A controlled trial of cimetidine for the treatment of nonulcer dyspepsia.

Sixty patients with nonulcer dyspepsia took cimetidine 1 g daily or a placebo for 5 weeks in a double-blind trial. Thirty-four men and 26 women with normal double-contrast barium meals were included. Five percent had the endoscopic appearances of duodenitis. Symptoms improved during treatment in 62% of those taking cimetidine and in 54% of the placebo group (NS, p = 0.5). We suggest that significant duodenitis is uncommon in the absence of active or quiescent duodenal ulcer disease. From this study there is no evidence that cimetidine benefits those patients with normal double-contrast barium …

Minimal Change in Minimally Invasive Surgery: National Trends in Laparoscopic Duodenal Atresia Repair, 2016-2024.

BACKGROUND: Laparoscopic duodenal atresia repair in neonates, introduced over two decades ago, is associated with select short-term benefits versus open repair, but recent U.S. population-level data are lacking. We evaluated national trends and compared operative time and outcomes. METHODS: A retrospective cohort study using the American College of Surgeons National Surgical Quality Improvement Program Pediatric database from 2016 to 2024 of neonates (≤28 days) undergoing duodenal atresia repair was conducted to assess the annual proportion of laparoscopic versus open repairs (primary outcome

Development and validation of an artificial intelligence classifier for histologic grading of superficial non-ampullary duodenal epithelial tumors.

BACKGROUND AND STUDY AIMS: Superficial non-ampullary duodenal epithelial tumors (SNADETs) are being increasingly diagnosed, and accurate endoscopic diagnosis is essential for determining appropriate treatment strategies. However, no standardized diagnostic method currently exists. We developed a deep learning-based artificial intelligence (AI) classifier to distinguish between low-grade and high-grade neoplasia and evaluated its diagnostic performance. PATIENTS AND METHODS: The training set of 85 SNADET cases (low-grade neoplasia, n = 41; high-grade neoplasia, n = 44; white-light images, n =

Intraoperative Duodenal Wall Thickening: "A Decision-Guiding Intraoperative Marker" in Clinically Benign Gastric Outlet Obstruction?

BackgroundDistinguishing benign from malignant gastric outlet obstruction (GOO) remains a major clinical challenge. Despite comprehensive preoperative evaluation, occult malignancy and advanced fibrosis may be overlooked, complicating treatment selection, particularly in the era of minimally invasive and endoscopic bypass techniques. This study aimed to evaluate the clinical and pathological characteristics of clinically benign GOO, focusing on the prognostic significance of intraoperative duodenal wall thickening.MethodsThis observational study included adult patients who underwent surgical t

Clinical outcomes of endoscopic submucosal dissection for previously treated superficial non-ampullary duodenal epithelial tumors.

BACKGROUND AND AIMS: Endoscopic submucosal dissection (ESD) is increasingly used for superficial non-ampullary duodenal epithelial tumors (SNADETs). However, evidence from the United States (US) is limited. Moreover, the effectiveness and safety of ESD for previously treated SNADETs remain unclear. This study aimed to evaluate the outcomes of ESD for previously treated SNADETs. METHODS: This single-center retrospective cohort study included patients undergoing ESD for SNADETs at a US tertiary center between October 2018 and February 2025. Short-term outcomes (en bloc/R0 resection and adverse

Mucin phenotype stratifies histological malignancy in superficial non-ampullary duodenal epithelial tumors: a prospective study.

BACKGROUND: Superficial non-ampullary duodenal epithelial tumors (SNADETs) are being increasingly detected during routine endoscopy. However, biological malignancy and optimal risk stratification remain poorly defined. We aimed to clarify the association between mucin phenotypes and histological malignancies in SNADETs using a prospective study design. METHODS: In this prospective observational study, patients with suspected SNADETs underwent a standardized magnified endoscopic examination with image-enhanced endoscopy (IEE-ME), followed by endoscopic resection. All specimens were centrally r

Multidisciplinary approach for treatment of duodenal perforation complicated with gastrointestinal hemorrhage in pediatric patient and relevant literature review.

OBJECTIVE: To investigate the key clinical diagnostic and therapeutic points, challenges and corresponding strategies for duodenal perforation complicated with recurrent gastrointestinal bleeding in children, and to provide a clinical reference for managing this rare pediatric acute critical condition. METHODS: The clinical data of a 2-year-and-7-month-old female child with duodenal perforation and recurrent gastrointestinal bleeding were retrospectively analyzed. A systematic review and analysis were conducted from the perspectives of etiology, diagnosis, treatment and postoperative manageme
