A 51-year-old male with a history of a mechanical Carbomedics aortic and mitral valve replacement in 2003 and several re-operations because of endocarditis of the mitral valve in 2007 presented with heart failure 68 days after operation. Echocardiography confirmed the presence of a fistulous connection between the aorta and the left atrium. Because of the multiple surgical interventions and high operative risk, an initial conservative medical treatment was initiated and the clinical course was uneventful to this date.

Aorto-oesophageal fistula is a well-reported pathology with several known causes. The co-existence of this pathology associated with the use of a Montgomery salivary bypass tube (MSBT) is exceptional and only one case is described in the literature. We present here a case report about an 81-year-old patient with an MSBT who died because of a massive upper gastrointestinal bleeding caused by an aorto-oesophageal fistula at the site of the MSBT. The literature on this pathology will also be reviewed.

An exceptional case of tracheal agenesis with no communication with the esophagus is described. This malformation needs surgical airway approach and is hardly classifiable. A 61-year-old woman presented with recent history of cholangitis. On evaluation, she was found to have a type I choledochal cyst and a cholecystoduodenal fistula. She underwent excision of the choledochal cyst and disconnection of the fistula. Tissue interposition in vesicovaginal repair is associated with better cure rates especially in failed repairs. Commonly used tissues include the labial fat pad in transvaginal approach and vascularized omental, peritoneal flaps or ileal patch in transabdominal approach. We report a case of a failed vesicovaginal fistula buttressed with a gluteal cutaneous flap.

In utilizing free-tissue transfers to reconstruct complex wounds, occasionally a mismatch exists between the donor-flap inflow and the recipient vessel outflow. This occurs more often with fasciocutaneous flaps as opposed to muscle flaps due to their higher resistance. Colosplenic fistula associated to splenic abscesses is a rare condition in Crohn's disease (CD). Diagnosis is difficult and a computed tomography (CT) scan is helpful. Treatment includes surgical procedure. Atypical locations of fistulas and abscesses should not be overlooked in CD.

The posterior intravaginal sling (IVS) is a device used to correct apical vaginal prolapse; there are limited data on associated complications. We present a case report of a rectocutaneous fistula forming after placement of a posterior IVS in a 59-year-old woman 2 months postoperatively. The sling was resected during sigmoidoscopy with scissors, and the colon laceration was closed. The buttock wound underwent delayed healing 2 weeks later. More data regarding complications associated with use of this device are needed before widespread use.

We report a rare case of pancreaticobronchial fistula caused by chronic pancreatitis. A 46-year-old man with a history of chronic alcoholic pancreatitis was referred to us for investigation of dyspnea and bloody sputum. Chest radiography showed a bilateral pneumonia-like shadow, with severe atelectasis in the left lower lung field.
