[Value of small bowel double contrast enema in clinical interventions].

The double contrast enema is the most effective morphological screening method for the evaluation of the whole small bowel. Its sensitivity is 85%, its specifity 96.7%. In specific clinical problems the number of pathological roentgen findings rises: from 34.4% when all indications are taken into consideration to 58% in indications specific to the small intestine such as Morbus Crohn or the malabsorption syndrome. Search for tumours and the double contrast of the small bowel in unclear gastro-intestinal bleeding are unproductive. The weak point of this screening method is the lower part of the

Perforated Jejunal Pseudodiverticulum Managed With Limited Segmental Resection: A Case Report.

Jejunal diverticulitis (JD) is a rare disease with significant clinical relevance, as it often causes intense pain in the epigastrium that radiates to the left flank, accompanied by hemodynamic instability and leukocytosis. Diagnosis must be complemented with clinical and imaging studies.  A 72-year-old woman presented with abdominal pain of colic type in the epigastrium and left flank for 12 hours. The computed tomography scan showed two diverticula in the jejunum, the first with localized pneumoperitoneum and signs of perforation, the second with no signs of an inflammatory process. An explo

American Society for Gastrointestinal Endoscopy guideline on the role of endoscopy in acute lower gastrointestinal bleeding.

This clinical practice guideline from the American Society for Gastrointestinal Endoscopy provides evidence-based recommendations regarding the role of endoscopy in the diagnosis and management of acute lower GI bleeding (LGIB). This document was developed using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) framework. In the broad context of acute LGIB, this guideline addresses the use of colonoscopy versus CT +/- angiography as the first modality, urgent versus nonurgent colonoscopy, and prepped versus unprepped colonoscopy. For diverticular bleeding in partic

Ovarian teratoma leading to colonic intussusception: a case report.

Intussusception in adults is relatively rare compared to that in children and is typically caused by underlying benign or malignant intestinal diseases. We report a case of a 40-year-old female who presented with colonic intussusception and obstruction caused by an ovarian teratoma invading a segment of the colon. Contrast-enhanced CT imaging revealed a soft tissue mass within the colon, and colonoscopy identified a large cystic lesion approximately 6.0 × 6.0 cm in size located 20 cm from the anal verge. This lesion obstructed the lumen, causing significant narrowing. Preoperatively, the condi

Perforated Jejunal Diverticulitis Managed Laparoscopically: A Case Report.

Jejunal diverticulosis is an uncommon condition that is frequently asymptomatic but may present with serious complications such as diverticulitis and, more rarely, perforation. Its clinical presentation is typically nonspecific and often mimics more common abdominal emergencies, which can delay diagnosis and treatment. We report the case of a 63-year-old woman who presented with diffuse abdominal pain, vomiting, and absence of flatus. Computed tomography revealed jejunal wall thickening, multiple diverticula, mesenteric fat stranding, and localized pneumoperitoneum. Diagnostic laparoscopy iden

Female prostatic occlusive syndrome: A case report of MRI analysis and literature review.

Female prostatic occlusive syndrome (FPOS) is a rare condition caused by obstruction of the female urethra due to hypertrophic or cystic Skene's glands-the embryologic homologues of the male prostate. Because its clinical manifestations are nonspecific, FPOS is frequently misdiagnosed as urethral diverticulum, cystitis, or other lower urinary tract disorders, often leading to delayed or inappropriate treatment. This study aimed to characterize the MRI features of FPOS and to discuss the diagnostic challenges associated with this entity. We report the case of an 87-year-old woman who presented

Perforated Duodenal Diverticulitis: A Case Report of a Rare Surgical Entity Treated by Roux-en-Y Deriving Intestinal Patch.

Perforated diverticulitis is a rare but serious complication associated with a significant mortality rate. Although many cases of conservative treatment have been reported, surgery remains the mainstay for perforated duodenal diverticulitis. We report a rare case of a 55-year-old female who presented with epigastric pain without fever. Computed tomography revealed a 3 cm perforated duodenal diverticulum of the D2 part of the duodenum with a localized abscess. After the failure of conservative treatment, we performed a deriving intestinal patch completed by cholecystectomy and biliary decompres

Rings and Slings: Not Such Simple Things.

PURPOSE OF REVIEW: Vascular rings are congenital malformations resulting from abnormal development of the great vessels, with the consequent encircling and compression of the trachea, esophagus, or both. We conducted a review of the current literature to identify the different management strategies that can be implemented based on the prognosis of each of these anomalies. RECENT FINDINGS: Although most vascular rings occur in isolation, they can also be associated with other congenital cardiac and/or respiratory diseases; therefore, thorough investigation is necessary before definitive surgic

Technical Considerations in Primary Repair of a Congenital Prostatic Rectourethral Fistula in an Adult-Sized Patient.

Congenital anorectal malformations are generally diagnosed and repaired as a neonate or infant, but repair is sometimes delayed. Considerations for operative repair change as the patient approaches full stature. We recently encountered a 17-year-old male with an unrepaired congenital rectourethral fistula and detail our experience with his repair. We elected to utilize a combined abdominal and perineal approach, with robotic assistance for division of his rectourethral fistula and pullthrough anoplasty. Cystoscopy was used simultaneously to assure full dissection of the fistula and to minimize

Use of Sugammadex in an octagenerian with Myaesthenia Gravis undergoing emergency laporotomy.

BACKGROUND: Myaesthenia Gravis is an autoimmune disorder that results in increased sensitivity to neuromuscular blockers. Anaesthesia and surgery in patients with Myaesthenia Gravis is associated with an increased incidence of complications, including post operative ventilation. We report the successful use of intra-operative train of four ratio monitoring and Sugammadex to reverse rocuronium induced neuromuscular blockade in an elderly octagenarian with Myaesthenia Gravis undergoing an emergency laporotomy. CASE REPORT: An 87year old man with Myaesthenia Gravis underwent an emergency laporot

Modification of the Koyanagi Technique for the Single-Stage Repair of Proximal Hypospadias.

We describe a modification of the Koyanagi technique for hypospadias in an attempt to reduce complications and improve results. Between January 2005 and July 2012, 21 patients underwent treatment of proximal hypospadias using a modified hypospadias repair. The procedure involved making a U-incision similar to that in a Thiersch-Duplay repair to preserve the blood supply to the tubular neourethra. The neourethra was reconstructed by island penile skin. An M-incision was made in the glans, and the meatus was attached to tip of glans. Flaps were harvested from both sides of the penis preventing p

Etiology of non-traumatic acute abdomen in pediatric emergency departments.

Acute abdominal pain is a common complaint in pediatric emergency departments. A complete evaluation is the key factor approaching the disease and should include the patient's age, any trauma history, the onset and chronicity of the pain, the related symptoms and a detailed physical examination. The aim of this review article is to provide some information for physicians in pediatric emergency departments, with the age factors and several causes of non-traumatic acute abdominal pain. The leading causes of acute abdominal pain are divided into four age groups: infants younger than 2 years old,

Intussusception of the bowel in adults: a review.

Intussusception of the bowel is defined as the telescoping of a proximal segment of the gastrointestinal tract within the lumen of the adjacent segment. This condition is frequent in children and presents with the classic triad of cramping abdominal pain, bloody diarrhea and a palpable tender mass. However, bowel intussusception in adults is considered a rare condition, accounting for 5% of all cases of intussusceptions and almost 1%-5% of bowel obstruction. Eight to twenty percent of cases are idiopathic, without a lead point lesion. Secondary intussusception is caused by organic lesions, suc

Obscure gastrointestinal bleeding and small bowel pathology: comparison between wireless capsule endoscopy and multidetector-row CT enteroclysis.

Obscure gastrointestinal bleeding is defined as bleeding of unknown origin, that persists or recurs after negative conventional barium contrast studies and upper and lower tract endoscopy. The causes of such a bleeding frequently arise in the small bowel, and they are represented by mucosal vascular abnormalities, neoplasms and other conditions such as Crohn's disease, Meckel's diverticulum, and vasculitis. Conventional barium contrast studies and push enteroscopy allow only a limited small bowel examination; moreover, intraoperative endoscopy may be inconclusive, since the small bowel is diff

Jejunal diverticular disease with unborn enterolith presenting as a small bowel obstruction: a case report.

Jejunal diverticuli are rare and usually asymptomatic. More commonly, they are seen as incidental findings on CT images, enteroclysis, or during surgery. Complications such as bleeding, perforation, obstruction, malabsorption, diverticulitis, blind loop syndrome, volvulus, and intussusception may warrant surgical intervention. An interesting case of an unborn enterolith (enclosed calculus) from a jejunal diverticulum presenting as a small bowel obstruction is presented. The patient is a 66-year- old woman with no prior history of abdominal surgery who presented with a high-grade bowel obstruct

Intestinal malrotations: a review and report of thirty cases.

Intestinal malrotation is a developmental anomaly affecting the position and peritoneal attachments of the small and large bowels during organogenesis in foetal life. It has been defined as absent or incomplete rotation and fixation of the embryonic gut around the superior mesenteric artery. In the present paper, we review the definition, history, embryology/aetiology, epidemiology, symptoms and signs, diagnosis and treatment of intestinal malformations. Moreover, we report the records of 30 cases of malrotation admitted to our department over a period of five years. The final intraoperative d

[Video-capsule endoscopy in children with unexplained symptoms of the small intestine].

Video-capsule endoscopy was used in 4 children with unexplained symptoms of the small intestine. Each patient swallowed a capsule of 11 by 27 mm, which contained a camera that takes 2 images per second (in children aged less than 8 years, the capsule was placed in the duodenum under sedation). In a 3-year-old girl with rectal bleeding following partial resection of the colon and small intestine, ulcers were seen proximal to the ileorectal anastomosis. In a 14-year-old boy with Crohn's disease and an abnormal growth curve, multiple stenoses of the small intestine were seen. In an 8-year-old boy
