Validation of a novel approach for dose individualization in pharmacotherapy using gabapentin in a proof of principles study.

STUDY OBJECTIVE: To demonstrate the premise of individualized dosing charts (IDCs) as a clinical-bedside decision-support tool to individualize dosage regimens for drugs in which the interpatient variability is controlled by the pharmacokinetic (PK) behavior of the patient, to calculate the optimal sampling schedule (OSS), which minimizes the number of blood samples per patient. The approach is illustrated with available PK data for gabapentin. DESIGN: Retrospective proof of principles study using gabapentin PK data from a published clinical trial. PATIENTS: Nineteen subjects in a trial desi

Positive impact of a large-scale HIV prevention programme among female sex workers and clients in South India.

OBJECTIVE: Estimate the potential impact of Avahan, the India AIDS Initiative, among female sex workers (FSWs) and their clients in five districts of Karnataka state, south India. DESIGN: Examination of time trends in sexually transmitted infection (STI)/HIV prevalence from serial cross-sectional surveys, combined with mathematical modelling. METHODS: Survey data from each district were used to monitor changes in FSW STI/HIV prevalence during Avahan. A deterministic model, parameterized with district-specific survey data, was used to simulate HIV/HSV-2/syphilis transmission among high-risk g

Impaired social brain network for processing dynamic facial expressions in autism spectrum disorders.

BACKGROUND: Impairment of social interaction via facial expressions represents a core clinical feature of autism spectrum disorders (ASD). However, the neural correlates of this dysfunction remain unidentified. Because this dysfunction is manifested in real-life situations, we hypothesized that the observation of dynamic, compared with static, facial expressions would reveal abnormal brain functioning in individuals with ASD.We presented dynamic and static facial expressions of fear and happiness to individuals with high-functioning ASD and to age- and sex-matched typically developing controls

Bayesian estimation of true prevalence, sensitivity and specificity of indirect ELISA, Rose Bengal Test and Slow Agglutination Test for the diagnosis of brucellosis in sheep and goats in Bangladesh.

The true prevalence of brucellosis and diagnostic test characteristics of three conditionally dependent serological tests were estimated using the Bayesian approach in goats and sheep populations of Bangladesh. Serum samples from a random selection of 636 goats and 1044 sheep were tested in parallel by indirect ELISA (iELISA), Rose Bengal Test (RBT) and Slow Agglutination Test (SAT). The true prevalence of brucellosis in goats and sheep were estimated as 1% (95% credibility interval (CrI): 0.7-1.8) and 1.2% (95% CrI: 0.6-2.2) respectively. The sensitivity of iELISA was 92.9% in goats and 92.0%

Cophylogeny on a fine scale: Geomydoecus chewing lice and their pocket gopher hosts, Pappogeomys bulleri.

Many species of pocket gophers and their ectoparasitic chewing lice have broadly congruent phylogenies, indicating a history of frequent codivergence. For a variety of reasons, phylogenies of codiverging hosts and parasites are expected to be less congruent for more recently diverged taxa. This study is the first of its scale in the pocket gopher and chewing louse system, with its focus entirely on comparisons among populations within a single species of host and 3 chewing louse species in the Geomydoecus bulleri species complex. We examined mitochondrial DNA from a total of 46 specimens of Ge

Possible risk modifications in the association between MnSOD Ala-9Val polymorphism and breast cancer risk: subgroup analysis and evidence-based sample size calculation for a future trial.

Manganese superoxide dismutase (MnSOD) has been identified as an important scavenger of reactive oxygen species (ROS), which can cause oxidative stress followed by breast cancer. A number of subsequent population-based studies have investigated the association between MnSOD Ala-9Val polymorphism and the risk of breast cancer. However, these studies have yielded conflicting results. This fact implies that the effect of MnSOD Ala-9Val polymorphism on the susceptibility to breast cancer may be modified by other risk factors. To provide a more definitive conclusion, a full meta-analysis combining

Mediastinal staging in lung cancer: a rational approach.

Mediastinal staging is generally performed by the use of imaging techniques such as CT (Computed Tomography) and PET (Positron Emission Tomography), mini-invasive techniques, as TBNA (Transbronchial Needle Aspiration), EBUS-TBNA (Ultrasound-Guided Transbronchial Needle Aspiration), EUS-FNA (Endoscopic Ultra Sound Fine-Needle Aspiration), and/or surgical techniques as mediastinoscopy, thoracoscopy, mediastinothomy. Each of these techniques provides different sensitivity, specificity and predictive value: all these characteristics need to be well considered and adequately used to achieve the bes

Population PK-PD model for Fc-osteoprotegerin in healthy postmenopausal women.

Osteoporosis is a metabolic bone disease resulting from increased bone resorption and characterized by low bone mass that leads to increased bone fragility and risk of fracture, particularly of the hip, spine and wrist. Bone resorption is dependent on receptor activator of NF-kappa B ligand (RANKL), which binds to RANK receptor on preosteoclasts to initiate osteoclastogenesis and maintains osteoclast function and survival. To neutralize the effects of RANKL, the body naturally produces the protein osteoprotegerin (OPG), which acts as a decoy receptor for RANKL and contributes to bone homeostas

Symmetrical event-related EEG/fMRI information fusion in a variational Bayesian framework.

In this work, we propose a symmetrical multimodal EEG/fMRI information fusion approach dedicated to the identification of event-related bioelectric and hemodynamic responses. Unlike existing, asymmetrical EEG/fMRI data fusion algorithms, we build a joint EEG/fMRI generative model that explicitly accounts for local coupling/uncoupling of bioelectric and hemodynamic activities, which are supposed to share a common substrate. Under a dedicated assumption of spatio-temporal separability, the spatial profile of the common EEG/fMRI sources is introduced as an unknown hierarchical prior on both marke

Preoperative diagnosis of ovarian tumors using Bayesian kernel-based methods.

OBJECTIVES: To develop flexible classifiers that predict malignancy in adnexal masses using a large database from nine centers. METHODS: The database consisted of 1066 patients with at least one persistent adnexal mass for which a large amount of clinical and ultrasound data were recorded. The outcome of interest was the histological classification of the adnexal mass as benign or malignant. The outcome was predicted using Bayesian least squares support vector machines in comparison with relevance vector machines. The models were developed on a training set (n=754) and tested on a test set (n

Bayesian modeling of embryonic growth using latent variables.

In a growth model, individuals move progressively through a series of states in which each state is indicative of developmental status. Interest lies in estimating the rate of progression through each state while incorporating covariates that might affect the transition rates. We develop a Bayesian discrete-time multistate growth model for inference from cross-sectional data with unknown initiation times. For each subject, data are collected at only one time point at which we observe the state as well as covariates that measure developmental progress. We link the developmental progress variabl

Bayesian spatial analysis and disease mapping: tools to enhance planning and implementation of a schistosomiasis control programme in Tanzania.

OBJECTIVE: To predict the spatial distributions of Schistosoma haematobium and S. mansoni infections to assist planning the implementation of mass distribution of praziquantel as part of an on-going national control programme in Tanzania. METHODS: Bayesian geostatistical models were developed using parasitological data from 143 schools. RESULTS: In the S. haematobium models, although land surface temperature and rainfall were significant predictors of prevalence, they became non-significant when spatial correlation was taken into account. In the S. mansoni models, distance to water bodies an

Using and interpreting cost-effectiveness acceptability curves: an example using data from a trial of management strategies for atrial fibrillation.

BACKGROUND: The cost-effectiveness acceptability curve (CEAC) is a method for summarizing the uncertainty in estimates of cost-effectiveness. The CEAC, derived from the joint distribution of costs and effects, illustrates the (Bayesian) probability that the data are consistent with a true cost-effectiveness ratio falling below a specified ceiling ratio. The objective of the paper is to illustrate how to construct and interpret a CEAC. METHODS: A retrospective cost-effectiveness analysis of the Atrial Fibrillation Follow-up Investigation of Rhythm Management (AFFIRM) randomized controlled tria

[Cost-benefit ratio of tick-borne encephalitis vaccination of French troops based in Kosovo].

BACKGROUND: French military troops based in Kosovo are exposed to Tick-Borne Encephalitis (TBE) biorisk. An efficacious vaccine is available to prevent this disease, burdened by high morbidity and mortality. The epidemiologic features of TBE in Kosovo are unprecise. To help the French Department of Defense to make a decision about immunization against TBE, we performed a cost-benefit analysis. METHODS: By a Bayes'analysis decision, we have estimated the net benefits for a three-injection vaccine program versus absence of vaccination, for all French military troops based in Kosovo. Time expect

Probabilistic reasoning in obsessive-compulsive and delusional disorders.

BACKGROUND: Delusional disorder (DD) and obsessive-compulsive disorder (OCD) have been investigated in previous studies using probabilistic reasoning paradigms and abnormalities in each group have been reported. No study to date has compared results between these groups. This study compares patients with these disorders with those who have both phenomena. METHODS: Thirty subjects with DD, 29 with OCD and 16 with obsessive and delusional features were compared with 30 normal controls in a study of probabilistic reasoning using two different computer-based tasks involving a Bayesian paradigm.

Polygenic Risk Scores for Incident Dementia in the Multi-Ethnic Study of Atherosclerosis.

Over 75 Alzheimer's disease (AD) and dementia-associated variants have been identified through genome-wide association studies, but the utility of polygenic risk scores (PRS) for predicting AD and dementia in diverse and admixed populations remains unclear. We compared how PRS approaches differing in p-value thresholds, variant weights, and source ancestry perform in predicting dementia in 6338 African American, Chinese, Hispanic, and White individuals from the Multi-Ethnic Study of Atherosclerosis. We tested clumping and thresholding (C+T) methods with varying parameters against Bayesian appr

Information borrowing in phase II randomized dose-ranging clinical trials in oncology.

INTRODUCTION: Over the past decades, the advent of new therapeutics such as immunotherapies and targeted therapies has challenged conventional clinical trial designs, such as single-arm studies. Selecting a single dose in phase I trials with short follow-up, typically based solely on toxicity endpoints, has been shown to lead to suboptimal dosing decisions. Consequently, dose optimization is now increasingly encouraged in oncology. This study was motivated by the case of ibrutinib in chronic lymphocytic leukemia, for which the initially approved dose of 420 mg/day, determined using conventiona

Evaluating age-dependent transmission and vaccination policy in Singapore's SARS-CoV-2 epidemic: A computational modelling approach.

The SARS-CoV-2 pandemic in Singapore revealed pronounced age-specific differences in disease transmission and severity. To investigate these dynamics, we developed a semi-mechanistic Bayesian hierarchical model that jointly estimated SARS-CoV-2 transmission across seven age groups from 2021 to 2023. Our model integrates both case and hospitalization data, while accounting for vaccination rollouts and time-varying contact patterns between age groups. Using a non-parametric approach, we estimated changes in contact behaviour directly from the epidemiological data, avoiding assumptions about thei

Post-marketing safety concerns with foscarbidopa/foslevodopa: A pharmacovigilance study with disproportionality analysis based on FAERS.

Foscarbidopa/foslevodopa, a 24-hour subcutaneous infusion of levodopa/carbidopa prodrugs, is approved for advanced Parkinson disease with motor fluctuations. Due to limited post-marketing data, the association between this novel formulation and related adverse events (AEs), particularly infusion-related or neurological complications, remains underexplored. This study aims to identify potential safety signals of foscarbidopa/foslevodopa-associated AEs using data mining of the FDA Adverse Event Reporting System (FAERS). To quantify safety risk signals of foscarbidopa/foslevodopa, disproportional
