Transapical valve-in-ring mitral valve implantation through the anterior mitral valve leaflet.

Transcatheter mitral valve implantation is a relatively novel intervention that replaces the mitral valve of individuals deemed too high-risk or unsuitable for surgery. It is associated with a number of specific risks, including left ventricular outflow tract obstruction. In this report, we present the case of a 75-year-old man who was unable to undergo redo surgical repair and had a number of risk factors for left ventricular outflow tract obstruction. To minimize this risk, we deployed transcatheter mitral valve implantation within the anterior mitral valve leaflet resulting in mild mitral v

Surgical Considerations in Shone Complex.

Shone complex is defined by 4 anomalies: parachute mitral valve, supravalvar mitral ring, subaortic stenosis, and coarctation of the aorta. Establishing a clear definition is one of the principal challenges in the study of Shone complex as not all patients have all lesions. The essential feature of Shone complex is multilevel left-sided obstruction involving both the left ventricular inflow and outflow. This anatomic variability is reflected in the clinical presentation as signs of left ventricular inflow obstruction are often masked by outflow obstruction and the multilevel nature of the cond

Haemodynamic and echocardiographic findings after aortoventriculoplasty.

Aortoventriculoplasty (AVP) is an established operative procedure for the enlargement of different types of severe left ventricular outflow tract obstruction. Between 1974 and 1985 75 aortoventriculoplasties were carried out in 72 patients ranging from 5 to 34 years of age. Three patients had to be reoperated upon due to significant right ventricular outflow tract obstruction, outgrown prosthesis, and dissecting aortic aneurysm. There were 7 early deaths (mortality rate 9.3%) and one late death (1.3%) following AVP. Out of the last 55 patients only 2 died (3.6%). In contrast to the unsatisfact

Mavacamten in Symptomatic Patients Resistant to Previous Advanced Therapy for Obstructive Hypertrophic Cardiomyopathy.

BACKGROUND: The clinical benefits of mavacamten in patients with obstructive hypertrophic cardiomyopathy previously treated with advanced therapies are not established. METHODS: Clinical and echocardiographic outcomes of patients treated with mavacamten for left ventricular outflow obstruction for at least 8 weeks were assessed based on prior treatment with one or more advanced therapies: disopyramide, septal myectomy, alcohol septal ablation, dual-chamber ventricular pacing with short atrioventricular delay; we also evaluated patients with left ventricular outflow obstruction that emerged as…

Left ventricular mid-cavity obstruction: A novel cause of duct-dependent systemic circulation in a newborn.

Accessory mitral valve tissue is a rare congenital anomaly that may cause left ventricular outflow tract obstruction. This may be the first case report in a neonate, describing accessory mitral valve tissue producing a significant left ventricular outflow tract obstruction, resulting in a duct-dependent systemic circulation perfusing the neck and head vessels. This report also discusses why a hybrid palliative strategy was adopted.

Anomalous left coronary artery in hypertrophic cardiomyopathy.

Hypertrophic cardiomyopathy and anomalous origin of the coronary artery from the opposite sinus are common causes of sudden cardiac death. These entities have rarely been reported together. Here we present the case of a 48-year-old woman with hypertrophic cardiomyopathy and significant left ventricular outflow tract obstruction. She was referred for septal reduction therapy for symptomatic left ventricular outflow tract obstruction refractory to medical therapy. Cardiac catheterization and coronary artery computed tomography angiogram revealed a single coronary artery arising from the right si

Cross-sectional echocardiographic assessment of aortic obstruction.

Cross-sectional echocardiographic features of aortic obstruction occurring at multiple levels of the left ventricular outflow tract are discribed. Specific pathologic entities considered include coarctation of the aorta, supravalvular, valvular, and discrete subvalvular aortic stenosis, as well as functional or idiopathic hypertrophic subaortic stenosis. At each of these levels the cross-sectional method permits direct visualization of the obstructing lesion, it's morphologic characteristics, and extent. In addition the relationship of the area of obstruction to more normal surrounding areas o

Mid-systolic drop in left ventricular ejection velocity in obstructive hypertrophic cardiomyopathy--the lobster claw abnormality.

UNLABELLED: In many patients with obstructive hypertrophic cardiomyopathy, an abrupt mid-systolic drop in left ventricular ejection velocity can be detected. We analyzed 27 patients with obstructive hypertrophic cardiomyopathy who had 43 echocardiographic examinations (mean gradient 53 +/- 6 mm Hg). Exams showing a mid-systolic drop had higher mean outflow tract pressure gradients (90 +/- 6 compared with 29 +/- 4 mm Hg, p < 0.001). After medical elimination of obstruction, the mid-systolic drop was no longer seen. We measured 105 pulsed-wave Doppler tracings in the left ventricular cavity and

Clinical and stent-related outcomes after transcatheter or operative placement of bare-metal stents in the ventricular septum or subvalvar systemic outflow tract.

BACKGROUND: Stenting of the systemic ventricular outflow tract and creation or enlargement of a ventricular septal defect using stents are potential therapeutic options in rare patients with congenital heart disease who develop significant ventricular hypertension due to outflow obstruction. These stents may be at increased risk of fracture because of exposure to cyclic compressive stresses. The objective of this study was to evaluate the safety and efficacy of this intervention and to determine the incidence of stent fracture and other adverse outcomes after placement of intraventricular or i

Midterm results of anatomic repair in a subgroup of corrected transposition.

BACKGROUND: Anatomic repair has become the preferred option in the subgroup of patients with congenitally corrected transposition of the great arteries with ventricular septal defect and pulmonary obstruction. We report our 14-year experience with this approach. METHODS: From April 2001 to February 2014, 22 patients with congenitally corrected transposition with ventricular septal defect and pulmonary obstruction underwent anatomic repair. Nineteen patients had a modified Senning-Rastelli procedure, 2 had a Mustard-Rastelli procedure, and one had a hemi-Mustard-Glenn-Rastelli procedure. The m

Management of right ventricular outflow tract reactivity following pulmonary valve dilatation under general anesthesia: experience of a medical mission.

During a recent volunteer medical mission to the Dominican Republic, we administered anesthesia to a child with severe pulmonary valvular stenosis and suprasystemic right ventricular pressure. This patient underwent a balloon pulmonary valvuloplasty but then developed worsening right ventricular outflow tract obstruction that required immediate therapy to prevent a low cardiac output state. As the development of hyperdynamic right ventricular outflow tract obstruction cannot be predicted, we emphasize the need for preemptive hydration and beta-blockade therapy prior to balloon dilatation.

Hypertrophic Cardiomyopathy.

Hypertrophic cardiomyopathy is a myocardial disease characterized by myocardial hypertrophy, disorganization of cardiac myocytes, and fibrosis. Twenty-five percent of patients have a dynamic left ventricular outflow tract gradient caused by the combined effects of rapid ventricular ejection, a narrowed outflow tract, and systolic anterior motion of the mitral valve. Most cases are caused by mutations in genes that encode cardiac sarcomeric proteins. Patients present at all ages with chest pain, dyspnea, palpitations, and syncope. The most important complications of the disease are sudden cardi

Mechanism of left ventricular outlfow obstruction in patients with obstructive asymmetric septal hypertrophy (idiopathic hypertrophic subaortic stenosis).

Left ventricular outflow obstruction in patients with idiopathic hypertrophic subaortic stenosis or obstructive asymmetric septal hypertrophy is due to abnormal forward motion during systole of the anterior mitral leaflet. To determine why some patients with this disease hav left ventricular outflow obstruction whereas others do not, we studied a large number of patiens with assymetric septal hypertrophy using both one- and two-dimensional echocardiography. In 100 patients with asymmetric septal hypertrophy and 22 normal subjects, mitral valve position at the onset of systole was quantitated b…

Totally endoscopic transaortic septal myectomy for hypertrophic obstructive cardiomyopathy.

Hypertrophic obstructive cardiomyopathy is characterized by asymmetric septal hypertrophy and dynamic left ventricular outflow tract obstruction, often associated with systolic anterior motion of the mitral valve. In symptomatic patients who are not responders to medical therapy, surgical septal myectomy is considered the gold standard. We present a case of totally endoscopic transaortic septal myectomy performed in a symptomatic patient with severe left ventricular outflow tract obstruction and systolic anterior motion of the mitral valve. Preoperative imaging demonstrated marked asymmetric s

Technical Features of Biventricular Repair for Unbalanced Transitional AVSD With LVOTO.

Surgical management of left ventricle outflow tract obstruction in infants with right-dominant unbalanced transitional atrioventricular septal defect poses difficulties. A two-month-old infant with transitional atrioventricular septal defect and complex left ventricle outflow tract obstruction presented in cardiogenic shock. The patient underwent successful biventricular repair. The operative procedure included detachment of the anterior bridging leaflet and resection of its chordal attachments. Septal myectomy was performed with creation of an interventricular communication, followed by patch

Unveiling the complexity of nonobstructive hypertrophic cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) is a genetic heart disorder defined by the presence of a maximal wall thickness of at least 15 mm with two main forms: obstructive (oHCM) and nonobstructive (nHCM). While oHCM is characterized by left ventricular (LV) outflow tract obstruction (LVOTO), nHCM lacks this feature and shows significant variability in its hemodynamic and anatomical traits. In nHCM, LV hypertrophy (LVH) presents diverse morphologies, including apical hypertrophy and reverse septal curvature, the latter potentially causing mid-ventricular obstruction and near-complete LV emptying. Hypertrophic cardiomyopathy: part 1 - introduction, pathology and pathophysiology.

Hypertrophic cardiomyopathy (HCM) is the most common genetic cardiovascular disease with many genotype and phenotype variations. Earlier terminologies, hypertrophic obstructive cardiomyopathy and idiopathic hypertrophic sub-aortic stenosis are no longer used to describe this entity. Patients present with or without left ventricular outflow tract (LVOT) obstruction. Resting or provocative LVOT obstruction occurs in 70% of patients and is the most common cause of heart failure. The pathology and pathophysiology of HCM includes hypertrophy of the left ventricle with or without right ventricular h

Pitfalls in clinical recognition and a novel operative approach for hypertrophic cardiomyopathy with severe outflow obstruction due to anomalous papillary muscle.

BACKGROUND: Ventricular septal myotomy/myectomy (Morrow procedure) is the standard surgical option for severely symptomatic patients with hypertrophic cardiomyopathy (HCM) and marked basal obstruction to left ventricular outflow due to mitral valve systolic anterior motion. In some patients, however, congenital malformations of the mitral apparatus may be responsible for outflow obstruction; the failure to recognize this morphology before operation could have adverse consequences. METHODS AND RESULTS: We recently evaluated 2 patients with obstructive HCM operated on at Mayo Medical Center in
