Risk factors for posttraumatic stress disorder in Australian Vietnam veterans.

OBJECTIVE: The aims of this paper are to determine the risk factors for combat-related posttraumatic stress disorder (PTSD) and to examine the relative contribution of pre-military factors, pre-trauma psychiatric diagnoses, military factors such as combat posting, and combat and casualty stress exposure. METHOD: An epidemiological cohort study using standardised psychiatric, social and health interviews was undertaken with a national random sample of male Australian Army Vietnam veterans. Multivariate logistic regression was used to examine the relative contribution of factors derived from in…

Predicting outcomes in HIV-infected veterans: I. Progression to AIDS.

This article and the following article (Parts I and II) report the development of two clinical staging systems for HIV-infected individuals. The objective of the research reported here (Part I) was to construct a clinical staging system to predict progression to AIDS. We analyzed data from VA Cooperative Study Number 298, a multicenter, double-blind, randomized trial that compared immediate versus deferred zidovudine therapy in 338 HIV-infected individuals who did not have AIDS at enrollment. Baseline variables were tested in univariate Cox regression for their relationship to progression to A

Prior head injury in male veterans with borderline personality disorder.

This study evaluated the relationship between traumatic brain injury (TBI) and borderline personality disorder (BPD). Forty-three males with BPD were compared with 49 age- and sex-matched controls with other psychiatric diagnoses. The occurrence of TBI was identified by chart review. Head injury was identified in 42% of patients meeting criteria for BPD but in only 4% of controls (p < .001). All patients but one had sustained a TBI prior to the diagnosis of BPD. The prevalence of TBI is higher in subjects with BPD than in controls. Since TBI occurred prior to the full expression of the BPD cli

Diseases preceding colon cancer. A case-control study among veterans.

Patients with regular use of nonsteroidal antiinflammatory drugs (NSAIDs) appear to have a reduced mortality from colon cancer. As NSAID use is associated with gastrointestinal bleeding, endoscopic exploration of patients on NSAID may lead to more efficient screening and frequent detection of colon cancer. A case-control study was conducted among 12,304 veterans with a colon cancer diagnosed between 1988 and 1992. Four controls were matched by age, sex, and race to each case. The frequency distributions of previous discharge diagnoses in cases and controls were compared. Arterial embolism and …

Crescendo transient ischemic attacks: a surgical imperative. Veterans Affairs trialists.

PURPOSE: In a randomized, prospective, multicenter trial at 16 medical centers, 189 of 5000 patients screened with cerebrovascular disease were identified as having angiographic internal carotid artery stenosis (> 50%) corresponding to presenting symptoms of transient ischemic attacks (TIAs), transient monocular blindness, or recent, minor completed stroke. METHODS: Patients were randomly assigned to carotid endarterectomy plus the best medical care (n = 91) versus the best medical care alone (n = 98). RESULTS: After 1 year there was a significant reduction in stroke or crescendo attacks in

The Impact of Clinically Significant Pleural Effusion on Survival of US Veterans With Cancer, Congestive Heart Failure, and Pneumonia: The Veterans Administration Lung Effusion Study.

BACKGROUND: Previous clinical data suggest that the presence of a pleural effusion is associated with poor survival. However, these studies were limited by either a small sample size or lack of an adequate control group. RESEARCH QUESTION: What is the impact of pleural effusion on survival in patients hospitalized with an admitting diagnosis of the 3 most common causes of pleural effusion: cancer, congestive heart failure, or pneumonia? STUDY DESIGN AND METHODS: This is a retrospective analysis of US veterans hospitalized between January 1, 2000 and December 31, 2020. International Classific…

Scoping review of military veterans involved in the criminal legal system and their health and healthcare: 5-year update and map to the Veterans-Sequential Intercept Model.

BACKGROUND: A previous scoping review of legal-involved veterans' health and healthcare (1947-2017) identified studies and their limitations. Given the influx of literature published recently, this study aimed to update the previous review and map articles to the Veterans-Sequential Intercept Model (V-SIM) - a conceptual model used by key partners, including Veterans Health Administration, veteran advocates, criminal justice practitioners, and local governments to identify intercept points in the criminal legal system where resources and programming can be provided. Developing an updated resou…

Mental health therapy for veterans with PTSD as a family affair: A qualitative inquiry into how family support and social norms influence veteran engagement in care.

Social support is important for posttraumatic stress disorder (PTSD) recovery and emerging literature indicate that social support could increase engagement in PTSD therapy. However, there is a need to understand how and why family involvement can increase treatment engagement to inform strategies used in clinical practice. This study explores how individuals with PTSD and family members of individuals with PTSD experience therapy and how social interactions help or hinder therapy engagement. We interviewed 18 U.S. military veterans who had been referred for psychotherapy for PTSD in the Veter…

Exploring Interactions Between Traumatic Brain Injury History and Gender on Medical Comorbidities in Military Veterans: An Epidemiological Analysis in the VA Million Veteran Program.

Epidemiological studies of medical comorbidities and possible gender differences associated with traumatic brain injury (TBI) are limited, especially among military veterans. The purpose of this study was to examine relationships between TBI history and a wide range of medical conditions in a large, national sample of veterans, and to explore interactions with gender. Participants of this cross-sectional epidemiological study included 491,604 veterans (9.9% TBI cases; 8.3% women) who enrolled in the VA Million Veteran Program (MVP). Outcomes of interest were medical comorbidities (i.e., neurol…

Service use and barriers to care among homeless veterans: Results from the National Veteran Homeless and Other Poverty Experiences (NV-HOPE) study.

The objective of this study was to examine type of services U.S. veterans seek when they experience homelessness, characteristics associated with service use, and reasons for not using services. Data from a 2021 nationally representative survey of 1004 low-income U.S. veterans were analyzed with descriptive and multivariable analyses. One-quarter of low-income U.S. veterans with experiences of homelessness reported using homeless services while they were homeless and about 27% reported using U.S. Department of Veterans Affairs (VA) healthcare or benefits while homeless. Virtual Diabetes Optimization Program (VDOP) for Veterans With Uncontrolled Diabetes.

OBJECTIVE: More than 25% of U.S. veterans have diabetes, and 78% have a BMI ≥25 kg/m2. Despite access to lifestyle interventions and specialty endocrine care through the Department of Veterans Affairs (VA), many veterans with uncontrolled diabetes and obesity do not achieve sustained glycemic control or weight loss. We developed the Virtual Diabetes Optimization Program (VDOP), a quality improvement initiative targeting veterans with uncontrolled diabetes (glycated hemoglobin A1c [HbA1c] ≥8% [≥64 mmol/mol]) with or without obesity. RESEARCH DESIGN AND METHODS: VDOP used a multidisciplinary te…

Virtual Reality to Reduce Anxiety in Veterans Undergoing Chemotherapy Infusions.

BACKGROUND: Veterans are predisposed to generalized anxiety, and a cancer diagnosis can intensify psychological distress and overall unpleasant experiences during treatment. Infusion clinic staff at a veterans hospital tend to focus care primarily on the medical needs of patients, but evidence suggests health outcomes improve when both the mental health and physical needs of patients with cancer are addressed. OBJECTIVES: This evidence-based practice project sought to reduce anxiety in veterans undergoing chemotherapy infusions through virtual reality (VR) meditation. METHODS: The Iowa Model…

Prevalence of Vision-Threatening Ocular Disease Among North Carolina Veterans.

INTRODUCTION: Prevalence studies on ocular diseases among veterans are limited and underreported. In this retrospective study, we examined the prevalence of vision-threatening ocular diseases among North Carolina veterans. We assessed the prevalence of 6 serious ocular conditions among veterans receiving primary care at the Salisbury Veterans Affairs Health Care System (SVAHCS) in North Carolina to better understand ocular disease prevalence among veterans. Glaucoma, the most common ocular disease in this population, underscores the need for targeted interventions to mitigate vision loss. MAT…

Evolving Inguinal Hernia Repair Practice at the Veterans Health Administration.

IMPORTANCE: Inguinal hernia repair (IHR) is one of the most commonly performed operations in the United States, with approximately 800 000 performed each year. US veterans are a unique group that is predominantly male and tends to have higher rates of medical comorbidities and tobacco use, common risk factors for hernia development. With significant advancements in IHR technique in recent years, the majority of national database studies have focused on civilian populations. OBJECTIVE: To examine trends and the evolution of IHR within the Veterans Health Administration (VHA) over the past 20 y…

Frailty Predicts Incident Osteoporotic Fractures in Veterans with Rheumatoid Arthritis.

OBJECTIVE: Rheumatoid arthritis (RA) is associated with an increased risk of frailty and osteoporosis, but the relationship between frailty and incident osteoporotic fractures in RA is underexplored. METHODS: Data were from the Veterans Affairs (VA) Rheumatoid Arthritis Registry. Frailty was measured using the VA Frailty Index (VAFI). Incident osteoporotic fractures were identified by administrative codes and validated by physician chart review. Multivariable Cox proportional hazard models quantified the relationship between frailty category and incident osteoporotic fracture. Models adjusted…
