Autologous adipose-derived stem cells for the treatment of complex cryptoglandular perianal fistula: a prospective case-control study.

BACKGROUND: Complex cryptoglandular perianal fistula (CPAF) is a kind of anal fistula that may cause anal incontinence after surgery. Minimally invasive surgery of anal fistula is constantly emerging. Over the past 20 years, there are several sphincter-sparing surgeries, one of which is autologous adipose-derived stem cell (ADSC) transplantation. However, to date, there is no study regarding the treatment of complex CPAF with ADSC in China. This is the first study in China on the treatment of complex CPAF with ADSC to evaluate its safety and efficacy. METHODS: Totally, 24 patients with comple

Laparoscopic colectomy: indications for conversion to laparotomy.

HYPOTHESIS: Although experience with laparoscopic colectomy continues to accumulate, criteria for patient selection for the procedure have yet to be developed. We propose that review of indications for conversion to laparotomy during laparoscopic colectomy should define some of the current technical limitations of the procedure. This information may facilitate development of selection criteria for laparoscopic colon and rectal surgery. DESIGN: Single-institution retrospective medical records review. SETTING: Tertiary referral center. PATIENTS: Two hundred patients who underwent laparoscopic

Perioperative Outcomes in Patients with and Without Chronic Preoperative Therapeutic Anticoagulation Undergoing Metabolic Surgery at an Academic Medical Center.

Background/Objectives: Patients on chronic anticoagulation undergoing metabolic surgery represent an increased risk of complications, including both bleeding and thrombotic events, such as deep vein thrombosis (DVT) and pulmonary embolism (PE). The optimal perioperative management of patients who are receiving chronic anticoagulation therapy (CAT) is complex. In the colorectal surgery literature, patients on CAT have a 10% rate of peri-procedural bleeding and a 3% rate of thromboembolism. The aim of this study was to evaluate and compare the safety and postoperative outcomes between patients w…

RASAL2 down-regulation in ovarian cancer promotes epithelial-mesenchymal transition and metastasis.

It's preferred by patients as well as surgeons to avoid permanent stoma in a patient of Carcinoma Rectum. But this should not be done at the cost of margin compromise. There is little literature on factors affecting margin status, therefore a comprehensive study to analyze all factors associated with margin status is required. This will not only allow surgeons to be cautious in a particular subgroup during surgery but also help in proper patient selection for restorative surgery. It's an observational study consisting of 77 patients of Carcinoma Rectum undergoing restorative radical surgery. D

Myotonic dystrophy type 1 as a cancer predisposition syndrome: A case report of quadruple primary malignancies with review of the literature.

BACKGROUND: Myotonic dystrophy type 1 (DM1) is the most common adult-onset muscular dystrophy and is increasingly recognized as a cancer predisposition syndrome, with 2- to 8.5-fold elevated risks of endometrial, ovarian, thyroid, and colorectal cancers. Evidence on the oncological management of DM1 patients-including perioperative care, chemotherapy tolerability, and cancer surveillance-remains scarce. CASE PRESENTATION: A 56-year-old woman with DM1 developed four primary malignancies over 16 years: papillary thyroid carcinoma at age 40, breast carcinoma at age 49, and synchronous endometria

Immunotherapy in Colorectal Cancer: Novel Perspectives and Next Steps.

Colorectal cancer (CRC) is the third most common malignant neoplasm worldwide and the second leading cause of cancer-related mortality. Currently, only about 15% of newly diagnosed CRC cases and 5% of metastatic cases are classified as being microsatellite instability-high (MSI-H) and can benefit significantly from immune checkpoint blockade, while the majority of patients are microsatellite stable (MSS) and typically show limited responses to this strategy. This review aims to describe recent advances and emerging perspectives regarding the use of novel checkpoint inhibitors for MSS CRC. We c

Nanotechnology-Enabled Combination Strategies to Enhance 5-Fluorouracil-Based Cancer Chemotherapy.

Cancer still ranks among the top causes of morbidity and mortality across the globe. Among the existing treatment modalities, including surgery, radiation therapy, biological therapy, and chemotherapy, 5-fluorouracil (5-FU) is one of the most widely employed drugs for the treatment of different solid cancers, such as colorectal, breast, pancreatic, and head and neck cancers. Although the drug has proven its efficacy in clinical settings, the therapeutic potential of 5-FU is greatly hampered by its rapid systemic clearance, severe dose-toxicity, lack of selectivity, and development of drug resi

Research Progress on the Optimal Timing for Colostomy Reversal.

This review comprehensively summarizes the latest research progress on the optimal timing for colostomy reversal, a critical surgical intervention aimed at restoring normal bowel function and improving the quality of life of patients with colostomies. Colostomy is commonly performed for various gastrointestinal conditions, including colorectal cancer, diverticulitis, and inflammatory bowel diseases, but its long-term presence imposes physical and psychological burdens on patients. The timing of reversal directly affects postoperative recovery, complication rates, and overall patient outcomes,

Joint Frailty Mixture Cure Model for Recurrent Event Data With Dependent Censoring: An MCEM Approach.

Advancements in modern medical technology have enabled cures for a fraction of patients while extending survival times for those who are not cured. For non-cured patients, disease recurrence is influenced by observed covariates and unobserved individual heterogeneity (random effects). In biomedical studies, dependent censoring is frequently encountered, for example, in cancer patients, where right censoring can be caused by death from unrelated diseases or due to an (unobservable) cure status. This study introduces a joint frailty model for recurrent event data with a cure fraction, effectivel

Jejunal Mixed Adenoneuroendocrine Carcinoma (MANEC) With a Novel PTPRK-RSPO3 Fusion Initially Presenting as a Large Ovarian Mass: Diagnostic Challenges and Poor Prognosis.

BACKGROUND/AIM: Jejunal mixed adenoneuroendocrine carcinomas (MANECs) are rare but aggressive tumors. We report such a case initially present as a large ovarian mass with a novel gene fusion. CASE REPORT: A 51-year-old female presented with multiple episodes of abdominal pain. Computed tomography (CT) scan showed a 16.1 cm right ovarian mass. Surgery was performed and intraoperative frozen sections of the ovarian mass showed adenocarcinoma. Further histopathologic evaluation of the ovarian tumor demonstrated intestinal type of adenocarcinoma with CDX2 and SATB2 immunoreactivities, suggestive

The Negative Impact of a Non-Therapeutic Laparotomy in Patients With Inoperable Colorectal Peritoneal Metastases.

BACKGROUND: The peritoneum is the third most prevalent location for metastases of colorectal cancer. In patients with resectable disease, cytoreductive surgery combined with hyperthermic intraperitoneal chemotherapy (CRS-HIPEC) is the preferred treatment in the Netherlands, achieving median overall survival (OS) of 36-42 months. However, during explorative laparotomy, CRS-HIPEC may appear unfeasible. Evidence on how such non-therapeutic laparotomies affect prognosis is limited. METHODS: This retrospective cohort study included all non-therapeutic laparotomies performed between 01 and 01-2010

A Case of Recurrent Refractory Stomal Variceal Bleeding Associated with Oxaliplatin-Induced Portal Hypertension.

INTRODUCTION: Stomal variceal bleeding is a rare but potentially life-threatening complication in patients with gastrointestinal stomas complicated by portal hypertension. Although oxaliplatin-based chemotherapy is widely used for colorectal cancer, it is known to cause sinusoidal obstruction syndrome (SOS), which may result in non-cirrhotic portal hypertension. However, stomal variceal bleeding secondary to oxaliplatin-induced portal hypertension has rarely been reported. CASE PRESENTATION: A 67-year-old man underwent curative surgery for rectal cancer, followed by 12 courses of oxaliplatin-

Multiple consecutive sphincter-preserving procedures improve overall healing outcomes in transsphincteric fistulae: a prospective cohort study of 101 patients.

BACKGROUND: The management of complex transsphincteric fistulae is challenging and surgical repair must be balanced against the risk of continence impairment. This study aims to demonstrate that multiple, carefully selected procedures can achieve long-term clinical healing without sphincter division for the majority of patients in this cohort. METHODS: This was a prospective cohort study in a large university hospital with a dedicated colorectal surgery unit. Patients underwent between one and four sphincter-preserving procedures, including advancement flap (AF), ligation of the intersphincte…

Acromegaly and clinical manifestations.

Acromegaly is a chronic multisystem disorder driven by persistent hypersecretion of growth hormone (GH) and consequent elevation of insulin-like growth factor 1 (IGF-1). Beyond its hallmark acral and craniofacial alterations, it manifests with a broad spectrum of systemic complications that substantially contribute to morbidity and mortality. Skeletal and soft tissue overgrowth cause characteristic disfigurement, while musculoskeletal involvement-including degenerative arthropathy, vertebral fractures, myopathy, and neuropathies-remains a major determinant of long-term disability despite bioch

When Size Matters: Surgical Challenges of Giant Rectal Foreign Bodies.

Rectal foreign bodies are an uncommon but increasingly recognized presentation in emergency colorectal surgery. Object size - particularly transverse diameter - and rural, low-resource settings add complexity to both diagnosis and treatment. A 57-year-old man from a rural area of southern Colombia was referred to a regional hospital for a retained lithic rectal foreign body (a stone). Plain abdominal radiography showed a cone-shaped radiopaque object. After multiple failed transanal extraction attempts, exploratory laparotomy was performed. A conical stone approximately 15 cm in diameter at it…

Neoadjuvant chemoradiation with or without PD-1 blockade in locally advanced rectal cancer: a randomized phase 2 trial.

Radiotherapy displays unique antitumor synergism with immune checkpoint inhibitors, which is indicated by high pathological complete response (pCR) rates from single-arm trials of locally advanced rectal cancer (LARC). Here we test the efficacy and safety of the radiation-immune checkpoint inhibitor combination in patients with LARC in a phase 2, randomized trial conducted in eight major colorectal cancer centers in Beijing. In total, 186 eligible all-comer (proficient mismatch repair and deficient mismatch repair) participants were enrolled. The patients were randomly assigned to receive neoa
