One-stage surgical correction of Shepherd's crook deformity in fibrous dysplasia using fibular strut allograft augmentation: a retrospective case-series.

PURPOSE: Shepherd's crook deformity is a characteristic complication of fibrous dysplasia (FD) of the femur, leading to coxa vara and mechanical instability. Two-stage surgical approaches involve initial valgus osteotomy followed by delayed intramedullary femoral stabilization, prolonged treatment, and increasing morbidity. This study evaluates a one-stage surgical correction that employs fibular strut allograft augmentation for both the femoral neck and shaft. METHODS: A retrospective review was performed on 17 patients with Shepherd's crook deformity who underwent one-stage hip lag screw an

Fixation and Grafting After Limited Debridement of Scaphoid Nonunions.

PURPOSE: To evaluate a surgical technique of treating nondisplaced waist and proximal pole scaphoid nonunions without avascular necrosis (AVN). METHODS: We performed a retrospective review of all patients with nondisplaced, scaphoid waist or proximal pole nonunions without AVN treated with the following technique. Two K-wires are positioned along the scaphoid axis to stabilize the proximal and distal poles. Debridement with a curette or burr is performed parallel to the nonunion site until the K-wires are visualized and punctate bleeding of the proximal and distal fragments is encountered. Th

Functional genioplasty in growing patients.

OBJECTIVE: To evaluate the role of age as a moderator of bone regeneration patterns and symphysis remodeling after genioplasty. METHOD: Fifty-four patients who underwent genioplasty at the end of their orthodontic treatment were divided into three age groups: younger than 15 years at the time of surgery (group 1), 15 to 19 years (group 2), and 20 years or older (group 3). Twenty-three patients who did not accept genioplasty and had a follow-up radiograph 2 years after the end of their orthodontic treatment were used as a control group. Patients were evaluated at three time points: immediate p

Reduction and fixation capabilities of different plate designs for pubic symphysis disruption: a biomechanical comparison.

BACKGROUND: Typical stabilisation of pelvic open book injuries consists in plate fixation of the symphysis, leading to many different plate designs and procedures that have evolved. However, implant loosening and development of chronic instability are still evident and represent major complications after plate fixation of the symphysis. The aim of this study was to analyse reduction and fixation capabilities of different classical plate techniques with dynamic compression (DC), prebending or modern interlocking screws. METHODS: Compression injuries (OTA B1.1) were simulated on synthetic compo

Delayed open reduction and fixation of ankle fractures.

Because of clinical circumstances in the period from 1970-1980, 26 consecutive ankle fractures were treated by delayed (14-31 days after injury) open reduction and internal fixation. Follow-up examination (two to 12 years) was obtained in 25 patients. Based on clinical performance index, outcome was judged satisfactory in 17 patients (68%). Seventeen (68%) had an anatomic reduction, and 15 (88%) of these 17 had a satisfactory result. Compared to a group of 26 similar patients treated within 24 hours after injury, the major effect of delay was an adverse influence on the probability of achievin

Treatment of pelvic fractures and associated dislocations.

Fractures of the pelvis result in high morbidity and mortality as a result of uncontrolled hemorrhage and associated soft tissue damage. Early reduction and fixation of these fractures has been shown to result in control of hemorrhage, decreased morbidity and mortality, and improved function. Seventeen pelvic fractures, so treated, are reported. Despite a high complication rate of almost 75%, surgical fixation resulted in rapid control of hemorrhage, restoration of hip joint congruity, early mobilization of the patient, and decreased incidence of decubitus ulcers, bronchopneumonia, thrombophle

Reverse Passing of Anterior Cruciate Ligament Reconstruction With Simultaneous Anterolateral Ligament Reconstruction Using Hamstring Autograft.

Anterior cruciate ligament reconstruction continues to evolve as surgical techniques aim to optimize graft placement, increase rotational control, and reduce graft failure in high-risk populations. This technical note describes an inverse anterior cruciate ligament reconstruction combined with an anatomic anterolateral ligament reconstruction, incorporating a retrograde tibial half-tunnel and outside-in femoral drilling. The technique is compatible with hamstring, quadriceps tendon, and bone-patellar tendon-bone autografts. Tibial fixation is achieved using a suspensory button that can be dire

Design and Manufacturing Process of 3D-Printed Metamaterial Calcaneal Plates via Scalar Field-Driven Porous Structure Fusion.

To develop a metamaterial calcaneal bone plate with integrated mechanical and biofunctional properties, this study investigated its design and manufacturing methodologies. A solid calcaneal bone plate was first designed via a hybrid reverse-forward engineering approach. Subsequently, the metamaterial plate was realized through topology optimization combined with porous structure filling, utilizing scalar field-driven fusion to integrate distinct porous structures. Finally, the plate was directly fabricated by 3D printing and postprocessed. Results show the hybrid-designed solid plate exhibits

Emerging MRI-based spine scoring techniques targeting bone quality to assess osteoporosis, vertebral fracture risk, other spinal degenerative diseases, and post-surgical outcomes.

Osteoporosis is a metabolic skeletal disease defined by reduced bone mass and a higher risk of vertebral fractures. Due to the increasing elderly population worldwide, it is considered a major healthcare challenge for now and the future. Magnetic resonance imaging (MRI), a non-radiation modality, is emerging as an opportunistic tool for assessing osteoporosis using T1-weighted images. While extensive research has been conducted in this area, a unified and comprehensive review encompassing current knowledge, methodologies, diagnostic efficacy, and prospective research direction is still needed.

Diagnostic, Surgical, and Technical Considerations for Lumbar Interbody Fusion in Patients with Osteopenia and Osteoporosis: A Systematic Review.

OBJECTIVE: Osteoporosis is increasing in incidence as the ageing population continues to grow. Decreased bone mineral density poses a challenge for the spine surgeon. In patients requiring lumbar interbody fusion, differences in diagnostics and surgical approaches may be warranted. In this systematic review, the authors examine studies performing lumbar interbody fusion in patients with osteopenia or osteoporosis and suggest avenues for future study. METHODS: A systematic literature review of the PubMed and MEDLINE databases was performed for studies published between 1986 and 2020. Studies e

Safety and Tolerability of Stromal Vascular Fraction Combined with β-Tricalcium Phosphate in Posterior Lumbar Interbody Fusion: Phase I Clinical Trial.

The rates of pseudarthrosis remain high despite recent advances in bone graft substitutes for spinal fusion surgery. The aim of this single center, non-randomized, open-label clinical trial was to determine the feasibility of combined use of stromal vascular fraction (SVF) and β-tricalcium phosphate (β-TCP) for patients who require posterior lumbar interbody fusion (PLIF) and pedicle screw fixation. Two polyetheretherketone (PEEK) cages were inserted into the intervertebral space following complete removal of the intervertebral disc. The PEEK cage (SVF group) on the right side of the patient w

A double-plating approach to distal femur fracture: A clinical study.

BACKGROUND: Locked plating is one of the latest innovative options for treating supracondylar femur fractures with relatively low failure rates. Single lateral plating was often found to have a relative higher failure rate. No clinical studies of double-plating distal femur fixation have thus far been reported. The aim of this study is to present our clinical experience with this surgical approach. PATIENTS AND METHODS: Thirty-two patients (26 females and 6 males, mean age 76 years, range 44-101) were included in the study. Eight of them patients had a periprosthetic stable implant fracture a

Mandibular reconstruction with a bioactive-coated cementless Ti6Al4V modular endoprosthesis in Macaca fascicularis.

The titanium mandibular modular endoprosthesis fixed with polymethylmethacrylate cement in the medullary space of the mandible has been introduced in previous studies. However, the internal parts of these devices have been found to be prone to loosening and wound dehiscence. The current study introduces a newly designed bioactive-coated cementless modular mandibular endoprosthesis, which was used for reconstruction in Macaca fascicularis. The devices were coated with hydroxyapatite (HA) and hydroxyapatite/bioglass (HA/BG) and implanted in unilateral mandibular segmental defects in nine monkeys
