Complement C9 deficiency as a novel risk factor for invasive Candida esophagitis in children: a single case in-depth analysis.

BACKGROUND: The complement system constitutes a critical component of innate immunity against fungal pathogens. Complement component 9 (C9), an essential constituent of the membrane attack complex (MAC), plays a pivotal role in pathogen elimination. However, isolated C9 deficiency has not been previously reported in association with fungal esophagitis. CASE PRESENTATION: We present the case of a nearly 7-year-old girl who suffered from progressive dysphagia, nausea, and vomiting for more than two months. Esophagogastroduodenoscopy revealed esophageal stricture, and histopathological examinati

Novel compound heterozygous MVK variants cause early-onset mevalonic aciduria in a Chinese infant.

Mevalonate kinase deficiency (MKD) is a rare autosomal recessive autoinflammatory disorder caused by mevalonate kinase (MVK) gene mutations, with phenotypes ranging from mild hyper-IgD syndrome (HIDS) to severe mevalonic aciduria (MA). Here, we report a 7-month-old Chinese male infant presenting with neonatal-onset MA. The proband presented with growth retardation, intractable diarrhea, recurrent fever, generalized rash, progressive hepatosplenomegaly, and persistent systemic inflammation. Laboratory tests revealed marked leukocytosis, elevated C-reactive protein, hyper-IgD and increased urina

The First Case of Lipoprotein Glomerulopathy in a Patient With Male Infertility.

Lipoprotein glomerulopathy (LPG) is a rare hereditary glomerular disease with lipoprotein thrombi deposition in glomerular capillaries, which is caused by pathogenic variants in APOE gene. Limited research has theoretically suggested that APOE gene mutations might affect sperm quality and contribute to male infertility. However, there have been no reports documenting the occurrence of LPG and male infertility in the same individual worldwide. Herein, we report a 36-year-old slim man who presented with nephrotic syndrome and had abnormal sperm quality. Kidney biopsy confirmed the diagnosis of L

Sequential sequencing reveals the architecture and complexity of genomic variants in patients with Alport syndrome.

Alport syndrome (AS) is a prevalent inherited kidney disorder mainly caused by mutations in COL4A3, COL4A4, and COL4A5 genes. To elucidate the genetic variants of AS, we implemented a sequential sequencing strategy within a Chinese cohort of 555 patients, comprising whole-exome sequencing (WES) for all participants, followed by whole-genome sequencing (WGS), RNA sequencing (RNA-seq), and nanopore long-read sequencing (NLR-seq) for selected individuals. We identify 431 distinct variants in 509 (91.7%) patients, with 42.2% being novel. Beyond WES, additional sequencing approaches resolve 23 pati

Clinical and Neurodevelopmental Course in a Case of EFNB1-Related Craniofrontonasal Syndrome With Unrepaired Craniosynostosis.

INTRODUCTION: Craniofrontonasal syndrome displays an unusual X-linked dominant inheritance pattern due to pathogenic variants in EFNB1, which encodes a membrane-bound ligand for the ephrin receptor. Females exhibit a more severe phenotype than males and can have craniosynostosis, hypertelorism, craniofacial asymmetry, thoracic skeleton abnormalities, and an assortment of other features. Males typically only develop a milder hypertelorism, or they have no discernable features at all. METHODS: Exome sequencing was used to identify a likely pathogenic EFNB1 c.129-2A>G splice site variant in an i

Cohen syndrome with novel VPS13B variants presenting as early-onset diabetes: a case report.

Cohen syndrome is a rare autosomal recessive disorder caused by biallelic pathogenic variants in the VPS13B gene, classically presenting with developmental delay, distinctive craniofacial features, neutropenia, truncal obesity, and progressive retinal dystrophy. Metabolic abnormalities, including insulin resistance and diabetes mellitus, have been increasingly recognized, but early-onset diabetes with diabetic ketoacidosis remains uncommon. We report a 28-year-old Chinese woman with early-onset insulin-resistant diabetes complicated by recurrent diabetic ketoacidosis and nephropathy. She prese

First reported case of adult-onset very long-chain acyl-coa dehydrogenase (vlcad) deficiency in Vietnam: a rare metabolic myopathy.

Very long-chain acyl-CoA dehydrogenase (VLCAD) deficiency is a rare genetic metabolic disorder involving impaired fatty acid β-oxidation. It is caused by mutations in the Acyl-CoA Dehydrogenase Very Long Chain (ACADVL) gene, which encodes the VLCAD enzyme. The clinical presentation is diverse, ranging from a severe neonatal-onset form to a milder adult-onset form. We describe the first reported case in Vietnam, which is a 20-year-old man who presented with exercise intolerance, myalgia, and recurrent rhabdomyolysis triggered by fasting and exertion. Acylcarnitine profiling suggested a fatty ac

Personalized Circulating Tumor DNA Profiling Enables Superior and Universal Residual Disease Detection in Acute Myeloid Leukemia.

Relapsed and/or refractory disease remains the leading cause of death in AML, highlighting the need for broadly applicable, high-sensitivity approaches to MRD detection. We developed AML-CAPP-Seq (Cancer Personalized Profiling by Deep Sequencing), a personalized hybrid-capture assay that tracks both canonical AML drivers and patient-specific variants identified by whole-exome sequencing. In 56 patients with longitudinal plasma and matched peripheral blood and bone marrow samples, AML-CAPP-Seq enabled universal MRD assessment and resolution of clonal dynamics using a median of 30.5 variants per

[A case of Berry syndrome associated with SOX11-related Coffin-Siris syndrome type 9].

A male infant was admitted to the neonatal intensive care unit for respiratory distress 12 minutes after preterm birth. Prenatal fetal echocardiography suggested transposition of the great arteries with an intact ventricular septum. Amniocentesis and family-based whole-exome sequencing identified a heterozygous missense variant in SOX11 (c.152G>C, p.Arg51Pro), leading to a diagnosis of Coffin-Siris syndrome type 9. Postnatal echocardiography and computed tomography angiography confirmed Berry syndrome. Single-stage corrective surgery was performed with good recovery. This report presents the f

Rare variants in FAT3 as possible contributors to non-syndromic orofacial cleft risk.

Non-syndromic orofacial clefts (NSOFCs) represent the most common human craniofacial malformations, yet the majority of their genetic causes remain unclear. This study, by analyzing whole-exome sequencing (WES) data from multiple patients with NSOFCs, identified eight de novo low-frequency missense variants in the FAT3 gene, all of which were located within the extracellular cadherin domains of the FAT3 protein. Sanger sequencing confirmed the authenticity of these variants, while bioinformatic predictions and protein structural modeling suggested potential disruption of FAT3 function.Using in

Case Report: Rare malignant teratoma in the right posterior thigh.

Malignant teratomas are rare germ cell tumors characterized by undifferentiated embryonic components, exhibiting invasive and metastatic potential. While teratomas commonly arise in the gonads, such as the ovaries and testes, as well as midline locations like the sacrococcygeal region and mediastinum, the occurrence of a malignant teratoma in the femur (right femur) is exceedingly rare, with limited research available. We report the case of a 19-year-old female presented with a subcutaneous mass in the right posterior thigh for 2 months, which was later confirmed as immature teratoma (CK5/6+,

ABCA7 Mutation in Behavioral Variant of Frontotemporal Dementia: A Case Report.

INTRODUCTION: Frontotemporal lobar degeneration (FTLD), a major cause of early-onset dementia, includes a heterogeneous group of neurodegenerative disorders with a strong genetic component. Mutations in MAPT, GRN, and C9orf72 are found in about 40% of patients with the behavioral variant (bvFTD). More recently, rarer pathogenic variants have been identified in other genes, such as ABCA7, initially linked to Alzheimer's disease but increasingly implicated in other neurodegenerative conditions. Here we describe a specific variant which has not previously been reported in the literature. CASE PR

A Combination of Variants in SEPTIN9 and MSX1 Genes Leads to the Formation of Orofacial Clefts.

Nonsyndromic cleft lip with or without palate (nCL/P) is a common congenital anomaly with a complex genetic basis. Previous whole-exome sequencing of Malagasy case-parent trios identified variants in several craniofacial development genes, including SEPTIN9, SKI, WNT5B, GPC4, and MSX1, enriched on East Asian ancestry segments. To assess functional contributions, we used Xenopus laevis embryos to evaluate the effects of individual and combined perturbations of candidate genes. Neither single-gene depletion nor ectopic expression of candidate genes induced orofacial clefts, although some craniof

Hereditary neurodegenerative disorders with spastic-ataxic manifestations: genetic and clinical insights into previously reported variants.

Hereditary neurodegenerative disorders comprise a heterogeneous group of genetic conditions that affect the central and/or peripheral nervous system, often presenting with hallmark clinical features including ataxia, spasticity, dystonia and neuropathy. Our study aims to investigate the underlying cause of rare and overlapping phenotypes of inherited neurodegenerative disorders in the four familial cases of the Pakistani population. Four Pakistani families with a wide range of spastic-ataxic features were evaluated using exome sequencing, homozygosity mapping, and Sanger sequencing. These mole
