[Risk factors for in-hospital course and long-term outcome in myocardial infarction].

The aim of the present study is to separate the most important in-hospital and long term outcome risk factors in patients with myocardial infarction. We analysed 251 women and 630 men hospitalised for acute myocardial infarction between 1992-96. We compared history data, in-hospital course and long term observation within 2-6 years in a group of patients who died versus group of patients who survived. The most important risk factors of in-hospital death were: cardiogenic shock--with mortality rate--6.2, pulmonary oedema--2.8, ventricular fibrillation--2.7, third degree A-V block--2.5, supraven

Experience with flecainide for the treatment of cardiac arrhythmias in children.

We treated 22 children, aged 3 days to 16 years 6 months (median 11 years 1 month), with flecainide for a variety of arrhythmias where a Class I agent was indicated. In 16, conventional antiarrhythmic treatment had failed. Structural heart disease was present in nine. The arrhythmia was paroxysmal re-entry atrioventricular tachycardia in nine; paroxysmal atrial tachycardia, flutter or fibrillation in five; paroxysmal ventricular tachycardia in five and frequent ventricular extrasystoles (with couplets) in three. Sinus rhythm was achieved in all four children who received flecainide during tach

[Plasma level studies of quinidine retard, their significance for dosage and onset of effect].

A new sustained-action preparation of quinidine-bisulfate (BM-Chinidin Retard) was investigated in 21 patients. After a single oral dose of 1 g peak plasma levels are reached within 3-4 hours with an unsubstantial decrease during the following 4 hours. After 2 g in three divided doses within 12 hours maximal plasma levels are found after 14-16 hours, but an effective level is reached as early as 4 hours after the first dose. With 1 g Chinidin retard given in two doses within 24 hours, varying plasma concentrations are reached after 24 hours, reaching a maximum after 48 hours and decreasing to

Diagnosis and management of wide complex tachycardia in the emergency department.

INTRODUCTION: While wide complex tachycardia (WCT) is potentially lethal, little is known about its incidence in the ED or about expertise of ED physicians in diagnosing and treating it. We sought to compare WCT ED cases that were primary arrhythmias versus those with rapid heart rate secondary to medical issues, as well as the accuracy of ED diagnosis and appropriateness of treatment. METHODS: We conducted a health records review at a large academic hospital ED staffed by 95 physicians and included consecutive patients over 28 months (2018-2020) with WCT (heart rate ≥ 120 bpm and QRS ≥ 120 m

Catheter ablation of macro-reentrant atrial tachycardia in adults with Ebstein anomaly: A narrative review.

Improving survival rates of patients with Ebstein anomaly have increased the relevance of catheter ablation of macro-reentrant atrial tachycardia (MRAT). Our objectives were, first, to present a case report and, second, to provide a comprehensive review. A search of articles providing detailed information on adults with Ebstein anomaly who underwent catheter ablation of MRAT was performed. PubMed was queried using "Ebstein" and "ablation" as Medical Subject Headings terms, followed by a review of references and citations. Of the 56 patients included in the final review, which included the case

Atrial flutter with flecainide-induced 1:1 conduction at a rate <200 b.p.m. at rest: a case report.

BACKGROUND: Class IC antiarrhythmic drug flecainide is commonly used in the management of atrial arrhythmias and in particular atrial fibrillation (AF). Although previously reported as a potential complication, atrial flutter (AFL) with 1:1 atrioventricular (AV) conduction is rare, with only few cases reported so far, most of which related to physical activity. In all previous reported cases, 1:1 conduction resulted in ventricular rates of >200 b.p.m. CASE SUMMARY: We report the case of a 60-year-old woman, who presented to our local emergency department with palpitations related to acute ons

Antiarrhythmic effects of intravenous timolol in supraventricular arrhythmias.

The antiarrhythmic effect of timolol was investigated in 160 subjects with supraventricular arrhythmias. In our double-blind, randomized, parallel, multiclinic study, subjects received timolol, 1 mg iv, or matching placebo as a starting dose, followed by a second and third dose of 1 mg each (or matching placebo) at 20-min intervals if the arrhythmia did not convert to sinus rhythm. Subjects in whom the sinus rhythm returned or the ventricular rate decreased to less than 100 bpm were transferred to a dosing regimen of timolol in 10-mg tablets twice a day by mouth, 1 hr after the last intravenou

[Effects of verapamil on sinusal function and on atrioventricular conduction along normal and abnormal pathways, in patients with pre-existing troubles of excitation conduction (author's transl)].

Effects of verapamil (5 mg i.v.) on excitation conduction in 20 patients who already had a basic alteration (7 with ventricular pre-excitation syndrome) were studied by His electrogram, during spontaneous sinusal rhythm and atrial electrostimulation in increasing frequencies. In 4 cases, a test crossed with atropine was carried out. The effects of the drug on the effective refractory period of the A-V node and the anomalous tract in 3 patients with W.P.W. syndrome of Kent's bundle were studied. The effects were: no significant modifications in the sinusal frequency; increase in the corrected p

Left Ventricular Thrombus Despite Prescribed Direct Oral Anticoagulant Therapy in Chronic Heart Failure With Reduced Ejection Fraction: A Case Report and Review of Risk Stratification.

Left ventricular thrombus (LVT) remains a clinically important complication associated with increased risks of systemic thromboembolism, ischemic stroke, and adverse cardiovascular outcomes. The optimal duration of anticoagulation treatment remains difficult to establish, and it should be individualized depending on patient characteristics. We present the case of a 65-year-old man with a history of coronary artery disease (CAD) status post-myocardial infarction, who underwent coronary bypass graft surgery, ischemic cardiomyopathy, chronic heart failure with reduced ejection fraction (HFrEF) wi

Effect of SGLT-2 inhibitors on arrhythmia events: insight from an updated secondary analysis of > 80,000 patients (the SGLT2i-Arrhythmias and Sudden Cardiac Death).

OBJECTIVE: We aimed to assess the effect of SGLT2i on arrhythmias by conducting a meta-analysis using data from randomized controlled trials(RCTs). BACKGROUND: Sodium-glucose co-transporter 2 inhibitors (SGLT2i) have shown cardioprotective effects via multiple mechanisms that may also contribute to decrease arrhythmias risk. METHODS: We searched in databases (PubMed, Embase, Cochrane Library, and clinicaltrials.gov) up to April 2023. RCTs comparing SGLT2i with placebo were included. The effects of SGLT2i on atrial fibrillation(AF), atrial flutter(AFL), composite AF/AFL, ventricular fibrillat

Prognosis of paradoxical low-flow low-gradient aortic stenosis after transcatheter aortic valve replacement.

AIMS: In paradoxical low-flow low-gradient severe aortic stenosis (PLFLG AS) patients, stroke volume index (SVI) is reduced despite preserved left ventricular ejection fraction (LVEF). Although reduced SVI is already known as a poor prognostic predictor, the outcomes of PLFLG AS patients after transcatheter aortic valve replacement (TAVR) have not been clearly defined. We retrospectively investigated the post-TAVR outcomes of PLFLG AS patients in comparison with normal-flow high-gradient aortic stenosis (NFHG AS) patients. METHODS: The current observational study included 245 patients with NF

Feasibility of complex transfemoral electrophysiology procedures in patients with inferior vena cava filters.

BACKGROUND: The presence of inferior vena cava filters (IVCFs) has been considered a relative contraindication to electrophysiology (EP) procedures that require transfemoral venous placement of multiple catheters and/or long sheaths. There are inadequate data related to complex EP procedures in this population. OBJECTIVE: The purpose of this study was to describe the experience of a single high-volume center with respect to complex EP procedures in patients with IVCFs. METHODS: Patients with IVCFs undergoing complex EP procedures between 2004 and 2018 were identified. Clinical characteristic

Management and Outcome of Periprocedural Cardiac Perforation and Tamponade with Radiofrequency Catheter Ablation of Cardiac Arrhythmias: A Single Medium-Volume Center Experience.

INTRODUCTION: Cardiac tamponade (CT) is a life-threatening complication of radiofrequency ablation (RFA). The course and outcome of CT in low-to-medium volume electrophysiology centers are underreported. METHODS: We analyzed the incidence, management and outcomes of CT in 1500 consecutive RFAs performed in our center during 2011-2016. RESULTS: Of 1500 RFAs performed in 1352 patients (age 55 years, interquartile range: 41-63), 569 were left-sided procedures (n = 406 with transseptal access). Conventional RFA or irrigated RFA was performed in 40.9% and 59.1% of procedures, respectively. Ablati

Atrial tachyarrhythmias and the Cox-maze procedure in congenital heart disease.

INTRODUCTION: Atrial tachyarrhythmias, particularly atrial flutter and fibrillation, are commonly associated with congenital heart disease and are a major cause of morbidity and mortality. The Cox-maze procedure, introduced by Dr. James Cox in 1987, is effective at controlling atrial fibrillation in structurally normal hearts. Though the Cox-maze procedure has been used for atrial tachyarrhythmias in patients with congenital heart disease, few studies have looked at its effectiveness. METHODS: A retrospective chart review was performed on 24 patients with congenital heart disease who underwen

Novel pore mutation in SCN5A manifests as a spectrum of phenotypes ranging from atrial flutter, conduction disease, and Brugada syndrome to sudden cardiac death.

OBJECTIVES: The purpose of this study was to determine the clinical and biophysical characteristics of a novel SCN5A mutation. BACKGROUND: Brugada syndrome and isolated cardiac conduction defect have been linked to SCN5A mutations. METHODS: Eleven members of a western European family underwent electrophysiologic investigations and mutation analysis of the SCN5A gene. Wild-type and mutant SCN5A channels were expressed in HEK293 cells, and whole cell currents were studied using patch clamp procedures. RESULTS: A novel mutation, R376H, in the first pore segment of SCN5A variably causes Brugada

Atrial flutter in normal heart could be first manifestation of Brugada syndrome.

Brugada syndrome is one of the important causes of sudden cardiac death in young adults. The condition is associated with typical ECG changes in anteroseptal leads V1 and V2 that can be unmasked by various medications, electrolyte disturbances, and even by fever in susceptible individuals. We here report the case of a 22-year-old female admitted to the emergency room with a typical atrial flutter who developed Brugada-like ECG changes after conversion to sinus rhythm following flecainide infusion with subsequent degeneration in ventricular fibrillation. The patient converted to sinus rhythm af
