Foot care education in patients with diabetes at low risk of complications: a consensus statement.

AIMS: To define and agree a practical educational framework for delivery by all healthcare professionals managing patients with diabetes, particularly those at low risk of developing foot complications. METHODS: A consensus meeting of a multidisciplinary expert panel. Prior to the meeting, relevant clinical papers were disseminated to the panel for review. The consensus was largely based upon the experts' clinical experience and judgement. RESULTS: Four main health behaviours were identified for those at low risk of developing foot complications, namely: control of blood glucose levels; atte…

The role of certified foot and nail care nurses in the prevention of lower extremity amputation.

The primary purpose of this article is to outline the role, opportunities, and benefits of the certified foot and nail care nurse (CFCN). Current demographic trends toward an aging population and associated health issues such as diabetes mellitus, arthritis, peripheral arterial disease, dementia, and loss of visual acuity hinder self-care of the feet and nails. The resulting foot and nail care needs create opportunities for CFCN to promote health and prevent adverse outcomes such as infection and limb loss. Routine foot and nail assessment, clinically appropriate care, and individualized patie…

The epidemiology of foot wounds in patients with diabetes: a description of 600 consecutive patients in Turkey.

Epidemiological studies describing demographic, clinical, and surgical characteristics of diabetic foot wounds are lacking in Turkey. To further describe the epidemiology of diabetic foot wounds in Turkey, we developed an evaluation form and performed a retrospective cohort study that entailed 600 diabetic patients who were admitted to the hospital for treatment of their foot wound(s). The mean age of the cohort was 62.3 ± 10.3 (range 23-92) years, and their mean duration of diabetes was 17.4 ± 7.4 years. Males accounted for 68.17% of the cohort, and 96.83% of the cohort had type 2 diabetes. P…

TUNEL-positive cells in the surgical border of an amputation due to infected diabetic foot.

Diabetic infected foot is the outcome of progressive vascular and neurological damage caused by persistent chronic hyperglycemia. Due to acute hypoxia and infection, the tissues develop extensive necrosis and gangrene, which often require amputation. The decision regarding the level of amputation relies mainly on the personal experience of the surgeon who must identify the healthy tissue without necrosis. However, tissue cells under stress may succumb before clear evidence of necrosis is present. In this study, dying cells with DNA damage were identified in the necrotic lesions and surgical bo…

Late migration of a foreign body in the foot 5 years after initial trauma.

An 11-year-old boy presented to our emergency department because of the extrusion of a small piece of glass through the plantar skin, 5 years after the initial trauma. Another foreign body was identified on ultrasounds and removed surgically. Ultrasound is an accurate test for detection of foreign bodies and to assess potential complications. The treatment of all foreign bodies is surgical exploration and removal, except if the risks of attempting removal are higher than the potential damage of the foreign body in its existing location. Migrations of foreign bodies are not uncommon. Ultrasound

The symptomatic os vesalianum as an uncommon cause of lateral foot pain: a case report.

The os vesalianum is an uncommon pedal accessory bone located lateral to the fifth metatarsal base. It may occasionally become symptomatic and require surgical excision, as in the case reported here in a 24-year-old woman. Simple excision of the ossicle, while effective in the present case, can be complicated by the attachment of fibers of the peroneus brevis tendon into the ossicle, thus requiring careful tenorraphy and sometimes tenodesis.

Flexible and rigid casting tape as a novel approach to offloading diabetic foot ulcers.

Offloading diabetic ulceration is a key component to the success in healing ulcers on the plantar aspect of the foot. New advances in offloading techniques allow for differing approaches in sometimes complex diabetic foot pathologies with associated ulceration. This case study looks at the use of flexible and rigid casting technique as part of the treatment in offloading plantar foot ulceration.

[Genotype-phenotype analysis of a Chinese family with split hand/split foot and syndactyly].

OBJECTIVE: To determine the causative gene mutation in a Chinese family with split hand/split foot malformation (SHFM) and explore the genotype-phenotype relationship. METHODS: Genomic DNA was extracted from peripheral blood samples of the patients and their family members. Polymerase chain reaction (PCR) was performed to amplify all the exons of P63 gene and HOXD13 gene. Then the PCR products were sequenced bidirectionally to screen mutations. RESULTS: A heterozygous 956G>A transversion in exon 7 of P63 gene was identified in all patients, which resulted in the substitution of histidine res…

Free-flap resurfacing of tissue defects in the foot due to large gouty tophi.

BACKGROUND: Large or extensive gouty tophi on the feet can cause functional impairment, drainage sinus, and infected necrosis, finally resulting in complex soft-tissue defects with tendon, joint, bone, nerve, and vessel exposure. Reconstruction of complex soft-tissue defects of the foot is still challenging. The purpose of this report was to review the outcomes of free-flap reconstructive surgery for treating the metatarsal joint defects of the feet caused by chronic tophaceous gout. METHODS: Ten patients who had large tophus masses (>5 cm) and ulceration on the feet were admitted to our hosp…

A case series to describe the clinical characteristics of foot ulceration in patients with rheumatoid arthritis.

The aim of this study was to describe the clinical characteristics of foot ulceration in patients with rheumatoid arthritis (RA). Adults with RA and current foot ulceration but without diabetes were recruited. Clinical examination included assessment of RA disease activity, foot deformity, peripheral vascular disease, neuropathy and plantar pressures. Location, wound characteristics and time to healing were recorded for each ulcer. Participants completed the Health Assessment Questionnaire and Leeds Foot Impact Scale. Thirty-two cases with 52 current ulcers were recruited. Thirteen patients (4…

[Prevalence of foot conditions in a geriatric population and their impact on mobility, gait and tendency to falls].

INTRODUCTION: Foot disease is common among the elderly. Little is known about its prevalence and impact over mobility, gait and tendency to fall. MATERIAL AND METHODS: A geriatric history was taken from 171 women living in a long-term care facility. Photographs were taken of the feet and evaluated by an orthopaedic surgeon and a dermatologist. A multivariate analysis was made to assess de effect of the independent variables over mobility, gait and tendency to fall. RESULTS: The foot diseases most commonly found were hammer toes (122), callus (79) and peripheral vascular disease (74). Hallux …

Case report: Dot-in-circle sign - An MRI and USG sign for "Madura foot".

Mycetoma is a chronic granulomatous disease that is more common in tropical than in temperate regions. Early diagnosis is important due to the therapeutic implications. Although biopsy and microbiological culture provide definitive diagnosis, they are time-consuming procedures and may not be able to provide a definite diagnosis in cases of fastidious organisms. The "dot-in-circle" sign has recently been proposed as a highly specific magnetic resonance imaging (MRI) and ultrasonography (USG) sign of mycetoma, which may allow a noninvasive as well as early diagnosis. We present a case of histolo

Gas gangrene and osteomyelitis of the foot in a diabetic patient treated with tea tree oil.

Diabetic foot wounds represent a class of chronic non-healing wounds that can lead to the development of soft tissue infections and osteomyelitis. We reviewed the case of a 44-year-old female with a diabetic foot wound who developed gas gangrene while treating her wound with tea tree oil, a naturally derived antibiotic agent. This case report includes images that represent clinical examination and x-ray findings of a patient who required broad-spectrum antibiotics and emergent surgical consultation. Emergency Department (ED) detection of these complications may prevent loss of life or limb in …

Development of a practical guide for the early recognition for malignant melanoma of the foot and nail unit.

BACKGROUND: Malignant melanoma is a rare but potentially lethal form of cancer which may arise on the foot. Evidence suggests that due to misdiagnosis and later recognition, foot melanoma has a poorer prognosis than cutaneous melanoma elsewhere. METHODS: A panel of experts representing podiatry and dermatologists with a special interest in skin oncology was assembled to review the literature and clinical evidence to develop a clinical guide for the early recognition of plantar and nail unit melanoma. RESULTS: A systematic review of the literature revealed little high quality data to inform t…

[Liver donors no longer foot the bill Compensation for medical costs and loss of income].

Organ donation is at the centre of medical and societal attention. An important reason for this is the shortage of donors and thus organs. One of these shortages concerns cadaveric-donor livers. The alternative is living-donor liver transplantation. Until recently, the donors' healthcare costs and loss of income were impediments to living-donor liver transplantation. However, the Dutch government has now removed these obstacles, on the one hand by covering the medical costs associated with the donation, the travelling costs of the donor and a companion, and on the other hand by a subsidy to co

High-pressure water injection of the foot with associated subcutaneous emphysema: a case report.

A review of the literature yields few reported cases of industrial-strength high-pressure water injection injuries involving the foot, and sources of information for industrial-strength high-pressure water injection injuries reside primarily in the hand surgical literature. Toxic materials, such as grease, paint, and diesel oil, are relatively common agents involved in injection injuries, and these substances are associated with increased morbidity in comparison with injection of water or air. Treatment of high-pressure injection of a toxic substance is a surgical emergency requiring irrigatio…
