The LYMFASIM simulation program for modeling lymphatic filariasis and its control.

The LYMFASIM modeling framework for the transmission and control of the tropical parasitic disease lymphatic filariasis is described and its use in the context of an endemic community in north-eastern Brazil is illustrated. Lymphatic filariasis is a disease with a complex natural history with many unknowns. This complicates decision making with respect to control strategies. With LYMFASIM, a variety of hypotheses can be tested about the life history of the parasite Wuchereria bancrofti, its transmission from man to man through mosquitoes, the role of the immune system in regulating parasite nu…

The ICT Filariasis Test: A rapid-format antigen test for diagnosis of bancroftian filariasis.

Antigen testing is now recognized as the method of choice for detection of Wuchereria bancrofti infections. Unlike tests that detect microfilariae, antigen tests can be performed with blood collected during the day or night. However, existing enzyme-linked immunosorbent assay (ELISA) tests for filarial antigenemia are difficult to perform in the field, and this has limited their use in endemic countries. In this article, Gary Weil, Patrick Lammie and Niggi Weiss review their experience with a new rapid-format filarial antigen test. They found that the ICT card test was very easy to perform and…

Bancroftian lymphadenopathy: absence of eosinophils in tissues despite peripheral blood hypereosinophilia.

We studied the association of eosinophils with Wuchereria bancrofti parasites in two children with biopsy-proven filarial lymphadenopathy and peripheral blood hypereosinophilia. In both cases, intact adult worms were present in lymph nodes but no inflammatory reaction was observed. The most remarkable histological feature in our patients was the absence of eosinophils in tissue sections in spite of persistently high numbers of eosinophils in peripheral blood. In the first case, treatment of intestinal parasites before lymph node excision promptly reduced eosinophil counts to normal levels even…

Antifilarial IgG4 antibodies in children from filaria-endemic areas correlate with duration of infection and are dissociated from antifilarial IgE antibodies.

To investigate the relationship of antifilarial IgG4 and IgE to the intensity of transmission and duration of filarial infections in endemic populations, antifilarial antibody levels in children residing in a village in Papua New Guinea where transmission of Wuchereria bancrofti was reduced by repeated insecticide spraying were compared with levels in residents of three nearby villages where no control measures had been used. Antifilarial IgG4 levels were significantly lower in children from the sprayed village than in children or adults in nonsprayed villages (P < .01) and correlated with age…

The use of monoclonal antibodies in the detection of circulating antigens in Malayan filariasis.

Wuchereria bancrofti, Brugia malayi and Brugia timori are the causative agents of lymphatic filariasis in Indonesia but in some endemic areas, B malayi is more commonly found. Diagnosis of filariasis is normally based on clinical, parasitological and immunological examinations but those methods have limitations. The discovery of monoclonal antibodies is expected to provide a new dimension to the efforts in the development of specific and sensitive immunological tests for the various stages of filariasis infection. This preliminary report, using monoclonal antibodies and dot-blot assay in human…

The dynamics of infection and disease in bancroftian filariasis.

This study examines the relationship between the dynamic of Wuchereria bancrofti infection and the development of chronic lymphatic disease. Data sets from Pondicherry, south India, and Calcutta are used to estimate the age-specific proportion of the endemic population which has converted from microfilaria positive to amicrofilaraemia, and is assumed to be at risk of disease. For men, but not women, the age-prevalence profile of the estimated population 'at risk' is shown to correspond closely to the observed age-prevalence of chronic lymphatic disease in the same community. For both sexes, an…

Changes in circulating parasite antigen levels after treatment of bancroftian filariasis with diethylcarbamazine and ivermectin.

This study assessed changes in circulating parasite antigen levels after diethylcarbamazine (DEC) and ivermectin treatment of bancroftian filariasis to determine effects of these drugs on adult Wuchereria bancrofti in vivo. Thirty adult Haitians with microfilaremia were treated with 1 mg of ivermectin to reduce counts of microfilariae. Later, subjects were treated with either one or two 200 micrograms/kg doses of ivermectin or with 12 daily 6 mg/kg doses of DEC. Macrofilaricidal activity of these drugs was indirectly monitored by measuring circulating W. bancrofti antigen by EIA. Antigen level…

Bancroftian filariasis in Haiti: preliminary characterization of the immunological responsiveness of microfilaremic individuals.

Patent infections with the lymphatic filariae, Wuchereria bancrofti and Brugia malayi, are associated with suppressed in vitro cellular responsiveness to filarial antigens. In studies of bancroftian filariasis in Haiti, a significant number of microfilaremic individuals can be characterized as "responders" to filarial antigens. Cells from 37/74 untreated microfilaremic subjects responded to B. pahangi antigen (stimulation ratio greater than 2) as detected by in vitro blastogenesis. A comparison of responders to nonresponders revealed a significant difference in mean B. pahangi reactivity (15,8…

Epidemiological investigations of Bancroftian filariasis in the coastal zone Liberia.

A survey in 5 study areas along the Liberian coast was conducted to gain information on the prevalence and importance of bancroftian filaria … from the survey results that in a population of 71,0000 there are 16,0000 to 20,0000 infected with Wuchereria bancrofti. An estimated 2500 to 40000 men suffering from hydroceles and 3700 to 40000 elephantiasis patients demonstrate that bancroftian filariasis has to be regarded a health problem in rural coastal Liberia.

Identification of key pharmacophore feature motifs from the potent leads of the major antioxidant enzymes of W. bancrofti and in vitro validation studies of the key enzyme WbPrx6 using the model organism S. digitata.

Lymphatic filariasis (LF), or elephantiasis, is a mosquito-borne parasitic disease that causes severe disability. A neglected tropical disease, LF is the second leading cause of permanent disability worldwide, mainly in tropical and subtropical regions. It is caused by Wuchereria bancrofti (90 %), Brugia malayi, and Brugia timori. The Current treatments are limited, cannot reverse chronic damage, and may cause side effects or resistance. This study aims to identify the key pharmacophore features in potent leads of major antioxidant enzymes (Cu/Zn SOD1, Prx6, Tpx1, GPx and GST) of W. bancrofti,…

Comparing antigenaemia- and microfilaraemia as criteria for stopping decisions in lymphatic filariasis elimination programmes in Africa.

BACKGROUND: Mass drug administration (MDA) is the main strategy towards lymphatic filariasis (LF) elimination. Progress is monitored by assessing microfilaraemia (Mf) or circulating filarial antigenaemia (CFA) prevalence, the latter being more practical for field surveys. The current criterion for stopping MDA requires <2% CFA prevalence in 6- to 7-year olds, but this criterion is not evidence-based. We used mathematical modelling to investigate the validity of different thresholds regarding testing method and age group for African MDA programmes using ivermectin plus albendazole. METHODOLGY/

Filarial excretory-secretory products induce human monocytes to produce lymphangiogenic mediators.

The nematodes Wuchereria bancrofti and Brugia spp. infect over 120 million people worldwide, causing lymphedema, elephantiasis and hydrocele, collectively known as lymphatic filariasis. Most infected individuals appear to be asymptomatic, but many exhibit sub-clinical manifestations including the lymphangiectasia that likely contributes to the development of lymphedema and elephantiasis. As adult worm excretory-secretory products (ES) do not directly activate lymphatic endothelial cells (LEC), we investigated the role of monocyte/macrophage-derived soluble factors in the development of filaria…

Lymphatic filariasis in the Caribbean region: the opportunity for its elimination and certification.

In order to support the case for a certification of elimination of lymphatic filariasis (LF) in some Caribbean countries, we compared the prevalence of circulating Wuchereria bancrofti antigen in communities in Guyana, Suriname, and Trinidad. For the study, we assayed school children in six communities in Guyana, five communities in Suriname, and three communities in Trinidad for the prevalence of circulating W. bancrofti antigen, using a new immunochromatographic test for LF. We also assayed adults in these three countries, with a special focus on Blanchisseuse, Trinidad, where mass treatment…

[On the transmission role of residual microfilaremia cases in the area with filariasis virtually eradicated].

The transmission role of the residual microfilaremia cases was studied in Pingle Village, Liangqing Township, Yongning County where bancroftian filariasis had been virtually eradicated during 1984-1988. The results indicated that after the microfilarial rate of the human population met the criterion of elimination of filariasis, namely below 1% of the total population in all the administrative villages, and the average density of microfilaria being around 5 per 60 microliters peripheral blood, subsequent control measures might be no longer instituted while the microfilaria rate and its density
