The role of high flow oxygen therapy in acute respiratory failure.

Acute respiratory failure represents one of the most common causes of intensive care unit admission and oxygen therapy remains the first-line therapy in the management of these patients. In recent years, high-flow oxygen via nasal cannula has been described as a useful alternative to conventional oxygen therapy in patients with acute respiratory failure. High-flow oxygen via nasal cannula rapidly alleviates symptoms of acute respiratory failure and improves oxygenation by several mechanisms, including dead space washout, reduction in oxygen dilution and inspiratory nasopharyngeal resistance, a

Weaning newborns from mechanical ventilation.

While there is a relative consensus as to whether mechanical ventilation should be initiated, the management of babies during recovery from respiratory failure remains largely subjective and is predominantly determined by institutional or individual practices or preferences. This can lead to babies either being left on the ventilator too long, or extubated too hastily, thus requiring repeated re-intubation. The current scientific literature fails to provide a uniform view of the most appropriate way to wean babies from mechanical ventilation. This might stem from a lack of understanding of the

Respiratory effects of tracheal gas insufflation in spontaneously breathing COPD patients.

OBJECTIVE: To evaluate the effect of tracheal gas insufflation (TGI) in spontaneously breathing, intubated patients with chronic obstructive … tracheal gas insufflation decreased dead space, increased alveolar ventilation and possibly reduced work of breathing. From the preliminary data reported here, we believe that TGI may help patients experiencing difficulty during weaning.

A comparative evaluation of pressure-triggering and flow-triggering in pressure support ventilation (PSV) for neonates using an animal model.

The triggering system in pressure support ventilation needs to respond rapidly, especially in neonates. The aim of this study was to compare the effects of flow-triggered and pressure-triggered pressure support ventilation on neonatal mechanical ventilation using an animal model. Respiratory flow, airway pressure, oesophageal pressure, and diaphragmatic electromyogram were measured during pressure support ventilation in five anaesthetized rabbits. The animals were connected to a VIPBIRD (Bird, U.S.A.) (CPAP mode, pressure support ventilation, 5 cm H2O and PEEP 0 cm H2O). Flow-triggering sensit

[Evaluation of lung surfactant function in terms of lipid monolayer micromechanics].

The analysis of compression isotherms of lipid monolayers and thermodynamical estimation of the conditions of the formation of layered systems underlies the explanation of some details of lung surfactant work. When the chemical potential of the monolayer during compression reaches the value of the chemical potential of the liquid bilayer membrane free of the mechanical stress, the conversion of monolayer into bilayer phase commences by means of squeezing out of monolayer in subphase creating bilayer ripples. During inspiration when alveolar surface expands, ripples turn back. Hence, in the cou

Patient-Specific Finite Element Analysis of Ventral Incisional Hernia Repair: Biomechanical Investigation of the Cause of Recurrence.

Ventral incisional hernia is a frequent postoperative complication following abdominal surgery, with recurrence largely driven by biomechanical factors such as elevated intra-abdominal pressure and stress concentration at the suture-mesh-tissue interface. This study presents a patient-specific finite element analysis (FEA) framework to investigate stress transfer mechanisms within ventral incisional hernia repair systems under physiologically relevant loading conditions. A patient-specific abdominal wall geometry was reconstructed from computed tomography images and modeled as a nonlinear hype

Hyperinflation and intrinsic positive end-expiratory pressure: less room to breathe.

Clinically, the symptoms and limited exercise capabilities of patients with chronic obstructive pulmonary disease (COPD) correlate better wi … xpiratory pressure (PEEPi) that places the diaphragm at a mechanical disadvantage and increases the work of breathing. Severe hyperinflation is the major physiologic cause of the resulting hypercarbic respiratory failure and patients' inability to transition (i.e. wean) from mechanical ventilatory support to spontaneous breathing. This paper reviews the basic physi …

Detection of Parkinson's disease using nocturnal breathing signals based on multifractal detrended fluctuation analysis.

Parkinson's disease (PD) is a highly prevalent neurodegenerative disorder that poses a significant challenge in terms of accurate and cost-effective diagnosis. This study focuses on the use of fractal features derived from nocturnal breathing signals to diagnose PD. Our study includes 49 individuals with Parkinson's disease (PD group), 49 relatively healthy individuals without PD (HC group), 49 individuals without PD but with other diseases (NoPD group), as well as 12 additional PD patients and 200 healthy individuals for testing. Using multifractal detrended fluctuation analysis, we extracted

When CO2 sensing fails: Protective effects of sex hormones and potential mechanisms.

Breathing movements driven by the brainstem rhythmogenic neural networks rely on chemosensory inputs to produce a motor output that is homeostatically adjusted to the prevailing metabolic demand. The central CO2 chemoreflex is a critical component of respiratory neural circuitry, as defects in these sensors cause hypoventilation syndromes, which are typically difficult to manage pharmacologically. Progesterone has long been known to stimulate breathing in both sexes, and remarkably, a potent progestin drug, etonogestrel (ETO), has been shown to enhance CO2-chemosensitivity in animal models and

Preclinical MRI Using Hyperpolarized 129Xe.

Although critical for development of novel therapies, understanding altered lung function in disease models is challenging because the transport and diffusion of gases over short distances, on which proper function relies, is not readily visualized. In this review we summarize progress introducing hyperpolarized 129Xe imaging as a method to follow these processes in vivo. The work is organized in sections highlighting methods to observe the gas replacement effects of breathing (Gas Dynamics during the Breathing Cycle) and gas diffusion throughout the parenchymal airspaces (3). We then describe

End-tidal CO2 and ventilation: Novel markers for assessing performance levels in elite long-distance runners.

Well-trained individuals, compared to less well-trained individuals, exhibit a lower minute ventilation (V̇E) and higher end-tidal partial pressure of CO2 (PETCO2) at a given work rate. This study investigated whether such breathing adaptations seen in well-trained individuals also applied to elite long-distance runners. Forty-one long-distance runners were categorized into high (Long-High, consisting of Tokyo-Hakone College Ekiden [relay marathon] runners and Olympic athletes, n = 23), or low performance-level group (Long-Low, n = 18) according to their race times. Ten middle-distance runners

Edmund Goodwyn and the first description of diving bradycardia.

Diving bradycardia is a primordial oxygen-conserving reflex by which the heart rate of air-breathing vertebrates, including humans, slows down in response to water immersion. Its discovery is attributed to Paul Bert, whose seminal observation was published in 1870 as part of a series of experiments that examined physiological adaptations to asphyxia in ducks and other animals. However, Edmund Goodwyn, a British physician who studied medicine at the University of Edinburgh, had already described this reflex in his doctoral thesis, which was originally published in Latin in 1786 and again in Eng

Pulmonary Rehabilitation in the Management of Chronic Lung Disease.

Pulmonary rehabilitation is a core component of management of patients with chronic lung disease that have exercise or functional limitations. Causes of these limitations are manifold but include loss of skeletal muscle mass, power and endurance, diminished respiratory capacity owing to respiratory muscle weakness, inefficient gas exchange, and increased work of breathing, and impaired cardiovascular functioning. Besides physical limitations, patients with chronic lung disease have high rates of depression and anxiety leading to social isolation and increased health care use. Pulmonary rehabil…

Use of point-of-care ultrasound in the diagnosis and treatment of pulmonary oedema in an infant.

This case demonstrates the value of perioperative point-of-care ultrasound for rapid bedside evaluation and treatment of pulmonary oedema in an infant. A nine-week-old male infant undergoing cleft lip repair received significant intravenous fluid resuscitation for intraoperative hypotension. After uneventful extubation, he developed increased work of breathing and a gradual decline in oxygen saturation despite supplemental oxygen by way of a facemask. Lung point-of-care ultrasound revealed confluent B-lines in multiple lung fields, consistent with pulmonary oedema, likely from fluid overload. …

Energy relaxation in CdSe nanocrystals: the effects of morphology and film preparation.

Ultrafast time-resolved absorption spectroscopy is used to investigate exciton dynamics in CdSe nanocrystal films. The effects of morphology, quantum-dot versus quantum-rod, and preparation of nanocrystals in a thin film form are investigated. The measurements revealed longer intraband exciton relaxation in quantum-rods than in quantum-dots. The slowed relaxation in quantum-rods is due to mitigation of the Auger-relaxation mechanism from elongating the nanocrystal. In addition, the nanocrystal thin film showed long-lived confined acoustic phonons corresponding to the ellipsoidal breathing mode

Obstructive sleep apnea: one more target in cardiac rehabilitation.

Obstructive sleep apnea: one more target in Obstructive sleep apnea (OSA) is a highly prevalent sleep-related breathing disorder, often unrecognized and undiagnosed, in patients with established cardiovascular diseases. Considerable evidence is now available in support of a significant association between OSA and increased risk for cardiovascular disease morbidity. OSA has also been recognised as a potential public health issue associated with societal consequences including accidents and work economics. Treatment of OSA would reduce cardiovascular risk and the overall usage of health care res
