PURPOSE: Our study examined whether reproductive and hormonal factors before, at the time of, or after radiation treatment for a first primary breast cancer modify the risk of radiation-induced second primary breast cancer.
METHODS AND MATERIALS: The Women's Environmental, Cancer and Radiation Epidemiology (WECARE) Study is a multicenter, population-based study of 708 women (cases) with asynchronous contralateral breast cancer (CBC) and 1399 women (controls) with unilateral breast cancer. Radiotherapy (RT) records, coupled with anthropomorphic phantom simulations, were used to estimate quadra

BACKGROUND: Reliable predictors of metastatic involvement would enable a better selection of candidate patients for sentinel lymph node biopsy (SLNB) and possibly allow identification of patients with such a low risk of axillary sentinel lymph node (SLN) involvement to be even spared SLNB.
METHODS: The authors evaluated 4351 consecutive patients surgically treated for breast carcinoma who also underwent SLNB. Clinicopathologic features significantly associated with SLN metastases by univariate analysis were included in a multivariate model.
RESULTS: By multivariate analysis, the prevalence o

Angiogenesis is essential for tumour growth and important in tumour metastasis and prognosis. Vascular endothelial growth factor (VEGF) stimulates endothelial proliferation in vitro and angiogenesis in vivo. VEGF expression has been correlated with high vascularity in tumours, including carcinoma of the breast. This study investigated VEGF expression and vascularity of invasive lobular (n = 10) and invasive ductal carcinoma (n = 28), and pure ductal carcinoma in situ of the breast (n = 33). VEGF protein expression was studied with immunohistochemistry and VEGF mRNA with in situ hybridization.

Cancer staging and grading are used to predict the clinical behavior of malignancies, establish appropriate therapies, and facilitate exchange of precise information between clinicians. The internationally accepted criterion for cancer staging, the tumor-node-metastasis (TNM) system, includes: (1) tumor size and local growth (T), (2) extent of lymph node metastases (N), and (3) occurrence of distant metastases (M). Clinical stage is established before initiation of therapy and is determined by physical examination, laboratory findings, and imaging studies. Pathologic stage is determined follow

Cancer-associated thrombosis (CAT) is a major cause of morbidity and mortality in oncology. Female-specific cancers-including breast, ovarian, endometrial, and cervical malignancies-demonstrate distinct thrombotic profiles influenced by hormonal mechanisms, anatomical features, and treatment strategies. This narrative review synthesizes epidemiological evidence, mechanistic pathways, and clinical trial data to characterize CAT risk across these cancer types. Key modulators of thrombosis include hormonal exposures, tumor histology, inflammatory signaling, tissue factor expression, and therapeut

The anterior (superficial) aspect of the mastectomy specimen is often treated as an anatomical boundary rather than a true resection margin. However, pathology sampling studies demonstrate that benign breast epithelium/residual breast tissue can be present at the superficial dissection plane when actively sought [e.g., benign ducts identified in 53% of additional superficial margin specimens after skin-sparing mastectomy (Cao et al. in Ann Surg Oncol 15(5):1372-1377, 2008) and residual breast tissue detected in 76.2% of intensively sampled mastectomy specimens (Griepsma et al. in Ann Surg Onco

Cancer screening seems intuitively logical: early detection to prevent worse outcomes. However, this article reveals the complexity behind this assumption. Cancer's linear growth model- on which early detection was based - proves outdated. Cancers grow at vastly different rates; some even stop or disappear spontaneously. The main problem is overdiagnosis: in Dutch breast cancer screening, approximately 2,000 women are treated annually for cancers that would never cause symptoms. Epidemiological data show a persistent increase in stage I cancers without a decline in stage IV cases - a sign of o

Invasive lobular carcinoma (ILC) accounts for 10-15% of breast cancers. Despite favorable responses to anti-estrogen therapy, the dissemination of cancer cells and resistance to therapies are significant risks for patients with ILC. Late recurrences are prevalent in ILC, suggesting that disseminated tumor cell (DTC) dormancy may be a mechanism preceding their late overt growth into metastatic lesions. Herein, we investigated the relationship between anti-estrogen resistance and dormancy through multidimensional, micro-compartmentalized in vitro models. The bioengineered platforms recapitulated

Breast-conserving surgery (BCS) is an effective treatment for early-stage cancer, provided that there is "no ink on tumor" reported in the excised breast specimen. However, approximately 15-35% of patients require additional surgery due to the presence of malignant cells at the surgical margin. In this review, we examine the available data on margin status in breast cancer and compare the advantages and disadvantages of different approaches in BCS margin evaluation. Of note, there are numerous concerns regarding traditional methods for assessing margins, and emerging technological approaches a

More than 20 years after the discovery of lunasin, a clear shift in lunasin research is observable-from an initial focus on its direct in vitro anticancer effects toward strategies aimed at improving its bioavailability and repositioning it as a potential adjunct in cancer therapy. Lunasin, a soy-derived bioactive peptide, has been extensively studied for its antineoplastic properties. However, its limited oral bioavailability restrains its efficacy in clinical trials. Therefore, recent research on lunasin points towards the possibility of using it as an adjunct in cancer treatment, rather tha

BACKGROUND: Immune checkpoint inhibitors (ICIs) targeting the PD-1/PD-L1 axis have demonstrated clinical benefit in early-stage breast cancer (eBC), enhancing antitumor immune responses. However, their incorporation into perioperative polychemotherapy has introduced new concerns regarding safety and immune-related toxicities (irAEs). We conducted a systematic review and meta-analysis to comprehensively evaluate the safety profile and tolerability of ICIs in patients with eBC.
METHODS: A systematic review was conducted in PubMed, Embase, Cochrane Library, and relevant conference databases (ASC

Syringomatous tumor of the nipple (SyT) is a rare benign tumor that arises in the nipple or areolar region. Due to its locally infiltrative growth pattern, it is often misdiagnosed as a malignant tumor. SyT primarily occurs in middle-aged women, with a reported mean age of onset of 46.1 years. Fewer than 100 cases have been documented in the literature to date, and occurrence in adolescents is exceedingly rare. This case report describes a rare instance of SyT in a 16-year-old asymptomatic female, who presented with progressive unilateral nipple enlargement over 5 years. Breast ultrasound and

After 60 years of surgical service and the creation/development of three breast centers at the Van Nuys Breast Center, the University of Southern California (USC), and the Hoag Family Cancer Institute, Dr. Melvin J. Silverstein has announced his retirement. Dr. Silverstein founded the first free-standing breast center in America, authored the first book devoted solely to ductal carcinoma in situ (DCIS), and was a pioneer in oncoplastic breast-conservation surgery. One of his many contributions to the field of breast cancer care was the development of the Van Nuys DCIS Prognostic Index (VPNI).

Metastatic breast tumors are rare and often misdiagnosed as primary breast cancer. Herein, we present a case of breast metastasis from malignant melanoma. A 46-year-old woman presented to our department with a lump in her left breast. Examination revealed a relatively soft mass measuring 4 cm in diameter in the left breast. A needle biopsy was performed, and immunostaining for S-100, human melanoma black-45, CD56/neural cell adhesion molecule, and Melan-A confirmed a diagnosis of malignant melanoma. The metastasis was confined to the breast and was thus treated by surgery to excise the tumor.

We present 2 women with advanced invasive ductal carcinoma who developed skeletal metastases despite extensive treatment, including surgery, chemotherapy, and hormonal therapy. Both patients experienced debilitating pain and were evaluated for bone palliation therapy using 177 Lu-FAPI-2286, after 99m Tc-FAPI-46 scintigraphy showed suitable uptake. Remarkably, both patients reported significant pain relief shortly after treatment. The only adverse effect noted was grade III thrombocytopenia in 1 patient. These cases highlight the potential of 177 Lu-FAPI-2286 in providing rapid and effective pa

As the number of curatively treated breast cancer patients grows and the burden on healthcare increases, effective use of methods to detect recurrences is becoming more important. This systematic review aims to examine the added value of physical examination (PE) in addition to mammography for breast cancer recurrence detection. PubMed, Web of Science, and Scopus were searched. Observational studies including women with a primary diagnosis of invasive or in situ breast cancer without metastases, which reported method of detection for locoregional recurrences (LRRs), and in which PE was defined

Cutaneous metastasis (CM) is a rare phenomenon, occurring in only 0.6% to 10.4% of oncology patients. Breast carcinoma represents nearly one-third of all CM cases, although it manifests in a minority of patients with breast cancer (BC). Notably, CMs are detected at the time of primary tumor diagnosis in 6.3% of patients with BC and serve as the initial clinical presentation in only 3.5% of patients. We present a case involving a 53-year-old woman with a 5-month history of asymptomatic infiltrating nodules in the axillae and trunk. Skin biopsy and immunohistochemical analysis confirmed metastat

Angiosarcoma of the breast is a rare malignant tumor and accounts for 1% of all soft tissue tumors of the breast. Breast angiosarcoma caused by previous radiation therapy more frequently affects older women with a history of breast cancer. The standard treatment for such patients is radical surgery, but there are still many debatable questions regarding the treatment of these malignant tumors. The purpose of this article is to review the medical literature on angiosarcomas associated with radiotherapy and describe a case as an example.

Breast cancer presents a significant risk to public health and is the primary cause of cancer-related death in women. Awareness of metastatic breast cancer (mBC) continues to increase, and advances have been made; however, challenges remain for many patient populations that do not receive equal opportunities along the treatment pathway. The Underserved Patient Population (UPP) Coalition Task Force, a group of international experts in mBC, held meetings between 2022 and 2023 to prioritise the needs of UPPs and propose solutions. The key unmet needs identified included the following: delayed dia

Leptomeningeal disease (LMD) associated with breast cancer (BC), characterized by the invasion of metastatic BC cells into the leptomeninges and cerebrospinal fluid, poses a significant clinical challenge. Current management strategies are not curative but rather aim to slow the rapid clinical decline associated with LMD, each with its own set of limitations. For instance, systemic chemotherapy faces delivery barriers while intrathecal administration directly targets the site of disease but struggles with uneven drug distribution, toxicity, and limited efficacy. Radiation therapy, including wh
