Dermatofibrosarcoma protuberans (DFSP) is a low-to-intermediate grade sarcoma of dermal origin that rarely presents in the vulva, typically occurring on the trunk of young to middle-aged adults. Even though it has a low potential for distant metastases, it often recurs locally. Therefore, surgical excision is the treatment of choice. Vulvar and vaginal cancer are uncommon gynecologic malignancies, most frequently diagnosed clinically. MR imaging is a powerful tool for local staging of these tumors and to detect posttreatment complications and recurrent disease. This review presents anatomic delineation of the female pelvis, pathology and staging of vulvar and vaginal cancer, MR imaging techniques of the pelvis, MR features of vulvar and vaginal cancer, and the differential diagnosis of potential mimickers.

BACKGROUND: Little information is available regarding the association between vulvar intraepithelial neoplasia grade 3 (VIN3) and Behçet's disease (BD). We report here concomitant VIN3 and genital ulcers in a patient with BD.
CASE: A 44-year-old Caucasian woman with a history of BD, which had been evolving for 6 years, presented with ulcerated and papillomatous lesions on the vulva. Biopsies revealed a multifocal VIN3 positive for high-risk human papillomavirus (HPV) 33. Multiple biopsies were performed to exclude invasive cancer and VIN3 was treated with laser vaporization.

Vulvar cancer develops onto vulvar dystrophies. Its development is linked with HPV infection in half of the cases. It can appear as a carcinoma in situ, a microinvasive carcinoma or a true invasive carcinoma. Prurit is the most common symptom. In situ carcinomas have to be treated by skinning vulvectomies. Radical vulvectomy was considered as mandatory for truly infiltrative cancers. One prefers today use the "wider local excision". However lymphadenectomy is still mandatory (less than 1 mm) in case of very limited dermal infiltration.

Methodological approaches have been developed toward raising the informative value of cytological diagnosis. There have been identified pathogenetic variants of background and precancerous affections of the vulva, their leading factors being viral infection, function disorders of the hypothalamic centres, hypoestrogenemia, hepatic and thyroidal abnormalities. The effectiveness of the treatment scheme developed with taking account of the above-stated items is 97.3%.
