[Ocular coloboma associated with cervico-oculo-acoustic syndrome].

INTRODUCTION: Cervico-oculo-acoustic syndrome is an uncommon syndrome consisting of Klippel Feil syndrome, deafness, and Duane syndrome. We relate a new case of cervico-oculo-acoustic syndrome. CASE REPORT: We report the case of 15-month-old girl referred for esotropia of the right eye. Ocular examination revealed type 1 Stilling Duane syndrome in the right eye, in which we also noted microcornea, iris coloboma, and papillary and inferior chorioretinal coloboma sparing the macular area. The left eye was normal. The physical examination revealed Klippel Feil syndrome characterized by an extrem…

Cochlear implantation in a child with osteogenesis imperfecta.

Osteogenesis imperfecta (OI) is a hereditary disease of connective tissue and affects bone, dentine, sclera, joint, tendon, blood vessels, heart valves, and skin. Approximately 50% of the adult patients with OI have associated hearing impairment. To date, only three cases of cochlear implantation in adults with OI have been reported, but none in children. We present a case of cochlear implantation in a congenitally deaf 6-year-old boy with OI. The Nucleus 24 Contour device was successfully implanted using the suprameatal approach (SMA). At 6 months post-initial stimulation there was no evidenc

Common symptoms--different diseases: coexistence of neurosyphilis and non-Hodgkin's lymphoma.

We describe the case of a 32-year-old man with generalized lymphadenopathy who was diagnosed with a low-grade follicular small-cleaved cell lymphoma. The patient developed hearing loss, tinnitus and cerebrospinal fluid (CSF) pleocytosis attributed to central nervous system (CNS) infiltration by his malignancy, while receiving chemotherapy with vincristine, cyclophosphamide and prednisone. Despite intrathecal chemotherapy with methotrexate, the CSF pleocytosis persisted. Neurosyphilis was suspected because of prior history of gonorrhea and was confirmed with serologic studies of blood and CSF a

Temporal bone pathology of a patient with cochlear implant.

A 66-year-old man with profound total deafness received a 22-channel cochlear implant. After surgery, his hearing improved and the postoperative course was uneventful except transient vertigo. The patient died of lung cancer after 2.5 years. Twelve hours after death, the temporal bone was harvested for histological and electron microscopic examination. The mastoid air cells were preserved, although a few mucosal folds were present. The Dacron mesh tie had largely been absorbed, but part remained embedded in fibrous tissue. No inflammation was noted in the tympanic cavity or the round window th…

[A case report of Epstein syndrome].

Epstein's triad which is a syndrome with the combination of macrothrombocytopenia, deafness and nephritis, is similar to Alport's syndrome. We report on a case of Epstein syndrome and describe the results of morphological examination of a renal biopsy, specimen. The patient was a 14-year-old girl with the diagnosis of chronic idiopathic thrombocytopenic purpura that had preseated from the age of 3 years. She was referred to Daisan Hospital of the Jikei University School of Medicine on April 1, 1991 for refractory thrombocytopenia. She had shown sensorineural hearing loss since the age of 6 yea…

Recurrent meningitis due to a congenital fistula of the stapedial footplate.

A rare case of a congenital fistula of the stapes footplate, in a 10-year-old girl, resulting in recurrent meningitis, is reported. A full ENT examination and a high index of suspicion, is essential if the diagnosis is to be made. Exploratory tympanotomy should be considered on clinical grounds, even if a high resolution CT scan of the temporal bones does not show any anomalies, in order to ascertain the definite diagnosis of a fistula and to seal it permanently.

Electrocochleographic study of summating potentials in Menière's disease.

(1) The polarity of summating potentials (SP) and the SP/AP ratio in Menière's disease depend on the electrode position in the tympanic cavity, on the interval between sound stimuli and the method of recording, whether it is by transtympanic or extratympanic recording. (2) In a series of 42 patients with unilateral Menière's disease abnormal SPs were not very closely related to the time elapsed since the onset of vertigo, audiometric pattern, or the results of the glycerol test. (3) It was not possible to differentiate Menière's disease from sudden deafness on the basis of large SP/AP ratios a…

Chronic cardiovascular pathology and hearing loss in the aged.

With the objective of studying a possible correlation between chronic cardiovascular disturbances and hearing loss in aged people, two groups were compared. The first was composed of apparently healthy subjects and the second, of subjects suffering from clinically evident chronic cardiovascular pathology. All the subjects were submitted to a pure tone audiometry, discrimination test, SISI test and TDT. The results showed a significant difference in pure tone threshold audiometry between the apparently healthy subjects and those presenting cardiovascular symptoms; the discrimination and the TDT
