Use of MR in Pancreaticobiliary Emergencies.

This article presents the MR protocols, imaging features, diagnostic criteria, and complications of commonly encountered emergencies in pancreaticobiliary imaging, which include pancreatic trauma, bile leak, acute cholecystitis, biliary obstruction, and pancreatitis. Various classifications and complications that can arise with these conditions, as well as artifacts that may mimic pathology, are also included. Finally, the emerging utility of abbreviated MR protocols is discussed.

Biliary tract infection caused by Haemophilus parainfluenzae.

Haemophilus parainfluenzae was isolated from the bile specimens of 2 patients with acute cholecystitis. The strains were genetically unrelated by the random amplified polymorphic DNA (RAPD). The 2 H. parainfluenzae strains represented 11.7% of all positive bile cultures inoculated in blood culture bottles. The routine inoculation of bile specimens in media able to support growth of Haemophilus species should be considered.

An internist's approach to acute abdominal pain.

To understand the surgical approach to acute abdominal pain, the internist must be familiar with common presentations of most abdominal emergencies; these emergencies include acute appendicitis, acute gall bladder disease (biliary colic, acute cholecystitis, and acute pancreatitis), ischemic bowel disease and ischemic colitis, abdominal aortic aneurysm, and intestinal obstruction. Nothing compares to experience; this article reviews the salient points that deserve consideration.

[Influence of sonography on indications for laparoscopic cholecystectomy].

Because of therapeutic consequences preoperative sonography should be performed with a check list. Nevertheless false positive and false negative findings result in cases of gall-stones in the ductus cysticus, of acute cholecystitis and thickness of the gall-bladder wall. Therefore where sonographic findings differ from the intraoperative laparoscopic situs the operating surgeon should change to conventional cholecystectomy early enough to avoid complications.

Spontaneous lesions of the mouse biliary tract.

During the routine histological examination of 2400 aged mice from lifespan carcinogenicity studies a low prevalence of glandular metaplasia of the gall bladder and of the intra-hepatic bile ducts (all associated with cholelithiasis), cholecystitis, cholangitis, papillomatous hyperplasia, papilloma, intra-mural cysts, and focal epithelial ulceration was found. The lesions were found predominantly in female mice and were not treatment related.

Fortuitous findings of Tc-99m PIPIDA hepatobiliary scintigraphy.

Hepatobiliary scintigraphy using Tc-99m labeled radiopharmaceuticals is employed primarily for the diagnosis of acute cholecystitis and for demonstration of biliary tract potency. This produce may also provide diagnostic information in the identification of unusual or atypical hepatobiliary disorders; frequently, this occurs as an incidental result of the test. We present five cases in which trauma, infection, or obstructive abnormalities were diagnosed fortuitously.

Surgical Management of a Complex High Hepatic Duct Injury: A Case Report.

This is a case of a patient who underwent an elective laparoscopic cholecystectomy after previously presenting with acute cholecystitis. His operation was complicated by a high common hepatic duct (CHD) injury requiring complex surgical repair. This case highlights the importance of the early recognition of this type of injury and the utility of a Kasai portoenterostomy as an operative repair in high CHD injuries in the absence of an extrahepatic duct viable for repair.

Detection of Rare Gallbladder Microperforation by 18F-FDG PET/CT in a Patient with Maxillary Sinus Cancer.

Gallbladder perforation is one of the most serious complications of cholecystitis and is rarely seen in 2-11% of cases. Pericholecystic abscesses secondary to gallbladder perforation are rare. Rapid diagnosis is important because of high morbidity and mortalityrates. A subcapsular abscess secondary to gallbladder microperforation is presented on 18F-fluorodeoxyglucose positron emission tomography/computed tomography performed for restaging in a patient with maxillary sinus cancer.

The Progression of Cholelithiasis to Gallstone Ileus: Do Large Gallstones Warrant Surgery?

Gallstone ileus is a rare cause of small bowel obstruction, classically occurring in patients with recurrent cholecystitis. The incidence of biliary enteric fistula and gallstone ileus in patients with large, asymptomatic gallstones is not known. We report a case of gallstone ileus, which occurred in the setting of a large, asymptomatic gallstone. This case suggests that large gallstones may warrant cholecystectomy, even in asymptomatic patients.

Laparoscopic repair of acute surgical diseases in the 21st century.

Laparoscopic surgery has an increasing use in today's world of general surgery, especially in the treatment of diseases such as acute appendicitis, acute cholecystitis, diverticulitis, lysis of adhesions in the setting of small bowel obstruction, incarcerated or strangulated inguinal hernia, and perforated peptic ulcer disease. The aim of this paper is to discuss the diagnosis and management of each disease while placing emphasis on the role of laparoscopy in its treatment.

[Leiomyoma of the large bowel].

The authors present a case study of a 48 year old man who was admitted to the hospital because of gall stone disease and a palpable abdominal tumour. Diagnostic laparoscopy was performed: chronic cholecystitis and an apple size tumour were found. The tumour was connected with colon ascendens. Laparotomy was performed and both the tumour and gallbladder were removed. The histological examination showed leiomyoma of the large bowel.

Doppler demonstration of cystic vein dilatation secondary to portal vein thrombosis.

High-resolution duplex ultrasound was used to demonstrate vascular dilatation in the gallbladder wall in eight patients with portal vein thrombosis. Gallbladder thickening, combined with a higher incidence of gallstone disease in patients with chronic hepatic disease can be misinterpreted as acute cholecystitis. The Doppler is helpful in detecting the vascular cause of the gallbladder wall thickening thus helping to reduce the frequency of inappropriate cholecystectomy.

Volvulus of the gallbladder. Report of two cases and review of the literature.

Torsion or volvulus of the gallbladder is a rare cause of acute cholecystitis. Although it has been reported in all age groups, it is most commonly seen in the elderly. With our increasingly aged population, it behooves the clinician to keep this entity in mind when treating the older patient with abdominal pain. We present two cases of torsion of the gallbladder and review the clinical and pathological aspects of this disease.

Spontaneous external biliary fistula in a patient with heroin addiction.

Spontaneous external biliary fistula, once common, is now a rare clinical entity. A patient with this lesion is described and attention is drawn to the fact that this rare condition is still being seen in certain patients with neglected cholecystitis. Heroin addiction was thought to have modified his response to gallbladder disease and contributed to the perforation. Diagnosis was suspected on physical examination and confirmed by spontaneous discharge of stones and fistulogram.

A bifid gallbladder? A challenging laparoscopic cholecystectomy.

INTRODUCTION: The modern-day gold standard treatment of acute cholecystitis is laparoscopic surgery. It is, however, associated with a higher risk of bile duct injury (0.1 %-1.5 %) when compared to the open approach. CASE PRESENTATION: We report a case of a patient with an acute cholecystitis in which we performed a laparoscopic cholecystectomy. We faced a destabilizing anatomy with what looked like the gallbladder and an unidentified mass, interpreted as a possible common bile duct cyst. Careful dissection allowed us to determine that what looked like a common bile duct cyst was a dilatation…

Challenges, Preoperative Assessment, and Surgical Strategies for the Left-Sided Gallbladder.

Left-sided gallbladder positioning, or sinistroposition, is a rare anatomical variation that poses challenges during surgical intervention due to associated vascular and biliary anomalies. While existing literature suggests an incidence of approximately 0.04-1.1%, it remains an underreported phenomenon that falls well outside the realm of "expected" anatomical variation and are rarely identified on preoperative imaging. Here, we present a case of acute cholecystitis in a patient with unexpected left-sided gallbladder, highlighting the associated challenges and outlining both preoperative and i…
