BACKGROUND: Therapeutic hypothermia has been demonstrated to improve neurological outcome in comatose survivors of cardiac arrest. Current temperature control modalities however, have several limitations. Exploring innovative methods of temperature management has become a necessity.
METHODS: We describe the first use of a novel esophageal cooling device as a sole modality for hypothermia induction, maintenance and rewarming in a series of four postcardiac arrest patients. The device was inserted in a manner similar to standard orogastric tubes and connected to an external heat exchange unit.

In hallux valgus syndrome conservative measures can only be applied to relieve the symptoms. Insoles, orthoses, and toe alignment splints cannot improve forefoot deformity. However, symptoms due to increased local pressure can be alleviated with orthotic devices. After surgically realigning the hallux, splints are helpful to retain joint congruency and assure proper toe position during soft tissue healing.

This paper presents an architecture for sensing nerve signals and delivering functional electrical stimulation to peripheral and visceral nerves. The design is based on the very large scale integration (VLSI) technology and amenable to interface to microelectrodes and building a fully implantable system. The proposed stimulator was tested on the vagus nerve and is under further evaluation and testing of various visceral nerves and their functional effects on the innervated organs.
