Adaptive reconfiguration of prefrontal networks during prolonged cognitive interference: Evidence from fNIRS.

In the era of information overload, understanding the brain's adaptive responses to prolonged cognitive tasks is critical. This study investigates the neural compensatory mechanisms that sustain cognitive performance under mental fatigue, offering insights into dynamic resource allocation and practical applications in high-demand settings. Twenty healthy participants performed a Stroop-based cognitive interference task while prefrontal hemodynamic activity was monitored using functional near-infrared spectroscopy (fNIRS). Subjective fatigue was assessed via the Multidimensional Fatigue Invento…

Assessment of Disease Burden Among Japanese Patients With Pulmonary Sarcoidosis: A Questionnaire-Based Study.

INTRODUCTION: Sarcoidosis is a systemic granulomatous disease that can form lesions in almost all organs and most commonly affects the lung. It often causes systemic symptoms such as generalized pain and fatigue, leading to reduced health-related quality of life (HRQoL). Although oral corticosteroids (OCS) are commonly used to manage sarcoidosis, they are associated with side effects and reduced HRQoL. In Japan, data on patient-reported outcomes remain limited. METHODS: This study assessed the disease burden in 158 Japanese patients with pulmonary sarcoidosis, recruited through a sarcoidosis …

Effects of an 18-week exercise programme started early during breast cancer treatment: a randomised controlled trial.

BACKGROUND: Exercise started shortly after breast cancer diagnosis might prevent or diminish fatigue complaints. The Physical Activity during Cancer Treatment (PACT) study was designed to primarily examine the effects of an 18-week exercise intervention, offered in the daily clinical practice setting and starting within 6 weeks after diagnosis, on preventing an increase in fatigue. METHODS: This multi-centre controlled trial randomly assigned 204 breast cancer patients to usual care (n = 102) or supervised aerobic and resistance exercise (n = 102). By design, all patients received chemotherap…

Experimental and Numerical Investigation on Fe-SMA Strengthening of U-Rib Butt-Welded Joints with Porosity Defects.

To investigate the influence of porosity defects and the strengthening effect of bonded iron-based shape memory alloy (Fe-SMA) plates, fatigue tests were conducted on defect-free, porosity-containing, and Fe-SMA-strengthened U-rib butt-welded specimens. A numerical model considering porosity defects and the bonded Fe-SMA plate was also established and validated against the experimental results. The results show that porosity defects significantly increased the local stress level near the crack. Under a load of 60 kN, the stress at the section 2 mm from the crack edge increased from 98 MPa to 1…

Obstructive Sleep Apnea as a Modifiable Contributor to Chronic Pain in Rheumatologic Disease: The Potential Role of Continuous Positive Airway Pressure Therapy.

Obstructive sleep apnea (OSA) is a common sleep-related breathing disorder marked by recurrent episodes of upper-airway obstruction, leading to intermittent hypoxia, recurrent arousals, and fragmented sleep. It is highly prevalent worldwide and remains underdiagnosed, particularly when symptoms such as fatigue, poor concentration, and nonrestorative sleep overlap with obesity, chronic pain, depression, or inflammatory disease. These diagnostic challenges are clinically relevant in rheumatology, where pain, fatigue, poor sleep, and immune activation frequently coexist. This narrative review exa…

Gingival Creep Failure: A Viscoelastic Theory of Recession in Thin Periodontal Phenotypes.

Gingival recession is commonly linked to alveolar bone dehiscence, inflammatory burden, traumatic brushing, or excessive orthodontic forces. However, recession is also observed in some patients despite apparently mild or "biologically acceptable" loading, particularly in thin periodontal phenotypes. Here, we propose the Gingival Creep Failure Theory, a hypothesis-driven conceptual framework in which gingival soft tissues undergo time-dependent viscoelastic deformation (creep) under sustained or repetitive tensile microstrain. Over time, accumulated deformation and microstructural fatigue may r…

Iron deficiency-related symptoms in non-anemic whole blood donors.

INTRODUCTION: Whole blood donors are at increased risk for iron deficiency (ID). ID anemia is associated with several symptoms, such as fatigue, cognitive dysfunction, pica, and restless leg syndrome (RLS). However, it is unclear if these symptoms also occur when a donor has developed ID without anemia. This study aims to determine whether non-anemic ID (NAID) is associated with the occurrence of ID-related symptoms. STUDY DESIGNS AND METHODS: We combined data from three studies in whole blood donors (i.e., Donor Insight-III, FIND'EM, and FORTE) to create a substantial sample size (N = 12,143…
