A 42 year old man presented with classical presentation of Acute ST elevation MI of 2 hours duration. His CAG revealed a spared LMCA with a totally occluded LAD in proximal segment, for which he underwent a successful PAMI with DES to LAD. 2 days later he developed high grade fever, elevated inflammatory markers (ESR/CRP) with a dull aching pain in root of neck. We report a successful reoperation of aortic valve replacement with the severely calcified, ''porcelain'' aorta due to Takayasu's disease. The patient was a 46-year-old female diagnosed with aortic prosthetic valve failure. Takayasu's aortitis had been noted at 17 years old and the patient underwent aortic valve replacement for aortic regurgitation at age 32. A case of isolated angiitis of the CNS was observed for 5 years. Initial response to cyclophosphamide was followed by relapse on therapy interruption. After renewed treatment, clinical stabilization was achieved despite progressive stenoses shown by angiography. The patient died of cyclophosphamide-induced myelodysplastic syndrome. Autopsy revealed lack of inflammation, vascular scarring, and amyloid angiopathy. We conclude that cure from isolated angiitis of the CNS is possible and that the risk of overtreatment should be minimized.

The authors describe a case of Takayasu's disease accompanied by complete occlusion of the bilateral subclavian, left vertebral (VA), and right common carotid arteries (CCA) and by severe stenosis of the right VA and left CCA. The patency of the right external and internal carotid arteries was demonstrated preoperatively by rapid sequence computed tomographic scanning. The right CCA was successfully reconstructed with an aorta-CCA bifurcation bypass. Simultaneously, the stenotic left CCA was also reconstructed with an aorta-CCA bypass using a Y-shaped woven Dacron graft.

This study reports on 13 cases of extra-intracranial bypass procedures using saphenous vein grafts. Ten patients had arteriosclerotic occlusive vascular disease, one patient had a giant aneurysm of the internal carotid artery, another 2 females, 18 and 28 years of age, respectively, suffered from Takayasu's disease. A case of herpes zoster ophthalmicus was followed, after a latent period of four weeks, by contralateral hemiparesis. An attempt is made to clarify the anatomical relationships involved in the pathogenesis of postherpetic cerebral complications. Detailed presentation of neuropathological findings in this case may provide evidence of vasculitis in the syndrome, with better understanding of the pathological anatomy.
