Oral contraceptives and venous thromboembolism. A case-control study.

To assess the influence of oral contraceptives (OC) on the risk for venous thromboembolism (VTE) in young women, a 5-year case-control study including all women 15-44 years old suffering a first deep venous thrombosis or a first pulmonary embolism from all Danish hospitals, along with 1200 control subjects during the period 1994-1995, was conducted. Of 586 patient and 1200 control subject questionnaires sent out, 523 patient (89.2%) and 1074 control (89.5%) questionnaires were returned with an agreement to participate. After exclusion of women with nonvalid diagnoses, women who were pregnant,

Mitral valve strands and the risk of ischemic stroke in elderly patients. The French Study of Aortic Plaques in Stroke (FAPS) Investigators.

BACKGROUND AND PURPOSE: Strands are thin and filamentous attachments on the cardiac valves shown by transesophageal echocardiography. Their nature and their potential for embolization are largely unknown. The objective was to estimate the risk of brain infarction in patients with mitral valve strands. METHODS: Using transesophageal echocardiography, we compared the frequency of strands on native mitral valves in 284 consecutive patients admitted with brain infarction and 276 control patients, all older than 60 years. In a second part, case subjects were followed up over a 2- to 4-year period,

Reduced mortality and morbidity for ascending aortic aneurysm resection regardless of cause.

BACKGROUND: This study was done to answer the question, "What is the current risk of resection of ascending aortic aneurysms regardless of acuity or cause?" METHODS: One hundred fifteen consecutive patients who underwent ascending aortic aneurysm repair from January 1, 1990, to July 1, 1995, were retrospectively reviewed, excluding those with acute ascending aortic dissection. The mean age was 59 years; 55% were male. Concomitant procedures included coronary artery bypass in 23 (20%) and arch repair in 12 (10%). In group 1, 54 patients had replacement of the aortic valve, root, and ascending

Pontine infarction extending to the basal surface.

BACKGROUND AND PURPOSE: Etiology and symptomatology in pontine infarction extending to the basal surface are supposed to be different from those in deep pontine infarction of the lacunar type. The aim of this study was to compare the infarct size and location, vascular lesions, risk factors, and neurological deficits in three different types of acute pontine infarction. METHODS: We studied isolated pontine infarction extending to the basal surface on brain imaging (group 1, n = 30), deep pontine infarction without extension to the basal surface (group 2, n = 23), and pontine infarction with s

Deliberate basilar or vertebral artery occlusion in the treatment of intracranial aneurysms. Immediate results and long-term outcome in 201 patients.

Deliberate occlusion of the basilar or vertebral arteries was performed in 201 patients with intracranial aneurysms, where the aneurysmal neck could not be clipped directly. The aneurysms arose from the basilar apex in 83 cases, the basilar trunk in 46, the vertebrobasilar junction in 35, and the vertebral artery in 37; 87% of the aneurysms were classified as giant lesions (> 2.5 cm). There were 85 upper basilar occlusions, 41 lower basilar occlusions, 29 bilateral vertebral occlusions, and 48 unilateral vertebral artery occlusions. The clinical follow-up period varied from 1 to 23 years, with

Association of preoperative antifibrinolytics with reduced blood transfusions after vascular surgery procedures.

OBJECTIVE: Major vascular surgery interventions are critical for treating peripheral artery disease but carry a high risk of intraoperative blood loss with resultant coagulopathy. Blood product transfusions are limited, costly, and associated with an increased risk of myocardial infarction and death. Planned preoperative administration of antifibrinolytic agents (AFAs), aimed at inhibiting intraoperative plasmin-mediated lysis, has become a standard practice for orthopedic, trauma, obstetrics, and cardiac surgery, effectively reducing the perioperative bleeding and transfusion requirements. Irregular Cycles, Ovulatory Disorders, and Cardiometabolic Conditions in a US-Based Digital Cohort.

IMPORTANCE: Polycystic ovary syndrome (PCOS), characterized by irregular menstrual cycles and hyperandrogenism, is a common ovulatory disorder. Having an irregular cycle is a potential marker for cardiometabolic conditions, but data are limited on whether the associations differ by PCOS status or potential interventions. OBJECTIVE: To evaluate the association of PCOS, time to regularity since menarche (adolescence), and irregular cycles (adulthood) with cardiometabolic conditions. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study used a large, US-based digital cohort of users of

Thromboembolic risk with IVIg: Incidence and risk factors in patients with inflammatory neuropathy.

Our objective was to evaluate whether IV immunoglobulin (IVIg) increases the risk of thromboembolic events in neurology outpatients with inflammatory neuropathies, as there is conflicting evidence supporting this hypothesis, mainly from non-neurologic cohorts. We investigated this question over 30 months in our cohort of patients with inflammatory neuropathies receiving regular IVIg and found a greater incidence of arterial and venous thromboembolic events than population-based rates determined by hospital admissions data. Vascular risk factors were more common in the event group but there wer

Neurofibromatosis type 1: a case highlighting pulmonary and other rare clinical manifestations.

Neurofibromatosis type 1 (NF1)-related lung disease is a rare but increasingly recognised, high morbidity associated feature of the condition. We present a 48-year-old male patient with NF1, who was initially admitted for a subarachnoid haemorrhage requiring aneurysmal coil embolisation. During his recovery, he developed a left-sided pneumothorax requiring chest tube placement followed by concerns for re-expansion pulmonary oedema requiring intubation. Subsequently, the patient also developed a right-sided pneumothorax requiring additional chest tube placement but did not develop right-sided p

Age dependency of ischaemic stroke subtypes and vascular risk factors in western Norway: the Bergen Norwegian Stroke Cooperation Study.

OBJECTIVES: Age dependency of acute ischaemic stroke aetiology and vascular risk factors have not been adequately evaluated in stroke patients in Norway. Aims of this study were to evaluate how stroke subtypes and vascular risk factors vary with age in a western Norway stroke population. MATERIALS AND METHODS: Patients aged 15-100 years consecutively admitted to our neurovascular centre with acute ischaemic stroke between 2006 and 2012 were included. The study population was categorized as young (15-49 years), middle-aged (50-74 years) or elderly (≥ 75 years). Stroke aetiology was defined by

Usefulness of measurement of fibrinogen, D-dimer, D-dimer/fibrinogen ratio, C reactive protein and erythrocyte sedimentation rate to assess the pathophysiology and mechanism of ischaemic stroke.

BACKGROUND: Classification defined in the Trial of Org10172 in Acute Ischaemic Stroke (TOAST) is widely used in trials and practice. Previous studies on pathophysiology suggest a role for endothelial inflammation in atherothrombotic strokes and intracardiac thrombosis in cardioembolic strokes. Data on lacunar and undetermined strokes are limited. The aim of the study was to assess non-specific inflammatory and thrombogenic parameters in patients with ischaemic stroke. METHODS: This was a prospective controlled clinical study involving 200 patients with ischaemic stroke and 50 controls. Patien…

Effects of statins on early and late results of carotid stenting.

OBJECTIVES: Increasing data suggest that statins can significantly decrease cardiovascular and cerebrovascular events due to a plaque stabilization effect. However, the benefit of statins in patients undergoing carotid angioplasty and stenting (CAS) for carotid stenosis is not well defined. The aim of this study was to investigate whether statins use was associated with decreased perioperative and late risks of stroke, mortality, and restenosis in patients undergoing CAS. METHODS: All patients undergoing CAS for primary carotid stenosis from 2004 to 2009 were reviewed. The independent associa

Incidence and risk factors of postoperative visual loss in cardiac surgery in the United States from the national inpatient sample 2013 to 2022.

BACKGROUND: Postoperative Visual Loss (POVL), a rare but severe complication, occurs more often in cardiac surgery. It leads to significant disability, emotional distress, and healthcare costs. In the U.S., where cardiac surgery is common, POVL is challenging to treat, thus identifying characteristics associated with POVL is key to identifying at-risk patients. Uncovering risk factors and prevention strategies for this serious event is crucial. This study aims to assess incidence trends and correlate patient and perioperative factors with POVL. METHODS: A retrospective study was conducted usin

Atrial Fibrillation Versus Nonatrial Fibrillation Coronary Embolism: A Multicenter Study.

BACKGROUND: Coronary embolism (CE) is an underrecognized cause of acute myocardial infarction, and limited data exist comparing atrial fibrillation-related CE (AF CE) with CE from other causes. METHODS: We conducted the first multicenter study in this challenging setting (NCT06948240). We retrospectively included 489 patients diagnosed with CE across 10 hospitals. CE was diagnosed using Shibata criteria. Patients were classified into AF CE and non-AF CE groups. RESULTS: A total of 241 patients (49%) had AF CE. Distinctive angiographic features were reported in patients with AF CE: less proxi

Perioperative antithrombotic management in degenerative spine surgery: a multicenter cohort of 767 consecutive patients.

BACKGROUND: Perioperative management of antithrombotic therapy in degenerative spine surgery remains a challenging clinical issue, particularly in elderly patients with cardiovascular comorbidities requiring chronic anticoagulant or antiplatelet treatment. Although several guidelines address perioperative anticoagulation in high-bleeding-risk procedures, procedure-specific evidence in elective spinal surgery remains limited, and clinical practice is still heterogeneous. METHODS: A multicenter retrospective cohort study was conducted including 767 consecutive patients aged 60 years or older wh

Balloon guide catheters improve first-pass effect and neurologic outcomes in mechanical thrombectomy: a matched cohort analysis.

BACKGROUND AND PURPOSE: The use of balloon guide catheters (BGCs) versus guide sheaths (GSs) for mechanical thrombectomy (MT) in large-vessel occlusive acute ischemic stroke (LVO-AIS) remains controversial. BGCs provide proximal flow arrest which may improve clot ingestion and reduce distal embolization. MATERIALS AND METHODS: A single-center retrospective matched-pair analysis to evaluate procedural efficacy and clinical outcomes as a function of BGC versus GS use for MT was performed. Consecutive patients who underwent MT for anterior circulation LVO-AIS from 2017 to 2025 were included. Sub
