Fever should be treated according to the individual situation. Securing a sufficient fluid intake is the most important primary measure. Hemodialysis with a low-bicarbonate dialysate was used in four patients for the simultaneous treatment of acute renal failure and fluid overload associated with alkalemia. All patients had a mixed acid-base disorder, namely respiratory alkalosis superimposed on metabolic alkalosis. Cryptococcal meningitis and a cerebral cryptococcoma developed in a patient with pulmonary cryptococcosis ("coin lesion") shortly after infected area of lung was removed. Treatment with amphotericin B and flucytosine failed to clear the organisms and antigen from the spinal fluid or alter the neurologic manifestations related to the cerebral lesion. In this study, 25 men, referred to our clinic for diagnosis and therapy of infertility were included. All had enlarged prostates. They were given 10 sessions of prostatic massage during 3--4 weeks and the fluid expressed was analysed for citric acid. The hypertrophy was seen to recede in almost all cases. Citric acid concentrations fell in only 6/25 cases analysed. In all the others, values did not fall and remained relatively stable. There was no apparent relationship between reduction of prostatic volume and the pattern of citric acid secretion.

Cerebrospinal fluid specimens from patients with tuberculous (17 cases), cryptococcal (15 cases), and viral (14 cases) meningitis were analyzed by frequency-pulsed electron capture gas-liquid chromatography and mass spectrometry. An 18-month-old boy developed meningitis due to group A beta-hemolytic Streptococcus. Despite adequate antibiotic therapy with aqueous sodium penicillin G and chloramphenicol for more than eight weeks, he showed persistent abnormal spinal fluid findings, recurrent elevations of temperature, irritability, and poor feeding. Further investigation revealed a congenital dermal sinus of the nose, with an entry into the anterior fossa that made a small opening intradurally at the level of the cribiform plate. Surgical removal of the entire sinus tract resulted in a speedy and complete recovery.

A study conducted on seventy-five cases of chronic maxillary sinusitis presenting at the E.N.T. Outpatients' Department of Dayanand Medical College, Ludhiana revealed the presence of fungi in 8 cases. The return fluids of proof puncture and antral lavage of the sinuses were subjected to standard extraction procedures. Methods of isolation and identification of the causal fungi are described. The role of fungi, particularly Aspergillus, as pathogen is discussed in the context of antibiotic and immuno-suppressive therapy and local predisposing factors.
