A case of Sjögren's syndrome complicated by nephrogenic diabetes insipidus and renal tubular acidosis.

Abstract We describe the case of a 46-year-old woman with Sjögren's syndrome (SS) presenting with a 6-year history of polyuria and polydipsia. Laboratory data revealed hyperchloremic metabolic acidosis, a normal anion gap, and an inability to acidify urine following an acid loading test and to concentrate the urine in response to water deprivation and antidiuretic hormone administration. Lymphocyte infiltration in the interstitium was found on renal biopsy. These findings allowed us to diagnose distal renal tubular acidosis (RTA) and nephrogenic diabetes insipidus (NDI). Steroid pulse therapy

Transdermal iontophoresis and solute penetration across excised human skin.

The potential of transdermal iontophoresis to facilitate drug delivery was evaluated by studying the transport kinetics of model compounds salicylate (anion), phenylethylamine (cation), mannitol (polar neutral compound of low molecular weight), and inulin (polar neutral compound of high molecular weight) using an excised human skin model. The transport kinetics of solutes were determined across both intact and cellophane-tape-stripped dermatomed skin both in the presence and absence of applied current to probe the mechanisms of iontophoretic delivery. Iontophoresis effectively enhanced deliver

Pathophysiology and treatment of hypertension and oedema due to renal failure.

Hypertension and oedema resulting from sodium retention are the main indications for diuretic treatment in patients with chronic renal failure. The pharmacokinetics of most loop diuretics and their active metabolites are altered in chronic renal failure, due both to reduced transtubular transport via the proximal tubular anion transporter and to protein binding within the tubule. In chronic renal failure, the reduced number of nephrons leads to adaptive reduction of tubular sodium reabsorption, i.e. high baseline fractional natriuresis and increased absolute distal tubular sodium reabsorption.

Isolation and characterization of oryzatensin: a novel bioactive peptide with ileum-contracting and immunomodulating activities derived from rice albumin.

A novel bioactive peptide was isolated from the tryptic digest of rice soluble protein based on ileum-contracting and anti-opioid activities in the isolated guinea pig ileum. The structure of the peptide was Gly-Tyr-Pro-Met-Tyr-Pro-Leu-Pro-Arg, and it was named oryzatensin. Oryzatensin showed a biphasic ileum-contraction, which was characterized by a rapid contraction followed by a slower one. The latter was mediated by the cholinergic nervous system because it was inhibited by tetrodotoxin and atropine. Although oryzatensin showed weak affinity for mu-opioid receptors, the apparent anti-opioi

Analysis of carbohydrates in glycoproteins by high-performance liquid chromatography and high-performance capillary electrophoresis.

We describe two methods for the analysis of oligosaccharide chains in glycoproteins by high-performance liquid chromatography (HPLC) and high-performance capillary electrophoresis (HPCE). O- and N-glycosidically linked oligosaccharides released from glycoproteins can be identified as their borohydride-reduced forms by anion-exchange HPLC with pulsed amperometric detection. N-Glycosidically linked oligosaccharides can also be analyzed as 2-aminopyridine derivatives by HPCE in direct zone electrophoresis mode in an acidic phosphate buffer and zone electrophoresis mode as borate complexes in an a

Sulfated glycosaminoglycans in amyloid plaques of prion diseases.

Brain sections from cases of human Creutzfeldt-Jakob disease, Gerstmann-Sträussler syndrome, kuru, and hamster scrapie containing amyloid were examined for the presence of sulfated glycosaminoglycans (GAGs), the anionic component of proteoglycans, using the sulfated Alcian blue method and Alcian blue technique with 0.3 M and 0.7 M magnesium chloride. These studies suggest that sulfated glycosaminoglycans are part of the CNS amyloid plaques in each of the above human prion disorders as well as in experimental scrapie. All the amyloid plaques stained positively with Alcian blue at 0.3 M, and les

Comparative kinetics and inhibition of a carbonic anhydrase from Chlamydomonas reinhardii.

A native carbonic anhydrase (CA) from Chlamydomonas reinhardii exhibited an apparent molecular weight of 165,000. Denaturation under reducing conditions yielded a single polypeptide, mol. wt 42,000, while the same treatment under nonreducing conditions yielded higher molecular weight aggregates at least two of which could be reactivated to exhibit enzyme activity. In CO2 hydration the enzyme has a Km value of 3.4 mM and a relatively high turnover number. It is very sensitive to inhibition by sulfonamide and moderately sensitive to anion inhibition. In general, the C. reinhardii enzyme resemble

Metabolic alkalosis in diabetic ketosis: a case report.

A mixed metabolic alkalosis and metabolic acidosis, resulting in an alkalemic state, occurred in a hyperlipemic patient with previously diagnosed non insulin dependent diabetes. The metabolic alkalosis, due to large loss of gastric HCl, was more severe than the diabetic acidosis and resulted in an alkaline blood pH. Initially the metabolic acidosis was due to ketoacidosis and coexistent lactic acidosis. During the improvement of the alkalemic and hyperglycemic state, lactic acidosis disappeared but a paradoxical rise of plasma NEFA and ketone body concentrations supervened so that the high ani

Hyporeninemic hypoaldosteronism in a patient with multiple myeloma.

A patient with progressive renal failure due to multiple myeloma presented with a mixed acid-base disorder (non-anion gap acidosis and respiratory alkalosis) with persistent severe hyperkalemia. Studies revealed an intact ability to lower urine pH during acid loading, markedly decreased plasma renin and aldosterone concentrations despite volume depletion, and an inappropriately low fractional excretion of potassium. Renal biopsy demonstrated plasma cell infiltration of the renal interstitium and typical proteinaceous intratubular casts. Both proximal and distal renal tubular acidification defe

Inhibition of neutrophil activation by nonsteroidal anti-inflammatory drugs.

Nonsteroidal anti-inflammatory drugs are thought to prevent inflammation in rheumatoid arthritis by inhibiting prostaglandin synthesis. This observation does not explain, however, why nonsteroidal anti-inflammatory drugs are able to control inflammation caused by other mediators. To determine whether nonsteroidal anti-inflammatory drugs also exert an effect on neutrophil activation, in vitro and in vivo studies were undertaken. Aggregation, superoxide anion generation, and lysosomal enzyme release were assessed. The nonsteroidal anti-inflammatory drugs were found to inhibit these neutrophil re

Effect of taurine on the gastric absorption of drugs: comparative studies with sodium lauryl sulfate.

In order to clarify the mechanism of taurine-induced enhancement of drug absorption, some comparison was made between the actions of taurine and sodium lauryl sulfate (SLS), an anionic surfactant, by using the loop of rat stomach in situ. SLS enhanced the gastric absorption of aspirin and salicylamide, and the SLS-induced enhancement seems to be Na+-dependent. SLS failed to enhance the absorption of both drugs when NaCl was completely replaced by mannitoL or in the presence of ouabain even in the NaCL medium. In addition, even at the lower concentration of 0.05 mM, SLS enhanced the absorption

Regulation of aconitate hydratase activity from rat kidney cortex by bicarbonate.

1. The increase in pH value and bicarbonate concentration stimulated citrate synthesis from pyruvate and malate, inhibiting simultaneously conversion of isocitrate to citrate. 2. Bicarbonate inhibited competitively the activity of aconitate hydratase, probably binding with the two active sites of the enzyme. The Ki values for the cytoplasmic and mitochondrial enzyme were, respectively, 27 and 38 mM. The pH optimum for both forms of the enzyme in Tris-HCl buffer was in the range 7.8-8.6, and in bicarbonate buffer varied from 7.2 to 8.0, depending on the form of the enzyme and the substrate used

Interaction of ambrisentan with clarithromycin and its modulation by polymorphic SLCO1B1.

PURPOSE: We assessed the effect of cytochrome P450 (CYP) 3A4 and the OATP1B1 inhibitor clarithromycin on ambrisentan steady-state kinetics and its relationship to the SLCO1B1 15 haplotype in healthy volunteers. METHODS: In this open-label, monocenter, one-sequence crossover clinical trial ten male healthy participants were stratified according to CYP2C19 and SLCO1B1 (encoding for OATP1B1) genotype into two groups: group 1 (n = 6), with CYP2C19 1/1 (extensive metabolizer, EM) and SLCO1B1 wild-type; group 2 (n = 4), with CYP2C19 EM and homozygous (n = 3) or heterozygous for SLCO1B1 15 (n = 1).

Organometallic-Free Catalytic Cross-Electrophile Methylation of Aryl Halides.

The "Magic Methyl Effect" stands for the welcoming effect of the methyl group in enhancing the potency of drug precursors. To date, no catalytic chemical route for methylation of aryl halides that excludes organometallic species is known. In this report, we introduce an organometallic-free cross electrophile methylation of aryl halides based on the generation of a methyl anion achieved under chemical conditions. It successfully methylated several strong bonds, such as CAr-Br, CAr-Cl, and CAr-F. This catalytic protocol was further diversified by preparing methyl derivatives of various clinicall

Chiral ruthenium complex/Ph2P(2-furyl)-catalyzed asymmetric nucleophilic addition of aryl aldehyde hydrazones to simple ketones.

An efficient synthesis of chiral tertiary alcohols through asymmetric nucleophilic addition reaction of aldehyde hydrazones to simple ketones (especially aryl/methyl ketones) has been well established. Aryl aldehydes could be used as latent benzyl carbanion equivalents via reductive polarity reversal. The key to the success of the asymmetric reaction was the combination of an achiral monophosphine ligand Ph2P(2-furyl) with a chiral diamine derivative, which enhanced the enantioselectivity and reactivity. Density functional theory calculations revealed the hydrogen bond interactions between the

Tipped Over the Edge: A Case of Diabetes Ketoacidosis Precipitated by High-Dose Steroids in the Setting of a Large Peritonsillar Abscess.

Peritonsillar abscess is an infection of tonsillar soft tissue which can spread into additional neck structures leading to symptoms of fever, sore throat, dysphagia, and airway compromise. We describe a case of diabetic ketoacidosis in a patient with a history of uncontrolled type II diabetes mellitus admitted for a peritonsillar abscess who received intravenous steroids for management of the abscess swelling. The patient was treated with an insulin drip, hydration, and electrolyte replacement with a resolution to his anion gap and metabolic acidosis. Diabetic ketoacidosis occurs during increa

Cysteine and resistance to oxidative stress: implications for virulence and antibiotic resistance.

Reactive oxygen species (ROS), including the superoxide radical anion (O2•-), hydrogen peroxide (H2O2), and the hydroxyl radical (•HO), are inherent components of bacterial metabolism in an aerobic environment. Bacteria also encounter exogenous ROS, such as those produced by the host cells during the respiratory burst. As ROS have the capacity to damage bacterial DNA, proteins, and lipids, detoxification of ROS is critical for bacterial survival. It has been recently recognised that low-molecular-weight (LMW) thiols play a central role in this process. Here, we review the emerging role of cyst

Multiple Dyselectrolytemia in a Chronic Alcohol Abuser: A Case Report.

Electrolyte disorders in alcohol-dependent patients can be due to a multitude of reasons. We discuss a patient with diabetes mellitus, seizure disorder, and alcoholism who presented with seizure episodes and vomiting following a binge alcohol intake. The evaluation showed life-threatening metabolic derangements that included hyponatremia, hypokalemia, hypomagnesemia, hypocalcemia, hypochloremia, hypophosphatemia with elevated blood glucose, and metabolic alkalosis with a normal anion gap. Subsequently, a detailed urinary analysis revealed a urinary loss of electrolytes. We emphasize that alcoh

A Co(II) compound: photocatalytic activity and application value in trigeminal neuralgia with minimally invasive interventional therapy guided by CT.

A new thermostable Co(II)-based compound, namely [Co3(L)2(HTEA)2]n (1, HL = isonicotinic acid, H3TEA = triethanolamine), has been successfully synthesized by the isomicotinic acid ligand and HTEA anion. The photocatalytic property of 1 was also investigated, indicating that it shows excellent photocatalytic activity for the degradation of Rhodamine B (MB) solution under the UV light irradiation. For the treatment of trigeminal neuralgia, the content of the inflammatory cytokines released into the trigeminal ganglion tissue fluid was measured with enzyme-linked immunosorbent assay (ELISA) assay

Distal renal tubular acidosis and lethargy associated with zonisamide treatment in a dog with idiopathic epilepsy.

A 3-year-old neutered male golden retriever administered zonisamide for the treatment of seizures showed lethargy and had normal anion gap metabolic acidosis with hypokalaemia, hyperchloremia, and alkaline urine. The serum zonisamide concentration was close to the upper limit, which raised a suspicion of adverse effects of zonisamide. This is the first report showing that the fractional excretion of bicarbonate after compensation for the plasma bicarbonate concentration by a sodium bicarbonate infusion was approximately 5%, indicating distal renal tubular acidosis (RTA). The serum zonisamide c
