasound methods unnecessary surgery could be evaded.

When double uterus is observed, absence of one kidney is common. These rare congenital anomalies are known in literature. In the years 1980-1984 we observed four cases. Symptoms, diagnosis and therapy are described. Preoperative diagnosis is now much better by ultrasonography. Nevertheless operative procedure has to be adapted to the real anatomic situation at first within the intervention. In one case a second operation was necessary because of a recurrence. This case is described in detail.

Two cases of uterus didelphys associated with unilateral imperforate vagina are reported with a review of the literature. In all cases investigated, ipsilateral renal agenesis was found on the affected side. A summary of the embryologic development of the female reproductive tract is included, as well as a possible explanation for this rare anomaly. The authors describe their proposed method of management to achieve freedom from symptoms and maintenance of a relatively intact reproductive tract.
