Model-Informed Recommendation of Mavacamten Posology for Chinese Adults With Obstructive Hypertrophic Cardiomyopathy.

Mavacamten is a cardiac myosin inhibitor for adults with obstructive hypertrophic cardiomyopathy (HCM). Dose optimization is performed 4 weeks after starting mavacamten, guided by periodic echo measurements of Valsalva left ventricular outflow tract gradient (VLVOTg) and left ventricular ejection fraction (LVEF). Previously, a population pharmacokinetic (PPK) model was developed and exposure-response (E-R) of VLVOTg (efficacy) and LVEF (safety) was used to identify the mavacamten titration regimen with the optimal benefit/risk ratio, now included in the US prescribing information. Inhibition of Soluble Epoxide Hydrolase Reduces Inflammation and Myocardial Injury in Arrhythmogenic Cardiomyopathy.

We analyzed the role of pro- and anti-inflammatory eicosanoids in the pathogenesis of arrhythmogenic cardiomyopathy (ACM). Lipidomics revealed reduced levels of anti-inflammatory oxylipins in plasma and increased levels of pro-inflammatory eicosanoids in hearts of Dsg2mut/mut mice, a preclinical model of ACM. Disease features were reversed in vitro in rat ventricular myocytes expressing mutant JUP by the anti-inflammatory epoxyeicosatrienoic acid 14-15-EET, whereas 14,15-EEZE, which antagonizes the 14,15-EET receptor, intensified nuclear accumulation of the desmosomal protein plakoglobin. Efficacy and safety of myectomy and radiofrequency septal ablation for treating hypertrophic obstructive cardiomyopathy.

BACKGROUND: Novel treatments are needed for patients with severely symptomatic obstructive hypertrophic cardiomyopathy (oHCM). Radiofrequency (RF) septal ablation has emerged as a promising technique for improving the left ventricular outflow tract (LVOT) gradient and alleviating symptoms, potentially achieving outcomes comparable to the gold-standard surgical myectomy. OBJECTIVES: To compare the 1-year efficacy and safety of surgical myectomy with RF septal ablation in patients with oHCM. METHODS: This observational, retrospective cohort study included patients aged≥18 years with oHCM and a

Outcomes of septal myectomy in patients with obstructive hypertrophic cardiomyopathy and minimal septal hypertrophy.

OBJECTIVE: To describe the clinical characteristics of patients with obstructive hypertrophic cardiomyopathy and less severe hypertrophy (septal thickness <15 mm) undergoing transaortic septal myectomy, explore possible mechanisms of obstruction, and assess operative outcomes. METHODS: We identified 51 consecutive patients with septal thicknesses <15 mm who underwent septal myectomy for obstructive hypertrophic cardiomyopathy between 2000 and 2021. They were matched in a 1:2 ratio with patients with obstructive hypertrophic cardiomyopathy with septal thickness ≥15 mm undergoing myectomy. RES

Anterior mitral valve leaflet length and response to mavacamten in obstructive hypertrophic cardiomyopathy.

OBJECTIVES: This study examines whether anterior mitral valve leaflet (AMVL) length is associated with response to mavacamten in patients with obstructive hypertrophic cardiomyopathy (HCM). AIMS: Obstruction of the left-ventricular outflow tract (LVOT) in HCM has been associated with asymmetric septal hypertrophy and abnormalities of the mitral valve and sub-valvular apparatus. Mavacamten is a myosin-inhibitor shown to decrease LVOT gradient and improve functional status in patients with obstructive HCM. METHODS AND RESULTS: Measurements of cardiac structural elements were obtained from magn

Natural History, Phenotype Spectrum, and Clinical Outcomes of Desmin (DES)-Associated Cardiomyopathy.

BACKGROUND: Pathogenic/likely pathogenic (LP) desmin (DES) variants cause heterogeneous cardiomyopathy and skeletal myopathy phenotypes. Limited data suggest a high incidence of major adverse cardiac events (MACEs), including cardiac conduction disease, sustained ventricular arrhythmias (VA), and heart failure (HF) events (HF hospitalization, left ventricular assist device/cardiac transplant, HF-related death) in patients with pathogenic/LP DES variants. However, pleiotropic presentation and small cohort sizes have limited clinical phenotype and outcome characterization. We aimed to describe t

Alcohol-Induced Cardiomyopathy Presenting With a Ventricular Arrhythmia Storm Causing Cardiac Arrest.

Alcohol has both protective and deleterious effects on the heart. Alcohol consumed in small quantities can have cardiovascular protective effects. However, consumption of excessive amounts of alcohol has been associated with electrolyte imbalances, neurologic disorders like Wernicke's encephalopathy and Korsakoff syndrome, cardiomyopathies, cardiac arrhythmias and sudden cardiac death. We present and discuss the diagnosis and management of a 26-year-old male patient with a history of alcohol use disorder who presented to our center with alcohol-induced cardiomyopathy causing a ventricular arrh…

Unveiling Mixed Apical Hypertrophic Cardiomyopathy: A Case Study on Hypovolemia-Induced Syncope.

Mixed apical hypertrophic cardiomyopathy (MAHCM) is a rare variant of hypertrophic cardiomyopathy characterized by hypertrophy of the left ventricular (LV) apex with septal involvement, increasing the risk of left ventricular outflow tract (LVOT) obstruction, mid-ventricular obstruction, and adverse cardiovascular events such as syncope and sudden cardiac death. We describe an 84-year-old female with a history of hypertension and hyperlipidemia who presented with a syncopal episode preceded by dizziness, blurred vision, and diaphoresis, with a history of poor oral intake and weight loss. She w

Peripartum Cardiomyopathy Associated With Gestational Transient Thyrotoxicosis and Severe Preeclampsia: A Case Report.

Human chorionic gonadotropin (hCG) exerts a thyrotropic effect and contributes to the pathophysiology of gestational transient thyrotoxicosis (GTT), a typically benign, self-limiting form of hyperthyroidism that usually resolves by the late first or early second trimester of pregnancy, which is generally considered to have minimal obstetric or fetal consequences. However, identifying hyperthyroidism in pregnancy remains essential, as maternal and fetal complications may occur depending on the etiology and the presence of overt hyperthyroidism. Thyroid hormones significantly influence cardiovas

Goreisan suppresses cardiac remodeling and dysfunction in a new mouse model with diabetic cardiomyopathy.

BACKGROUND: The global increase in diabetes, driven by aging populations and lifestyle changes, has led to an increase in the incidence of diabetic cardiomyopathy (DCM). DCM is characterized by metabolic abnormalities, oxidative stress, and inflammation, leading to cardiac remodeling and dysfunction. Goreisan (GRS), a traditional Japanese Kampo medicine, is commonly used to treat fluid control such as edema, due to its diuretic effect. In this study, we examined the effects of GRS on DCM. METHODS: We first established a new mouse model of DCM and then evaluated the effects of GRS on DCM using

Septal Myectomy in Pediatric Obstructive Hypertrophic Cardiomyopathy: A Systematic Review and Meta-analysis.

Evidence regarding outcomes of septal myectomy in pediatric patients with obstructive hypertrophic cardiomyopathy (HOCM) remains limited. This meta-analysis aimed to assess surgical outcomes and long-term survival in this population. A systematic review was conducted using PubMed, Embase, Scopus, and Cochrane databases to identify studies evaluating septal myectomy in pediatric patients with obstructive HCM. Eight clinical trials comprising a total of 490 pediatric patients met the inclusion criteria. Among children undergoing septal myectomy for HOCM, the pooled early and late mortality rates

Phenotypic overlap in apical hypertrophic cardiomyopathy: one size does not fit all.

Hypertrophic cardiomyopathy (HCM) is characterised by left ventricular hypertrophy with several well-defined phenotypes varying in morbidity and mortality risk. However, phenotypic overlap is seen between apical and mid-ventricular HCM subtypes. We present the case of a woman in her late 20s for whom multimodality imaging rendered a phenotypically overlapping apical HCM (ApHCM) with disparate levels of risk for sudden cardiac death. In doing so, this case illustrates the importance of understanding the complementary role of multimodality imaging in the management of HCM patients with phenotypi
